Lateral Rectus Muscle Injury During Double-Headed Pterygium Surgery: A Rare Complication

A 43-year-old man developed double vision, esotropia, and abduction limitation after excision of a doubleheaded pterygium. Exploration during strabismus surgery 4 months later revealed a partially severed lateral rectus muscle and extensive conjunctival scarring. Direct injury to the lateral rectus muscle during pterygium surgery is extremely rare. Prevention and management strategies are discussed. [ J Pediatr Ophthalmol Strabismus . 2020;57:e96–e99.]

Double extraocular muscle avulsion following injury by goat’s horn

Introduction: Trauma to extraocular muscle without globe perforation is rare. This case report describes the clinical features and principles of repair of the simultaneous injury to two extraocular muscles sustained from a goat's horn. Methods: Case records of the 36-year-old man who suffered trauma to his left eye were reviewed. Results: Examination revealed diplopia, hypotropia of the left eye, and a lacerated superior conjunctiva through which a muscle tendon prolapsed. Magnetic resonance imaging (MRI) of the orbit indicated discontinuity and fuzziness of the left superior rectus muscle (SR). Surgical exploration showed that the prolapsed tendon belonged to the superior oblique muscle (SO). Avulsion of SR was also noted 14 mm from its insertion. The proximal end of SR was anastomosed with the distal segment. The proximal end of the SO could not be traced. Postoperatively, the elevation improved marginally and there was a small hypotropia. His diplopia was managed with prisms. Discussion:The goat's horn acted like a hook and avulsed two contiguous muscles from orbit in a posterior to anterior direction. The hypertropia due to SO avulsion was compensated partly by the hypotropia due to the concurrent damage to the SR. Immediate surgical intervention resulted in a good alignment.

Clinical and molecular aspects of congenital aniridia – A review of current concepts

Congenital aniridia is a pan ocular disorder characterized by partial or total loss of iris tissue as the defining feature. Classic aniridia, however, has a spectrum of ocular findings, including foveal hypoplasia, optic nerve hypoplasia, nystagmus, late-onset cataract, glaucoma, and keratopathy. The latter three are reasons for further visual compromise in such patients. This entity is often due to mutations in the PAX6 (Paired box protein Pax-6) gene. Recently, aniridia-like phenotypes have been reported due to non- PAX6 mutations as in PITX2, FOXC1, FOXD3, TRIM44 , and CYP1B1 as well wherein there is an overlap of aniridia, such as iris defects with congenital glaucoma or anterior segment dysgenesis. In this review, we describe the various clinical features of classic aniridia, the comorbidities and their management, the mutation spectrum of the genes involved, genotype-phenotype correlation of PAX6 and non- PAX6 mutations, and the genetic testing plan. The various systemic associations and their implications in screening and genetic testing have been discussed. Finally, the future course of aniridia treatment in the form of drugs (such as ataluren) and targeted gene therapy has been discussed.

Measures and variability with age of low contrast acuity and near stereoacuity in children

Low contrast acuity (LCA) and near stereoacuity (NS) testing are integral to the comprehensive assessment of sensory visual function in children. However, routine ophthalmological evaluations seldom take these measures into consideration. Additionally, there is limited literature regarding the normative values of these parameters in children.This study investigated LCA and NS measures and their variability in children with normal visual acuity. The aim was to provide a benchmark for distinguishing normal measures from abnormal ones.A prospective observational study was conducted in primary and secondary schools across North India. The participants numbered 240 children, aged between 3 and 15 years. They were split into 12 smaller groups of 20 participants in each age group. Only participants with normal monocular uncorrected visual acuity, no refractive error, normal birth history, and no systemic ailments, were recruited. All the participants underwent a complete ophthalmic examination and non-cycloplegic retinoscopy. LCA was measured, using the low contrast Lea number chart at three metres. NS was measured using the Randot® stereo test at 40 cm.The percentages of males and females, were found to be 55%, and 45%, respectively (p = 0.093). The mean NS was found to be 38.7 ± 11.5 arcsecs in the 3-9-year age group, and 26.7 ± 5.6 arcsecs in the 9-15-year age group (p-value <0.001). The mean NS showed an increasing trend up to 9 years of age. The mean LCA was 64.4 ± 20.1 in the age group of 3-8 years, and 76.38 ± 11.39 in the age group of 8-15 years (p-value <0.001). Considerable variability was noted in the LCA in the younger age group (p-value = 0.000).LCA and NS mature gradually during childhood. LCA stabilises after the age of 8, while NS stabilises after the age of 9.

Subconjunctival silicone oil complicating strabismus surgery.

Purpose: To report a case of an otherwise simple strabismus surgery that became complex due to the presence of extensive subconjunctival silicone oil. Methods: A 15-year-old boy underwent strabismus surgery for sensory exotropia. He had undergone two prior vitreoretinal surgeries for traumatic endophthalmitis and retinal detachment, respectively. The second procedure involved injection of silicone oil as an endotamponade. During the strabismus surgery, subconjunctival nodules filled with silicone oil were noted. This made the surgery difficult due to episcleral fibrosis. The multiple silicone oil cysts were removed by rupturing or resecting them along with the tenon's fascia. The extraocular muscles were meticulously isolated and operated using adjustable suture technique. Results: Following surgery, the strabismus was corrected satisfactorily and the patient was relieved of his ocular discomfort and congestion. Conclusions: Subconjunctival leakage of silicone oil can lead to capacious inflammation and complicate strabismus surgery manifold.

Maternal Night Blindness and Microphthalmos, Anophthalmos, and Coloboma (MAC) Disorders – Is there an Association?

Abstract OBJECTIVES Microphthalmos, Anophthalmos, and Coloboma (MAC) are common causes of childhood blindness. In 1996, World Health Organisation (WHO) proposed night blindness (NB) as a functional indicator of Vitamin A deficiency (VAD). We aimed to investigate the association of maternal antenatal NB, environmental risk factors and socioeconomic determinants with MAC. METHODS A case-control study was conducted between 2018 to 2021 comparing risk factors between MAC and control cases. Data regarding maternal antenatal NB, birth order, consanguinity, family history, maternal dietary habits during pregnancy, history of spontaneous abortions, and father’s income were collected retrospectively and through telephonic interviews with mothers. Bilateral anophthalmos, severe microphthalmos or a combination defined a severe MAC phenotype. RESULTS 220 children with MAC and 219 normal controls aged 0–3 years were included. Antenatal maternal NB was observed in 33.2% of MAC and 3.2% of controls (p < 0.001). A higher birth order (2nd or more), parental consanguinity, and a history of prior spontaneous abortions were also more common in MAC. A history of antenatal maternal NB was more common in severe MAC (67.3%) as compared to the less severe MAC (22.6%) (p < 0.001). CONCLUSIONS The study identified antenatal maternal NB, higher birth order, parental consanguinity, and history of spontaneous abortions as significant risk factors for MAC disorders, highlighting the influence of maternal nutrition and genetics. To the best of our knowledge, this is the largest case-control study to show an association of maternal NB and MAC. Maternal NB has been used as a functional indicator of VAD.
