Objective - to investigate spectral characteristics of laserinduced fluorescence (LIF) of bone substances, and to develop a new technique of osteoporosis diagnosis. Sixty-nine patients surgically treated for thoracic and lumbar spine fractures were examined. Clinical examinations, plain radiography in two projections, CT and MRI were used for spine fracture diagnosis. Saville index was a criterion for osteopenia. Bone mineral density was assessed by dual-energy X-ray densitometry. Histological and histomorphometric analyses were applied to 46 fragments of vertebral bodies and spinous processes taken intraoperatively, and 96 autopsy fragments of vertebral bone, spinous processes, iliac crest, and the ninth rib taken from patients died of trauma who had no metabolic bone diseases in their previous history. Fluorescence spectra were analyzed with multichannel system of spectrum registration. Gas-discharge KrF excimer laser with a wave length of λ = 28 nm and impulse energy of 5-10 mJ was used as a source of radiation. Spectral characteristics of fluorescence intensity of biological substances forming a bone tissue have shown that the most expressed fluorescence of hydroxyapatite is observed in the band of 380-450 nm. The LIF technique permits not only to differentiate the structural-functional layers of bone, but also to determine a bone tissue mineralization degree. The possibility to perform express diagnosis is one of the advantages of the technique. It is especially topical for traumatology.

Aims:The aim of this study is to compare the concentration of brain natriuretic peptide (BNP), inflammatory markers , tumour necrosis factor-a (TNF-a), and C-reactive protein], echocardiographic parameters of left ventricular function, and physical capacity and to investigate how they change after carvedilol in patients with various aetiologies of heart failure (HF).Material and methods: A total of 86 patients with symptomatic HF in NYHA classes II and III [22 patients with non-ischaemic (NIC) and 64 patients with ischaemic aetiology-IC].Diagnostic work-up consisted of NYHA class assessment, echocardiographic parameters of left ventricular systolic [left ventricular diastolic dimension, left ventricular end-diastolic volumes, and ejection fraction (EF)], and diastolic function (E/A, isovolumic relaxation time, and deceleration time), measurements of BNP, TNF-a, C-reactive protein, and cardiopulmonary exercise stress test (VO2peak and VE/VCO2) at baseline and at 12 months after carvedilol therapy (at a mean dose of 28.5+ 5.8 mg/day) added to enalapril and a diuretic.Results: At baseline patients with NIC were characterized by higher heart rate at rest HRs (P ¼ 0.001), restrictive filling pattern (P ¼ 0.045), and lower C-reactive protein concentration (P ¼ 0.05).The remaining parameters did not differ significantly.After 12 months, carvedilol therapy patients with NIC were characterized by higher HRs (P ¼ 0.04), lower EF (P ¼ 0.04), higher IL-6 concentration (P ¼ 0.01), and by lower VO 2 peak%N (P ¼ 0.04) as compared with IC ones. Conclusion:In patients with HF irrespective of its aetiology chronic treatment with carvedilol reduces HR and improves left ventricular ejection fraction, whereas patients with ischaemic HF additionally the concentrations of IL-6 and C-reactive protein are decreased and VO2peak%N is increased.

Methods of operating mode control of the rotational biological reactor

Scaffold technology is a new method for replacement of damaged tissues in a living organism using patient's own cells. A new rotational bioreactor was designed for the task of cell-seeded scaffold cultivation for the restoration of critical bone defects. The numerical simulation of fluid flow in the rotational bioreactor was performed. The calculated shear stress and static pressure values acting on the stem cells at different rotational frequencies allow selection of operating modes depending on the desired conditions. Optical control of stem cells differentiation was worked out. The obtained laser-induced fluorescence excitation-emission matrices of investigated samples were analyzed using principal component method. The spectra of fluorophores were reconstructed by combining positive narrow linear combinations from alternating principal components. Evaluating the ratio of narrow component contributions allows distinguish seeded and unseeded by stem cells scaffolds, as well as scaffold samples exposed by shear stress in the rotational bioreactor.
