Before infiltrative anesthesia could be performed effectively, an appropriate delivery device had to be invented and an effective local anesthetic had to be isolated. Interestingly, it was the ancient Javanese practice of using darts to deliver poison that provided the first experimental model for subcutaneous infiltration. Infiltrative anesthesia is the intradermal or subcutaneous infiltration of an anesthetic either directly into the surgical site or circumferentially around the surgical field. Infiltration of an anesthetic directly below a lesion creates a small ring of anesthesia around its base that is sufficient to obtain a shave or punch biopsy. The atmosphere in the surgical suite should be calm and soothing. The room should be organized to avoid last-minute running around for instruments and kept at a comfortable temperature. Personnel should be unhurried, courteous, and professional. Local anesthetic solutions exist in water-soluble ionized and lipid-soluble non-ionized form. Alcohol is an excellent agent to cleanse the skin before infiltration of an anesthetic.

Laser Treatment of Facial Vascular Lesions

For years, the traditional 577 to 585-nm pulsed dye laser has provided safe and effective treatment for many facial vascular lesions. Currently, interest is focused on how selective epidermal cooling, longer wavelengths, and varied pulse durations may expand the pulsed dye laser's therapeutic range to include treatment-resistant port wine stains, nonablative wrinkle reduction, scar revision, and nonsurgical flap delay. Despite these manipulations, the generation of objectionable purpura remains a concern to patients with limited lesions. To this effect, a long pulsed 532-nm laser has recently become available. This laser is effective for the treatment of facial telangiectasias and does not cause any purpura. It has had minimal complications in two series.

Dermatologic Surgery Training in Residency

BACKGROUND Dermatologic surgery is an integral part of dermatology residency training. OBJECTIVE To characterize the current level of surgical training in dermatology residency programs. METHODS A survey was mailed to the 112 dermatology residency programs in North America and Puerto Rico. RESULTS A total of 71 residency programs (63%) responded. All programs offer training in surgical excisions, 97% of programs train residents in at least one cosmetic procedure, 92% of programs offer Mohs micrographic surgery, and 90% of programs train their residents in laser surgery. CONCLUSION Surgical training in dermatology residency varies widely, with a trend toward more cosmetic procedures when compared with prior studies.
