A Rare Case of Simultaneous Fracture Neck of Femur on One Side and Contralateral Intertrochanteric Fracture Femur in a 41-Year-Old Female – A Case Report

Simultaneous fractures in the neck of femur on the one side and contralateral intertrochanteric fracture of the femur with only trivial injury are among rarest injuries. Fracture neck of femur or intertrochanteric fracture either isolated or in combination such as simultaneous bilateral fracture neck of femur and simultaneous bilateral intertrochanteric fractures are relatively commonly reported in literature. Herein, we report a very rare case of a young female with chronic kidney disease who presented with simultaneous fractures neck of femur on the one side and contralateral intertrochanteric fracture of femur after a fall from standing height.A 41-year-old female with chronic kidney disease from the past 5 years presented to us with severe pain at both hip and inability to stand after a fall from standing height. Clinical examination and investigations were done. She suffered fracture neck of femur on the right side and intertrochanteric fracture femur on the left side. Single stage fixation of both these fractures by two different methods was done successfully after optimization of her medical condition with multidisciplinary approach. She was advised weight bearing according to fixation method used and progress of fracture union. She regained her preoperative walking status gradually in 6 months.Simultaneous fractures in the neck of femur on the one side and intertrochanteric fracture of the femur on the other side are very rare presentation and can happen in patients with primary or secondary bone disease. Multidisciplinary team effort is needed for overall effective management and prompt surgical treatment can help achieve favorable outcome.

Effect of collagen fibre orientation on the Poisson's ratio and stress relaxation of skin: an ex vivo and in vivo study

During surgical treatment skin undergoes extensive deformation, hence it must be able to withstand large mechanical stresses without damage. Therefore, understanding the mechanical properties of skin becomes important. A detailed investigation on the relationship between the three-dimensional deformation response of skin and its microstructure is conducted in the current study. This study also discloses the underlying science of skin viscoelasticity. Deformation response of skin is captured using digital image correlation, whereas micro-CT, scanning electron microscopy and atomic force microscopy are used for microstructure analysis. Skin shows a large lateral contraction and expansion (auxeticity) when stretched parallel and perpendicular to the skin tension lines, respectively. Large lateral contraction is a result of fluid exudation from the tissue, while large rotation of the stiff collagen fibres in the loading direction explains the skin auxeticity. During stress relaxation, lateral contraction and fluid effluxion from skin reveal that tissue volume loss is the intrinsic science of skin viscoelasticity. Furthermore, the results obtained from in vivo study on human skin show the relevance of the ex vivo study to physiological conditions and stretching of the skin during its treatments.

Numerical Investigation of Stable Crack Growth in Ductile Materials Using XFEM

In the present work, extended finite element method (XFEM) has been extended to simulate nonlinear stable crack growth problems. In XFEM, the cracks are modeled by adding enrichment functions into standard finite element approximation. The modeling of large deformations is done using updated Lagrangian approach. Von-Mises yield criterion has been used along with isotropic hardening to check the plasticity. Elastic-predictor and plastic-corrector algorithm has been used for the computation of stress fields. The nonlinear equations are solved by Newton-Raphson iterative scheme. Two problems (crack growth in compact tension and triple point bend specimens) are solved using J-R curve to show the capability of XFEM in modeling large deformation crack growth problems.
