The multiple sclerotic osteogenic sarcoma of early childhood

Two cases of sclerosing osteogenic sarcoma that occurred in early childhood are reported. They and the similar cases reviewed are unusual in their development at an unusually early age, their multicentric presentation and their densely sclerotic nature. These are uncommon manifestations of osteogenic sarcoma and are considered due to proliferative metastatic osteoid formation in areas of rapid bone growth.

Childhood multifocal osteosarcoma. Clinicopathologic and radiologic correlates

The authors reviewed the clinical charts and pathologic and radiographic materials of nine pediatric patients (mean age, 11.0 years) presenting with multiple skeletal foci of osteosarcoma. Four patients presented with pulmonary metastases, and five had osseous disease only. All had densely sclerotic, predominantly osteoblastic tumors, usually in metaphyseal locations. Evidence of aggressive tumor cell behavior—such as vascular, articular, epiphyseal, or soft-tissue invasion—was usually present in surgical specimens. Despite intensive chemotherapy, all patients died from 6 to 37 months after diagnosis (median survival, 12 months). Comparison of clinical and pathologic features for patients presenting with or without pulmonary metastases failed to disclose any notable differences between the two groups, indicating a common pathogenesis and evolution for their multifocal disease. The authors conclude that this variant of osteosarcoma is a highly aggressive form of the disease that may be accompanied by pulmonary metastases.

Forest fragmentation and biodiversity conservation in human-dominated landscapes.

The future of forest biodiversity is increasingly dependent on the ability of species to persist in isolated forest remnants embedded within a mosaic of human land uses. We review the impacts of forest loss, habitat degradation and alteration of the spatial structure of the landscape on connectivity and population persistence in fragmented landscapes. Conservation management is just beginning to come to grips with the challenges faced in moving away from the traditional patch-focused approach of conservation in gazetted nature reserves towards a landscape-focused approach of conserving biodiversity in a mosaic that includes managed and semi-natural habitats. Contentious debate has centred on the ability of degraded forest systems to sustainably support forest specialist species, and the utility of traditional species-area approaches to predict biodiversity loss relationships without appropriately accounting for varying species responses to landscape matrix quality and edge effects. While there is no doubt that some area-sensitive and disturbance-sensitive species require large areas of primary forest to ensure population persistence, there is an emerging consensus that the maintenance of biodiversity will depend as much, if not more, on the extent, magnitude and spatial structuring of landscape processes within the degraded matrix surrounding primary forest remnants. Accordingly, biodiversity conservation must refocus more on the interaction between patch and landscape processes than on patch processes per se. This will demand more effective discrimination of the relative importance of total habitat loss, declining habitat quality and altered spatial structuring of suitable habitat as mechanistic drivers of biodiversity loss in fragmented forest landscapes.

Undifferentiated sarcomas of children: Pathology and clinical behavior—an Intergroup Rhabdomyosarcoma Study

Undifferentiated soft tissue sarcoma (UND-STS) is the most poorly defined tumor eligible for Intergroup Rhabdomyosarcoma Studies (IRS). Recent IRS UND-STS experience was reviewed to assess the histologic characteristics and clinical behavior of undifferentiated sarcomas. Of the 1,527 patients entered on IRS-III and IRS pilot-IV, 96 had tumors classified by the IRS Pathology Committee as UND-STS. Of these, 52 had adequate histologic material for this study. After application of immunohistochemistry, 18 tumors were reclassified, mostly as embryonal rhabdomyosarcomas (RMS), primitive neuroectodermal tumors, and intra-abdominal desmoplastic small round cell tumors. The remaining 34 UND-STS had a diffuse hypercellular histologic pattern made up of sheets of medium-sized cells. The tumor cells had a minimal to moderate amount of cytoplasm and a variable nuclear morphology, predominately vesicular with finely granular chromatin. Except for reactivity with antibodies against vimentin, most tumors had a negative immunohistochemical profile. The 5 year Kaplan-Meier survival estimate for patients with non-metastatic disease was 72%, a significant improvement when contrasted with patients diagnosed to have UND-STS in IRS-I and IRS-II. Med. The response of Ewing's sarcoma to sequential cyclophosphamide and adriamycin induction therapy.

Twenty-four consecutive patients with Ewing's sarcoma were treated in a protocol designed to deliver induction chemotherapy with postinduction surgical-pathologic evaluation of the primary tumor site. This was followed by delayed radiotherapy, the dose and port of which was dependent on the response to induction chemotherapy. All patients received 5 courses of sequential cyclophosphamide and adriamycin during the 3-mo induction period. Nineteen of 23 evaluable patients had no gross residual tumor following this therapy. Of the remaining 4, 2 had complete surgical excision of residual gross disease. Of the 22 patients who were free of gross tumor following induction chemotherapy and surgery, 5 received no radiotherapy, 16 received moderate-dose limited port radiotherapy (3000-3500 R), and 1 received high-dose limited port radiotherapy (5000 R). All 14 patients with localized disease attained remission and are surviving 9-41+ mo (median 21+) with 2 local recurrences occurring after 10 and 33 mo of remission. Of the 10 patients presenting with metastatic disease, 8 attained complete remission with 4 of the 8 remaining disease-free 12-34+ mo from diagnosis. This study indicates that Ewing's sarcoma is very sensitive to moderate-dose 2 drug chemotherapy of low toxicity and that it is possible to delay radiotherapy and any extensive surgical procedure until remission is induced.

A new Afrotropical genus and species of Tetrastichinae (Hymenoptera: Eulophidae) inducing galls on Bikinia (Fabaceae: Caesalpinioideae) and a new species of Ormyrus (Hymenoptera: Ormyridae) associated with the gall

Gallastichus mutuus Rasplus & La Salle gen. nov. & sp. nov. (Hymenoptera: Chalcidoidea: Eulophidae) and Ormyrus nkoloensis Rasplus sp. nov. (Hymenoptera: Chalcidoidea: Ormyridae) are described from Cameroon. Both taxa were collected from leaf galls formed on Bikinia letestui (Pellegr.) Wieringa subsp. letestui (Fabaceae: Caesalpinioideae). Gallastichus mutuus appears to be responsible for gall induction, whereas O. nkoloensis is likely a secondary coloniser, either a parasitoid or an inquiline. This is the first recorded instance of a gall being induced in an extrafloral nectary.
