Effect on heart rate and blood pressure after mental stress in coronary artery disease patients and normal individuals

Introduction: Cardiovascular diseases are the leading cause of death in almost every region of the world. One of the major risk factors for coronary artery disease is stress. The study was carried out to find the effect of mental stress on heart rate and blood pressure in coronary artery disease patients and in their age-matched normal counterparts. Materials and methods: Blood pressure and heart rate were recorded before and immediately after standardized mental stress (mental calculation for a minute) after necessary instruction among the volunteers (n=100, 50 coronary artery disease patients and 50 control, age 40-80 years) and then analyzed. Results: The mean HR was 75.40/min and the mean SBP was 121.84 mmHg of patient before stress which increased to 80.26/min (HR) and 135.12 mmHg (SBP) after stress. Similarly, the mean HR was 76.72/min and SBP was 116.24 mmHg before stress which increased to 80.5/min (HR) and 121.56 mmHg (SBP) after stress in normal individual. The result of the study showed mental stress induced rise in heart rate and blood pressure in both the groups. Nevertheless, mental stress induced increase in systolic blood pressure in patients exhibited higher than that of their aged-matched normal counterparts (p<0.05). Conclusions: The incidence of coronary artery disease is increasing day by day in the modern society. The measurement of heart rate and blood pressure (casual and mental stress induced) may help the clinicians to predict/screen coronary artery disease, especially the silent ones, and can prevent sudden angina pain.

Nepal is a South Asian multi-ethnic (125 distinct ethnic groups) country with nearly 30 million populations and located (latitudes 20⁰ and 31⁰N and latitude 80⁰ and 89⁰E) largely in the Himalayas and partly in Indo-Gangetic plain, surrounded by China and India. International tourism is the leading industry in Nepal and around 1.75 million foreigners visit in every year. The first Novel corona virus disease (COVID-19) case was detected in Nepal on January 5, 2020. After that Government of Nepal immediately took some drastic steps to prevent the spread of COVID-19 like - mandatory health check-up in airports, cancellation of visas, prevention of infiltrations in borders, sending back all foreigners/tourists in their home countries and also reformed their healthcare systems, by building up isolation centers in hospitals and involving trained personnel for the management of COVID-19. Besides that, they tried earnestly to educate public through media and imposed/ implemented rule for home quarantine. All these planning to fight against COVID-19 were implemented strictly and granted rewarding results in this pandemic. Till mid-November, 2020 the incidence of COVID-19 infection in Nepal was 208299, of whom, 1215 died.

Association Between Body Mass Index and Blood Pressure Among Medical Students of Tertiary Care Teaching Hospital in Birgunj

Introduction: Body mass index is an important parameter associated with a variety of disease processes. The risk of hypertension and cardiovascular diseases increases with an increase in body mass index. The study was conducted to compare the relationship between body mass index and blood pressure. Objective: To evaluate the prevalence and correlate the Obesity and Hypertension in medical students Methodology: The study was conducted in 200 students (113 males and 87 females). Height, weight and blood pressure were recorded from all participants and body mass index was calculated. The recorded body mass index was utilized to divide the student into underweight, normal, over weight and obese category according to the World Health organization body mass index classification. Hypertension was determined from the measure of blood pressure. Then comparison of blood pressure with body mass index was made. Results: Among 200 students 6% were obese, 22% were overweight, 65% were normal and 7% underweight. The mean height was 163cm and mean weight 60.48kg. The mean value of systolic blood pressure (105.85, 115.45, 134.95, 137.16 mmHg) and diastolic blood pressure (70.14, 76.15, 90.72, 93.33 mmHg) increased with increasing body mass index. Conclusion: Overweight and obesity increases the risk of hypertension among students.

Pulmonary Function Tests in Transport Workers Within the Ring Road of Kathmandu Valley

Background: Transport workers (drivers and helpers) are exposed to dust and vehicle emissions continuously throughout their duty hours. Most common health hazards on exposure to air pollution is related to respiratory system. The main objective of the study was to measure lung function parameters of transport workers and compare results with those of the general public. This study also aimed to evaluate the effect of job duration on transport workers.Methods: A cross-sectional comparative study was done in a total of 78 apparently healthy, non-smoking, male transport workers working for at least one year within the Ring Road in Kathmandu valley. The results obtained from these professionals were compared to 78 apparently healthy, non-smoker male general public, working as students in medical college and administrative staff in transport office.Results: Statistically significant decrease in Forced Vital Capacity (FVC), Forced Expiratory Volume in one second (FEV1), Peak Expiratory Flow Rate (PEFR) and Forced Expiratory Flow between 25% and 75% (FEF25%-75%) among transport workers was noted as compared to the general public. Transport workers working for more than five years also have deteriorated above mentioned parameters as compared to their colleagues working five years or less in the same profession.Conclusions: This study concludes that there is an adverse effect of ambient air pollution on pulmonary functions of the transport workers. The findings of this study also demonstrate that the transport workers working in Kathmandu valley for more than five years have deteriorated pulmonary functions.Med Phoenix. Awareness of Yoga and Its Benefit Among Medical Students of Tertiary Care Teaching Hospital in Birgunj

Introduction Yoga is a practice of mind and body that combines physical postures, breathing exercises, and meditation. Yoga helps reduce stress, lower blood pressure and heart rate. It is considered one of many complementary and integrative health approaches. Yoga brings together physical and mental discipline that may help achieve a healthy body and peaceful mind. In this study, we aim to assess knowledge and awareness regarding yoga and its benefit among preclinical students. Materials and Methods This cross-sectional study includes 176 preclinical medical students of National Medical College, Birgunj, Nepal. A total of 14 self-designed close-ended questionnaires regarding knowledge and awareness about Yoga were used as a study tool. Data analysis was done using Microsoft Excel, 2010. Results Among 176 preclinical students, 77.84% had the habit of practicing yoga, but only 14.20% of students continued it. About 96.59% of students knew the benefits of yoga. Conclusion The result of the study reveals that there is a positive influence of practicing yoga among students. By practicing yoga, it is capable to achieve personal abilities such as stress management, attention in their work, and maintenance of physical and mental health.

Seroprevalence of PPR in Goats of Buffer Zone of Chitwan National Park

A study conducted from September to December 2019 aimed to assess the seroprevalence of Peste des Petits Ruminants (PPR) in goats residing in the buffer zone of Chitwan National Park. A total of 221 serum samples were collected from unvaccinated goat flocks across the buffer zone regions (48 from Bhojad, 64 from Bhandara, 66 from Padampur, 25 from Gondrang, and 18 from Kathar) using random sampling techniques. Competitive Enzyme-Linked Immunosorbent Assay (ELISA) was employed to detect antibodies against the PPR virus (PPRV). The study revealed an overall seroprevalence rate of 28.96%. The highest seroprevalence rate of 39.58% was observed in goats from Bhojad, while the lowest rate of 15.63% was found in goats from Bhandara. The findings emphasize the need for regular vaccination of small ruminants in Bhojad, Padampur, and Kathar to enhance immunity and prevent PPR infection. Additionally, such measures can help mitigate the transmission of PPR between goats and wild animals as they share common grazing lands.
