Application of immunohistochemical markers in diagnostics of various histological types of pleomorphic adenomas of the salivary glands

The morphological verification of tumors as a diagnostic method has been the basis for establishing the final diagnosis for a long time. According to various data, the accuracy of the diagnosis ranges from 80–85 %, depending on the type of tumor (benign or malignant) and its differentiation. With the introduction of immunohistochemical studies for the final diagnosis of tumors, the percentage of verified tumors has increased significantly. The aim of the study – determination of peculiarities of pathogenesis of various histological types of pleomorphic adenomas of the salivary glands applying the methods of immunohistochemical diagnostics. Materials and Methods. The study was conducted based on the excisional biopsies of the pleomorphic adenomas of the salivary glands in 28 patients. The immunohistochemistry analyses were performed with Monoclonal Mouse Anti-Epstein Barr Virus, Polyclonal Rabbit Antibody p16, Monoclonal Rabbit Anti-Human Estrogen Receptor (ER), and the Mouse Anti-Human Progesterone Receptor (PR), PLAG1 (pleomorphic adenoma gene 1) Monoclonal Antibody. Results and Discussion. The results of the conducted study of the own surgical material show that the structure of histological types of pleomorphic adenomas of the salivary glands is as follows: mesenchymal 15 (53.57 %), epithelial-mesenchymal – 10 (35.47 %), epithelial – 3 (10.71 %). The mesenchymal and epithelial-mesenchymal types of histological structure of pleomorphic adenomas of the salivary glands are characterized by the similar immunohistochemical peculiarities (Plag1 – positive response in tumor cells of both histological types in 100 % of cases; HPV 16 – 73.33 % and 80.0 % respectively and EBV – 33.33 % and 30.0 % respectively). The epithelial type of structure of the pleomorphic adenomas of the salivary glands is characterized by slightly different immunohistochemical pattern: Plag1 – positive response in tumor cells in 66.66 % of cases; HPV 16 – also in 66.66 % of cases. The mesenchymal and epithelial-mesenchymal types of histological structure of pleomorphic adenomas of the salivary glands are characterized by more frequent invasion of HPV 16 and EBV. Conclusions. The mesenchymal and epithelial-mesenchymal types of histological structure of pleomorphic adenomas of the salivary glands are characterized by more frequent invasion of HPV 16 and EBV.

EXPRESSION PROFILE OF miR-145, -182, -21, -27a, -29b, and -34a IN BREAST CANCER PATIENTS OF YOUNG AGE

Background. Breast cancer (BC) in young women remains a significant public health concern. While progress has been made in understanding the etiology, diagnosis, and treatment of BC in this population, challenges persist. The identification and utilization of prognostic biomarkers offer valuable tools for tailoring treatment strategies and improving outcomes for BC patients. Aim. To evaluate the relationship between the expression of tumor-associated microRNAs and the clinical and pathological features of BC in young patients. Materials and Methods. The work is based on the results of the examination and treatment of 50 women younger than 45 years with stage I—II BC. miR-145, -182, -21, -27a, -29b, and -34a expression in tumor samples was analyzed by the real-time reverse transcription polymerase chain reaction. Results. Higher expression of miR-182, -21, and -29b and lower levels of miR-27a were associated with tumor stage in young BC patients. Patients without lymph node metastases (N0) had significantly higher levels of miR-182, -27a, and -34a and lower levels of miR-29b compared to N1 cases (p < 0.05). Expression of miR-145, -182, -21, -27a, and -29b was associated with molecular BC subtypes. Conclusion. Obtained results show that a high malignancy degree of BC in young women is associated with an increase in the miR-182, -21, -29b, and -34a expressions and a decrease in the miR-27a level in the tumor tissue, which indicates the prospects of the use of them for predicting the aggressiveness of the disease.

Predictive factors of pathological response to neoadjuvant chemotherapy in patients with breast cancer.

PURPOSE To identify predictive factors connected with pathologic response in patients with breast cancer (BC) having received neoadjuvant chemotherapy (NACT). METHODS 49 patients with BC were investigated before and after treatment in this prospective research. Different chemotherapy regimes were administered. The Miller-Payne scoring system was used to assess the tumour response. The nuclear proliferation markers Ki67 and the expression of topoisomerase IIα (Topo IIα) were evaluated. RESULTS Six patients (12.2 %) achieved pathological complete response (pCR). Noticeable decrease of tumor cellularity was detected in all BC subtypes and pCR in the triple-negative BC (TNBC) group (p=0.007) was observed. Poorly differentiated tumors could be considered as predictive factors of pCR (p=0.07). Ki67 appeared to be a predictive marker of achieving pCR (p<0.001) with a threshold of 28% (AUC=0.89, 95% CI 0.75-0.96). The additional factor of reaching pCR was operable BC (p=0.04). The expression level of Topo IIα (p=0.50) and using different regimens of NACT (p=0.97) did not influence pCR achievement. CONCLUSION To sum it up, poorly differentiated carcinomas with high cellularity in the primary tumor, TNBC, Ki 67 with a threshold above 28% and operable BC can be considered as early predictors of reaching pCR.

Distribution of lymphoma subtypes in Ukraine according to the WHO 2016 classification

The Ukrainian Lymphoma Registry (ULR) was established in 2019 with the aim of monitoring the quality of diagnosis, staging, and treatment of lymphoma in Ukraine. Between September 2019 and October 2021, 546 patients with newly diagnosed lymphoma were prospectively registered. All cases were diagnosed according to the 2016 updated WHO lymphoma classification. The male-to-female ratio (M/F) for the whole population was 0.7, with a median age of 46 years (range 18-95). The adoption of the 2016 WHO classification resulted in the identification of 36 different lymphoma subtypes, with 132 cases (24.2%) classified differently compared to the 2008 WHO classification. Only 12 cases (2.8%) were true new entities, including seven cases of high-grade B-cell lymphoma NOS, three of anaplastic large B-cell lymphoma, ALK-negative, 1 case of HHV8+ DLBCL NOS, and 1 of high-grade B-cell lymphoma with C-MYC and BCL2/BCL6 rearrangement. Moreover, 55 (61.1%) entities, including 37 defined by WHO 2008 and 18 defined by WHO 2016, were not represented at all. The analysis of cases registered in the ULR provides a comprehensive breakdown of the subtypes, stage distribution, and treatment of malignant lymphomas (ML) in Ukraine, supporting the usefulness of prospective data collection and timely reporting. We believe that this study is the first step toward a better understanding of the real-life outcomes of patients with ML.

CLINICAL AND MORPHOLOGICAL FEATURES OF BONE METASTASES DIAGNOSIS IN PATIENTS WITH PROSTATE CANCER

Summary. Aim: to highlight the current clinical, laboratory and morphological methods of prediction and diagnosis of bone metastases (BM) in the prostate cancer (PC) patients. Objects and methods: in the prospective non randomized clinical study were included 521 patients with localized PCR, that underwent radical prostatectomy (RP) and were observed until the development of BM. Main group included 411 (78.8%) patients with Grade ОРИГІНАЛЬНІ ДОСЛІДЖЕННЯ 6 8 ОНКОЛОГІЯ • Т. 25 • № 1 • 2023 Одержано: 20.03.2023 group (GrG) 2–5, control group — 110 (21.2%) patients with GrG 1 that hadn’t progression of the disease with development of BM. We evaluated all relevant clinical, laboratory, morphology indicators and predictors of PCR BM development. Every 3 months postoperatively, the levels of total PSA, calcium, and alkaline phosphatase (ALP) in blood serum were determined. Results: according to the data of the initial levels of total PSA in all studied groups, the average levels in GrG 2,3 and GrG 4,5 without the development of BM were 10.6 and 21.6 ng/ml; in GrG 2,3 and GrG 4,5 with the development of BM — 19.8 and 25.7 ng/ml. Among 411 PCR patients with GrG 2–5, perineural invasion (PNI) was detected in postoperative material in 197 patients, among whom 25 (12.4%) developed BM as opposed to 12 (5.9%) patients with BM without PNI. In patients with the development of CM, the level of LF activity in blood serum is almost twice as high as in patients without metastases, regardless of the initial PSA values. LF, calcium, and hemoglobin levels are sensitive laboratory markers of CM development in patients with radiologically confirmed CM compared with patients without CM. The median time to the development of BM was 26 months in patients with GrG 2,3 and 16 months in patients of the GrG 4,5. Conclusion: total assessment of PNI in post-surgery histological material, PSA level, ALP level, calcium, hemoglobin can be an effective tool for predicting risk of PCR BM development.

Ovarian Epithelioid Hemangioendothelioma. Case Report

Epithelioid hemangioendothelioma is a rare vascular sarcoma commonly occurring in the liver, soft tissues, and organs of the chest cavity. This study provides a detailed description of the clinical presentation, diagnosis, and treatment of epithelioid hemangioendothelioma of the ovary in a 71-year-old patient. According to the pathohistological examination, a mesenchymal tumor of the ovary with epithelioid cells was diagnosed. The immunohistochemical tests confirmed the diagnosis of epithelioid hemangioendothelioma. This clinical case is of interest for clinical practitioners due to the rare location of the tumor in female reproductive organs, which has not been previously documented in the medical literature.

Nucleocytoplasmic distribution of S6K1 depends on the density and motility of MCF-7 cells in vitro

Background:The ribosomal protein S6 kinase 1 (S6K1) is one of the main components of the mTOR/S6K signal transduction pathway, which controls cellular metabolism, autophagy, growth, and proliferation. Overexpression of S6K1 was detected in tumors of different origin including breast cancer, which was associated with a worse disease outcome. In addition, significant accumulation of S6K1 was found in the nuclei of breast carcinoma cells suggesting the implication of kinase nuclear substrates in tumor progression. However, this aspect of S6K1 functioning is poorly understood. The main aim of the present work was to study the subcellular localization of S6K1 in breast cancer cells with focus on cell migration.Methods:Multicellular spheroids of MCF-7 cells were generated using agarose-coated Petri dishes. Cell migration was initiated by spheroids seeding onto growth surface and subsequent cultivation for 24 and 72 hours. S6K1 subcellular localization was studied in human breast cancer and normal tissue, 2D and 3D MCF-7 cell culture using immunofluorescence analysis and confocal microscopy.Results:Analysis of histological sections of human breast cancer and normal tissue revealed predominantly nuclear localization of S6K1 in breast malignant cells and mainly cytoplasmic one in conditionally normal cells.In vitrostudies of MCF-7 cells showed that the subcellular localization of S6K1 depends on the cell density in the monolayer culture. S6K1 relocalization from the cytoplasm into the nucleus was detected in MCF-7 cells migrating from multicellular spheroids onto growth surface. Immunofluorescence analysis of S6K1 and immunocoprecipitation assay revealed the colocalization and interaction between S6K1 and transcription factor TBR2 (T-box brain protein 2) in MCF-7 cells. Bioinformatical analysis revealed existence of several phosphorylation sites in TBR2 for S6K1 suggesting that TBR2 can be a target for phosphorylation and regulation by S6K1.Conclusions:Subcellular localization of S6K1 depends on the density and locomotor activity of the MCF-7 cells.

Effects induced by a 50 Hz electromagnetic field and doxorubicin on Walker-256 carcinosarcoma growth and hepatic redox state in rats

We compare the effects of an extremely low-frequency electromagnetic field (EMF) with the chemotherapeutic agent doxorubicin (DOX) on tumor growth and the hepatic redox state in Walker-256 carcinosarcoma-bearing rats. Animals were divided into five groups with one control (no tumor) and four tumor-bearing groups: no treatment, DOX, DOX combined with EMF and EMF. While DOX and DOX + EMF provided greater inhibition of tumor growth, treatment with EMF alone resulted in some level of antitumor effect (p < .05). Superoxide dismutase, catalase activity and glutathione content were significantly decreased in the liver of tumor-bearing animals as compared with the control group (p < .05). The decreases in antioxidant defenses accompanied histological findings of suspected liver damage. However, hepatic levels of thiobarbituric acid reactive substances, an indicator of lipid peroxidation, were three times lower in EMF and DOX + EMF groups than in no treatment and DOX (p < .05). EMF and DOX + EMF showed significantly lower activity of serum ALT than DOX alone (p < .05). These results indicate that EMF treatment can inhibit tumor growth, causing less pronounced oxidative stress damage to the liver. Therefore, EMF can be used as a therapeutic strategy to influence the hepatic redox state and combat cancer with reduced side-effects.
