Bilateral near total blindness due to miliary neurocysticercosis

A man aged 18 years presented to our neuro-ophthalmology clinic with symptoms of headache, and painless and gradual onset bilateral visual loss since 2 months. While systemic examination was normal, visual acuity was noted to be perception of light in both eyes. Both pupils reacted sluggishly to light. Both eyes were found to be normal on examination apart from minimal temporal optic disc pallor. Urgent MRI of the head and orbits was ordered, which revealed numerous foci of neurocysticercosis (NCC) scattered throughout the brain parenchyma, giving it a ‘starry sky appearance’ (figure 1A). Ocular ischaemic syndrome following coil embolisation for direct carotid cavernous fistula

A 36-year-old man presented with proptosis and external ophthalmoplegia of the left globe following road traffic injury. Cerebral angiogram revealed moderate flow direct carotid cavernous fistula on left side for which coil embolisation was done repeatedly. Subsequently, the patient developed decreased vision in left eye and developed features of left-sided ocular ischaemic syndrome. The patient was treated conservatively with spontaneous reversal of ocular ischaemic syndrome and complete regain of visual function.

Atypical histopathology findings in presumed epibulbar cysticercosis

A 20-year-old young man presented with a painless epibulbar mass noted suddenly over the medial side of right eye (RE) 1 month back. Systemic history was insignificant and there was no history of trauma. On examination, visual acuity was 20/20 in both eyes (B/E) with no limitation of ocular movements. On examination, a mass lesion measuring about 1 cm in its longest dimension was noted over the medial side of the RE, with no apparent compression effect (figure 1). Overlying conjunctiva was mildly oedematous and congested. The posterior extent of the mass could not be appreciated. Gene Therapy for Inherited Retinal Diseases: From Laboratory Bench to Patient Bedside and Beyond

This comprehensive review provides a thorough examination of inherited retinal diseases (IRDs), encompassing their classification, genetic underpinnings, and the promising landscape of gene therapy trials. IRDs, a diverse group of genetic conditions causing vision loss through photoreceptor cell death, are explored through various angles, including inheritance patterns, gene involvement, and associated systemic disorders. The focal point is gene therapy, which offers hope for halting or even reversing the progression of IRDs. The review highlights ongoing clinical trials spanning retinal cell replacement, neuroprotection, pharmacological interventions, and optogenetics. While these therapies hold tremendous potential, they face challenges like timing optimization, standardized assessment criteria, inflammation management, vector refinement, and raising awareness among vision scientists. Additionally, translating gene therapy success into widespread adoption and addressing cost-effectiveness are crucial challenges to address. Continued research and clinical trials are essential to fully harness gene therapy's potential in treating IRDs and enhancing the lives of affected individuals.

Foreign body in ocular coats causing a pseudo optic nerve head shadow

Small intraocular foreign body in the outer coats of the eye may be wrongly interpreted as optic nerve head on ultrasound imaging. Such errors can be avoided by performing multiple sonography scans in different axes.
