Investigation of a fatality among parathion applicators in California

Abstract Staff of the California Department of Health Services investigated the death of a parathion applicator in California that was thought to be pesticide‐related. A crew of eleven workers (including six sprayers, three mixeriloaders, one mixerisprayer and one foreman) had been applying 0.125% parathion spray to almond orchards for approximately three weeks. On the day of the fatality, a sprayer rapidly developed symptoms of salivation, sweating, and convulsions after a half‐day of work. Despite aggressive medical treatment, the worker died within one hour of his initial symptoms. Significant laboratory results for the deceased case included: parathion residue on the inner and outer garments worn by the worker, parathion in the post‐mortem gastric contents, and elevated urinary metabolites consistent with acute parathion intoxication. Interviews with the work crew revealed that three of the 10 workers had complaints of headaches, vomiting, and/or sore throat; yet, subsequent plasma and red cell cholinesterase tests of the co‐workers did not show any significant depressions in comparison with pre‐season baseline values. This worker death is consistent with prior reports of parathion‐related sprayer/applicator intoxications and is the first worker‐related parathion death in California since 1972. Substitution of pesticides with less toxic active ingredients or the elimination of parathion is recommended.

Feeding Method Does Not Affect Mortality of Infants of HIV-Infected Women

A sample of HIV-infected women were recruited from antenatal clinics in Nairobi Kenya between 1992 and 1998 to compare the effects of breastfeeding and formula-feeding on mortality morbidity and nutrition among infants. 425 HIV- infected women participated in the study: 212 were assigned to breastfeed and 213 to formula-feed. Overall the study showed that of infants born to HIV- infected women those assigned to be formula-fed were no more likely than those assigned to be breastfed to die before their second birthday even when infection with HIV was taken into account. However HIV-free survival at 2 years was significantly more frequent among infants in the formula-fed group than among those in the breastfeeding group. It notes that diarrhea pneumonia and malnutrition were the most common causes of death but none occurred more often in one feeding group than in the other. The researchers conclude that the use of formula to prevent HIV-1 transmission can be a viable option even in resource poor settings if maternal education clean water a supply of formula and access to health care are available.

Substantial Variation in Hospital Rankings after Adjusting for Hospital-Level Predictors of Publicly-Reported Hospital-Associated Clostridium difficile Infection Rates

Across 366 California hospitals, we identified hospital-level characteristics predicting increased hospital-associated Clostridium difficile infection (HA-CDI) rates including more licensed beds, teaching and long-term acute care (LTAC) hospitals, and polymerase chain reaction testing. Clinical Reasoning: An Older Woman With Headaches and Lethargy After a Fall

In this case, a 77-year-old woman presented with generalized weakness, difficulty ambulating, lethargy, loss of appetite, and headaches after a mechanical fall. This case discusses the management of acute neurologic emergencies such as subdural hematoma, status epilepticus, and bacterial meningitis. Potential etiologies for stroke and CNS infection are highlighted. Readers are led through the diagnostic approach to a patient presenting with a complex array of neurologic symptoms causing rapid clinical decompensation.

Enhanced Control of an Outbreak ofMycoplasma pneumoniaePneumonia with Azithromycin Prophylaxis

There are currently no recommended epidemic-control measures for Mycoplasma pneumoniae pneumonia outbreaks in closed communities. Previous studies have suggested the usefulness of chemoprophylaxis administered to close contacts of case-patients. To evaluate the effectiveness of various epidemic-control measures during an institutional outbreak, an observational study was undertaken during a very large outbreak of M. pneumoniae pneumonia at a facility for developmentally disabled residents (n = 142 cases). Control measures evaluated included no control, standard epidemic-control measures, and targeted azithromycin prophylaxis (500 mg on day 1, 250 mg/day on days 2-5) plus standard epidemic-control measures. The combined use of azithromycin prophylaxis and standard epidemic-control measures was associated with a significant reduction in the secondary attack rate. This study suggests that the addition of antibiotic prophylaxis to standard epidemic-control measures can be useful during institutional outbreaks of M. pneumoniae pneumonia.
