Exploring the synergistic behaviour of paclitaxel and vorinostat upon co-loading in albumin nanoparticles for breast cancer management

Abstract Breast cancer is challenging to treat accompanied with poor clinical outcomes. Paclitaxel (PTX) is a first-line chemotherapeutic agent, but possesses limitations due to side effects, high dose, non-specific tissue distribution, and drug resistance. An epigenetic modulator, Vorinostat (VOR) is known to enhance PTX efficacy and therefore to resolve the issues of conventional PTX formulations we designed PTX-and VOR-bound albumin nanoparticles (PTX-VOR-BSA-NPs) using antisolvent precipitation technique where albumin is used as a carrier and a targeting agent. The PTX-VOR-BSA-NPs were of 140 nm size, PDI around 0.18, and about 78% and 68% of entrapment efficiency for PTX and VOR, respectively. A bi-pattern release of both PTX and VOR was observed from PTX-VOR-BSA-NPs with a burst release for 2 h succeeded by sustained release till 24 h. A significantly lower %cell viability was observed in MCF-7 cell lines, while efficient cellular drug uptake was found in MDA-MB-231 cells. Furthermore, a greater apoptotic index was found compared to free PTX and VOR because of the synergistic activity of these drugs. PTX-VOR-BSA-NPs had better PTX pharmacokinetic profile than Intaxel® with ~1.4-fold improved AUC. The self-targeting property of BSA along with the synergistic action of PTX and VOR from PTX-VOR-BSA-NPs aided in noteworthy reduction in the tumor volume compared to Intaxel®. Further, the PTX-VOR-BSA-NPs showed similar levels of toxicity biomarkers as that of control. Overall, the developed PTX-VOR-BSA-NPs were found to have less toxicity and more effectiveness compared to the marketed formulation, thus affirming the generation of a potent as well as and safe product.

MANUFACTURING AND POSITIONING (GENERATIONS) OF OIL‐IN‐WATER NANOSIZED EMULSIONS

In conjunction with excipients and technology development that happened over the time periods of approximately seven decades, the changes occurred not only to the terminology of emulsion but also to the therapeutic advancements/applications of emulsions. The clarification of terminology is being discussed in Chapter 1. The intention of this chapter is to elaborate and portray the detailed advancement that occurred on the o/w nanosized emulsions and this advancement is simply named as "generations." Considerable and significant changes and advancements in each one of the generations are unequivocally materialized so that this group of emulsions are put in the next generations. The criteria considered to bifurcate the generations of o/w nanosized emulsions includes new physical property inclusion due to the presence of particular excipients or advancement in the therapeutic applications. There are four bifurcated generations thus far created that all discussed/delineated with proper justifications and in some cases with case studies.

BIOFATE OF NANOSIZED EMULSIONS

Pharmaceutically, the oil-in-water nanosized emulsions find therapeutic applications in almost all routes of administration. However, the administered emulsion particles do encounter many body defensive mechanisms that sometimes may be of beneficial and most of the times may not be desirable. In principle, the monitoring of administered emulsion particles is naturally related to the normal catabolic reactions of the human body. In other words, the catabolic reaction is called as biofate of administered emulsion particles. Since emulsion particles somehow resemble with the endogenous lipid/lipoprotein molecules at least structure-wise, the biochemical lipolysis and other lipid breakdown catabolic reactions are the major emulsion particles' degradation/engulf process. Taking ocular and parenteral routes of administration as litmus test, the biofate of emulsion particles is discussed. In fact, the oil-in-water nanosized emulsions grouped under first-generation category really provide valuable information concerning the biofate of administered emulsion particles grouped under second- and third-generation categories. This chapter thus elaborates mainly the biofate of first-generation emulsions including the clinical complications and immunological issues related to the administration of emulsion particles into the human body.

Nanoparticle-Based Drug Delivery System: The Magic Bullet for the Treatment of Chronic Pulmonary Diseases

Chronic pulmonary diseases encompass different persistent and lethal diseases, including chronic obstructive pulmonary disease (COPD), idiopathic pulmonary fibrosis (IPF), cystic fibrosis (CF), asthma, and lung cancers that affect millions of people globally. Traditional pharmacotherapeutic treatment approaches (i.e., bronchodilators, corticosteroids, chemotherapeutics, peptide-based agents, etc.) are not satisfactory to cure or impede diseases. With the advent of nanotechnology, drug delivery to an intended site is still difficult, but the nanoparticle's physicochemical properties can accomplish targeted therapeutic delivery. Based on their surface, size, density, and physical-chemical properties, nanoparticles have demonstrated enhanced pharmacokinetics of actives, achieving the spotlight in the drug delivery research field. In this review, the authors have highlighted different nanoparticle-based therapeutic delivery approaches to treat chronic pulmonary diseases along with the preparation techniques. The authors have remarked the nanosuspension delivery via nebulization and dry powder carrier is further effective in the lung delivery system since the particles released from these systems are innumerable to composite nanoparticles. The authors have also outlined the inhaled particle's toxicity, patented nanoparticle-based pulmonary formulations, and commercial pulmonary drug delivery devices (PDD) in other sections. Recently advanced formulations employing nanoparticles as therapeutic carriers for the efficient treatment of chronic pulmonary diseases are also canvassed.

OVERVIEW OF TOCOL‐BASED EMULSIONS, OXYGEN‐CARRYING EMULSIONS, EMULSIONS WITH DOUBLE OR TRIPLE CARGOS AND EMULSION‐LIKE DISPERSIONS

Very interesting peculiar structure found in the dispersed oil droplets of oil-in-water (o/w) nanosized emulsions is acquainted/exemplified with an overview of atherosclerosis theragnosis. The o/w nanosized emulsions containing the observed peculiar structure is termed as Janus emulsions. Auspiciously, the o/w nanosized emulsions find their applications not only on syndrome-managing aspects but also open a new venture to meet the highly demanding dosage form development for plant-derived active pharmaceutical ingredients (APIs). The o/w nanosized emulsions conceded in this venture of carrying the plant-derived APIs may get different nomenclature in medicinal and pharmaceutical literatures. Such differently named o/w nanosized emulsions include imaging emulsions, oxygen-carrying emulsions (called as fluorocarbon emulsions), and tocol emulsions. Similarly, the o/w nanosized emulsions may also have compromised/omitted their basic components such as oil(s) and emulsifier(s) in order to accommodate the plant-derived APIs. The o/w nanosized emulsions prepared devoid of oil(s) is called as oil-less emulsions. In a similar manner, the omission of traditional emulsifier/surfactant from o/w nanosized emulsions allows the name conversion as emulsifier/surfactant-free emulsions. Interestingly, the complete elimination of both emulsifier/surfactant and oil(s) from o/w nanosized emulsions results in the name change of emulsifier/surfactant- and oil(s)-free emulsions. The latter one is also termed as aqueous two-phase systems (ATPS) or water-in-water emulsions or collectively ATPS emulsions. The recently observed Cerberus architecture onto the dispersed oil droplets of the emulsion needs to be stabilized in terms of multiple medicament loadings, storage stability of the structure, medicament leakage from the structure, and development of viable therapeutic applicability. The potential applications of all of the abovesaid newly introduced emulsions are briefly discussed/highlighted in this chapter.
