ECHOCARDIOGRAPHIC MANIFESTATIONS OF COVID 19 ILLNESS AND DEVELOPMENT OF PERSISTENT RV DYSFUNCTION AND PULMANARY HYPERTENSION AS A LONG TERM SEQUELAE OF COVID 19 ILLNESS: A STUDY AMONG PATIENTS OF SOUTH EAST ASIAN REGION

Abstract Objectives To study the Echocardiographic manifestations of covid 19 illness among patients admitted in our facility, Correlate MAPSE, TAPSE, PASP, CRP levels and CTSI among covid 19 patients with their 28 day outcome as survivors and non survivors and to look for evidence of residual RV dysfunction and Pulmonary hypertension using TTE after 1 year of follow-up. Study design Prospective observational study at various medical wards and ICUs in SMS medical college and associated hospitals. Methods 258 patients with a Covid-19 RT-PCR positive report from a throat or a nasal swab within 72 hours of admission were included in the study. Each patient underwent a complete clinical assessment and routine blood investigations including CRP levels were done. A complete transthoracic echocardiogram was done within 48 hours of admission. Patients also underwent a HRCT chest and CTSI scores were estimated. All patients were followed for a period of 28 days. The MAPSE, TAPSE, PASP, CTSI and CRP levels were then correlated with the outcome of the patient. The survivors again underwent a TTE at 1 year after their recovery from covid-19 illness to look for residual RV dysfunction by TAPSE and the development of pulmonary hypertension as measured by PASP using Bernoulli’s equation. Results Amongst patient of covid 19 illness the MAPSE, TAPSE, PASP, CTSI and CRP levels all correlated well with outcome of patients. While most covid-19 survivors recovered from their illness yet some patients showed evidence of persistent RV dysfunction and pulmonary hypertension even after 1 year of follow up.

A STUDY OF CLINICAL AND ETIOLOGICAL PROFILE OF PATIENTS OF MINOCA (MYOCARDIAL INFARCTION WITH ANGIOGRAPHICALLY NON OBSTRUCTIVE CORONARY ARTERIES) AMONG THE PATIENTS WITH ST ELEVATION MYOCARDIAL INFARCTION

Objective: Myocardial infarction (MI) with non-obstructive coronary arteries (MINOCA) is a relatively new term that is characterized by clinical evidence of MI with normal or near-normal coronary arteries on coronary angiography. The aim of this was to document the prevalence and demographics of MINOCA in an Indian population. Methods: It is a single center study. All consecutive patients who were â‰¥18 years old, diagnosed with MI according to the Third Universal Definition of Myocardial Infarction, and had undergone coronary angiography were included in the study. Patients with stable coronary artery disease, unstable angina pectoris, a history of revascularization, and type 4/5 MI were excluded. Results: A total of 548 patients who were diagnosed with MI and had undergone coronary angiography were enrolled. The prevalence of MINOCA was 6.2% in the overall study population. Compared with non-MINOCA patients, those with MINOCA were younger, had a higher prevalence of the female gender, and had a history of COVID 19 infection. Also, the median left ventricular ejection fraction as seen on echocardiography and the ratio of Killip Class I status at presentation was significantly higher in MINOCA patients than in non-MINOCA patients (p<0.001).

Abstract Objectives To study Get with the guidelines heart failure Score (GWTG-HF) among patients of acute decompensated heart failure, correlate it with outcome of survival or death till 4 week of illness, compare its performance with APACHE 2 score and to Compare its results with NT PROBNP in predicting mortality amongst patient of south east Asian ethnicity. Study design prospective observational study at medical wards and ICUs in the Department of Cardiology, SMS medical college. Methods In this study 300 patients of acute decompensated heart failure, as per Framingham criteria were analyzed and each patient underwent a complete clinical evaluation along with calculation of GWTG HF and APACHE 2 score. All the patients had NT pro BNP test within 48 hours of admission. Patient were then followed till 4 weeks of illness for outcome of survival or death, whichever occurred earlier. GWTG HF score of the patient was then compared with APACHE 2 score and it was correlated with the outcome of patients and NT pro BNP levels. Results The GWTG-HF score could predict the outcome of survival and death in ADHF patients of south east Asian ethnicity. When compared, APACHE 2 score performed better than GWTG-HF score in predicting outcome. However, GWTG-HF score was easily calculated at the bedside and was easy to use. GWTG-HF score correlated with NT-pro BNP levels with rising score having higher NT pro BNP levels.

Abstract Objectives Assessment of RV function to predict proximal RCA stenosis and identification of higher-risk patients in IWMI by echocardiographic parameters. Study design Two-arm, single-center cross-sectional study in SMS medical college and Hospital Jaipur. Methods We enrolled 67 patients with first episode of acute IWMI within 24 hours of symptom onset. TAPSE, S’, and MPI were measured. Within one month, patients underwent coronary angiogram or intervention. Based on angiographic findings patients divided into group A (significant proximal RCA stenosis) and group B (without significant proximal RCA stenosis). Results 25 patients were in group A and 42 in group B. RV involvement was significant among patients with proximal RCA stenosis. A significantly low SBP, DBP, HR, and raised JVP, NT-pro BNP, Troponin I with ST elevation V4R, RV diastolic dysfunction, arrhythmia, and underwent intervention were found among the group with proximal RCA stenosis. Patients with proximal RCA stenosis had significantly low TAPSE-M, ET-PW, ET by TDI, S’, E’, RVFAC, and high MPI by PW, IRT by TDI, ICT by TDI, MPI by TDI on ECHO. Sensitivity to detect proximal RCA stenosis by TAPSE-M mode 92%, MPI by TDI 88%, and 100% specificity by S’ by TDI < 10 cm/s, MPI by TDI, and ST elevation V4R. Conclusion Echocardiographic assessment of RV parameters like TAPSE, S’ velocity, and MPI are useful in predicting proximal RCA as infarct-related artery in IWMI. Patients with proximal RCA stenosis had raised JVP, NT-proBNP, Troponin I, ST elevation V4R, RV diastolic dysfunction, arrhythmia and underwent intervention.
