Validation of the New Interpretive Guidelines for the Clinical Dementia Rating Scale Sum of Boxes Score in the National Alzheimer's Coordinating Center Database

Background

It was recently demonstrated that the Clinical Dementia Rating scale Sum of Boxes (CDR-SB) score can be used to accurately stage severity of Alzheimer dementia and mild cognitive impairment (MCI). However, to our knowledge, the utility of those interpretive guidelines has not been cross-validated or applied to a heterogeneous sample of dementia cases.

Objective

To cross-validate the staging guidelines proposed in a previous study using the National Alzheimer's Coordinating Center (NACC) database.

Design

The previously published cut scores were applied to the NACC sample and diagnostic accuracy estimates obtained. Next, analyses were restricted to NACC participants with a CDR global score (CDR-GS) of 0.5 and receiver operating characteristic curves generated to determine optimal CDR-SB cut scores for distinguishing MCI from very early dementia.

Setting

The 2008 NACC uniform data set.

Participants

There were 12 462 participants (5115 controls; 2551 patients with MCI; 4796 patients with dementia, all etiologies) in the NACC data set used for the current analysis.

Main Outcome Measure

Accurate prediction of diagnoses (MCI or dementia) using the CDR-SB score.

Results

The previously proposed CDR-SB ranges successfully classified the vast majority of patients across all impairment ranges with a κ of 0.91 and 94% overall correct classification rate. Additionally, the CDR-SB score discriminated between patients diagnosed with MCI and dementia when CDR-GS was restricted to 0.5 (overall area under the curve = 0.83).

Conclusions

These findings cross-validate the previously published CDR-SB interpretative guidelines for staging dementia severity and extend those findings to a large heterogeneous sample of patients with dementia. Additionally, the CDR-SB scores distinguished MCI from dementia in patients with reasonable accuracy when CDR-GS was restricted to 0.5.

Individuals with amnestic mild cognitive impairment (aMCI) experience cognitive declines in learning and memory greater than expected for normal aging, and are at a high risk of dementia. We previously reported that sedentary aMCI patients exhibited neuroinflammation that correlated with brain amyloid beta (Aβ) burden, as determined by 18F-florbetapir positron emission tomography (PET). These aMCI patients enrolled in a one-year randomized control trial (AETMCI, NCT01146717) to test the beneficial effects of 12 months of moderate-to-high intensity aerobic exercise training (AET) or stretching/toning (ST) control intervention on neurocognitive function. A subset of aMCI participants had PET imaging, cognitive testing, and immunophenotyping of cerebrospinal fluid (CSF) and peripheral blood after AET or ST interventions. As adaptive immune responses were similar between AET and ST groups, we combined AET/ST into a general 'physical activity' (PA) group and compared Aβ burden, cognitive function, and adaptive immune cell subsets to sedentary lifestyle before intervention. We found that PAinduced immunomodulation of CD4+ and CD8+ T cells in CSF correlated with changes in Aβ burden in brain regions associated with executive function. Furthermore, after PA, cognitive scores on tests of memory, processing speed, attention, verbal fluency, and executive function were associated with increased percent representation of circulating naïve B + T cells. We review the literature on aMCI-related cognition and immune changes as they relate to exercise, and highlight how our preliminary data suggest a complex interplay between the adaptive immune system, physical activity, cognition, and Aβ burden in aMCI.

Life-Long Aerobic Training Preserves White Matter Integrity: A First Look in the Masters Athlete's Brain

Brain structure and some aspects of cognitive function deteriorate with aging. Maintaining cognitive vitality and preventing Alzheimer's disease (AD) in older adults are urgent public health concerns. Previous studies suggest that aerobic exercise may preserve brain gray and white matter volume in the elderly. However, the effects of aerobic training on white matter integrity are not clear. PURPOSE: To determine the effect of life-long aerobic training on brain white matter integrity in older adults. METHODS: Ten Masters athletes (MA)(7 males, age 73.0±5.6 yrs, aerobic training>15 yrs), and 10 age, sex, and educational level-matched sedentary elderly (SE)(8 males, age 73.9±5.3 yrs) were screened by a rigorous protocol to eliminate neurological and cardiovascular diseases. MRI was performed on a 3T magnet (Philips Medical System). White matter microstructure was measured by fractional anisotropy (FA) and apparent diffusion coefficient (ADC) using diffusion tensor imaging (DTI). FA measures axial water diffusion and higher FA indicates better fiber integrity; ADC measures radial water diffusion and higher ADC indicates poorer fiber integrity. The volume of white matter hyperintensities was measured using fluid attenuated inversion recovery (FLAIR) images. Voxel-based morphometry was conducted using Statistical Parametric Mapping (SPM2) to identify clusters showing significant difference between groups with t-tests. RESULTS: MA demonstrated higher FA in Brodmann area 8 (motor learning, motor control, visuospatial and visuomotor attention) than SE (p=0.005, voxel threshold=200, max T=5.26, max Z=4.04) while SE showed higher ADC in Brodmann 10 (working memory) and 32 (motor planning and inhibition) (p=0.005, voxel threshold=200, max T=4.65, max Z=3.72). FLAIR images did not show significant difference between groups although lower mean WMH volume was observed in MA (3469.5±3584.7mm3) than in SE (6791.0±7041.9mm3). CONCLUSION: These findings suggest that white matter integrity in the brain areas related to motor control, visuospatial function and working memory may be better preserved with life-long aerobic exercise training in older adults.

Single Neuropsychological Test Scores Associated With Rate of Cognitive Decline in Early Alzheimer Disease

Alzheimer disease (AD) characteristically begins with episodic memory impairment followed by other cognitive deficits; however, the course of illness varies, with substantial differences in the rate of cognitive decline. For research and clinical purposes it would be useful to distinguish between persons who will progress slowly from persons who will progress at an average or faster rate. Our objective was to use neurocognitive performance features and disease-specific and health information to determine a predictive model for the rate of cognitive decline in participants with mild AD. We reviewed the records of a series of 96 consecutive participants with mild AD from 1995 to 2011 who had been administered selected neurocognitive tests and clinical measures. Based on Clinical Dementia Rating (CDR) of functional and cognitive decline over 2 years, participants were classified as Faster (n = 45) or Slower (n = 51) Progressors. Stepwise logistic regression analyses using neurocognitive performance features, disease-specific, health, and demographic variables were performed. Neuropsychological scores that distinguished Faster from Slower Progressors included Trail Making Test - A, Digit Symbol, and California Verbal Learning Test (CVLT) Total Learned and Primacy Recall. No disease-specific, health, or demographic variable predicted rate of progression; however, history of heart disease showed a trend. Among the neuropsychological variables, Trail Making Test - A best distinguished Faster from Slower Progressors, with an overall accuracy of 68%. In an omnibus model including neuropsychological, disease-specific, health, and demographic variables, only Trail Making Test - A distinguished between groups. Several neuropsychological performance features were associated with the rate of cognitive decline in mild AD, with baseline Trail Making Test - A performance best separating those who declined at an average or faster rate from those who showed slower progression.

Comparing Acute Symptoms and Recovery Time Following Sports-Related Concussion Across School Age Groups

Abstract Purpose Despite considerable research on pediatric sports-related concussion (SRC), few studies have analyzed groups from school-age through young adulthood. This study aimed to examine acute symptoms (i.e., emotional, physical, and cognitive) and recovery times across this age range. Methods Participants age 5–25 with SRC (n=611) presented to concussion clinics in the North Texas Concussion Registry within 2 weeks of injury. Subjects were stratified into 4 age groups: early elementary (age 5-9; n=19), late elementary (10–13; n=181), high school (14-17; n=384), and college (18-25; n=39). The Patient Health Questionnaire-9 (PHQ-9), General Anxiety Disorder-7 Item Scale (GAD-7), and concussion symptom log (Sx log), were administered. ANOVA was used to compare symptom scores and recovery times across groups. Results Differences were found on PHQ-9 scores (p=.05), with the early elementary and high school groups reporting significantly higher levels of depressive symptoms than the late elementary and college groups. No differences were seen on GAD-7 scores. Differences in Sx log severity scores were nonsignificant (p=.09), although scores increased with age (early: M=21.4, late=25.7, high school=30.0, college=35.5). Lastly, recovery time differed across groups (p=.008), with early elementary participants having the longest recovery (M=8.3 days) and the college group having the shortest (M=5.1 days). Conclusion Older age groups reported higher levels of acute post-concussion symptoms and more rapid recovery compared to younger age groups, suggesting that symptomatology and recovery time vary developmentally and the effects of age warrant consideration in young athletes.

Premorbid intelligence estimation and level of dementia in Alzheimer's disease

Abstract Simple sight-word reading tasks have demonstrated utility in the estimation of premorbid intelligence, although the effects of progressive dementia on such tasks has not been thoroughly examined. The present investigation sought to examine estimated IQ scores from the National Adult Reading Test-Revised (NART-R; Blair & Spreen, 1989) in relation to a WAlS-R-bascd (Wechsler, 1981) estimate of IQ in a series of patients with probable Alzheimer's disease across varying levels of dementia. Results suggest that while NART-R scores do show a decrement with dementia severity, this decline is mild, in contrast to traditionally based IQ scores and other measures of cognitive function, which show more marked declines. Similarly, compared with other tasks, the NART-R showed the strongest correlation with education across the sample as a whole, while the other indices were more related to level of dementia. These findings support the use of measures such as the NART-R in estimating premorbid intellectual functioning in patients at various levels of dementia severity, including those with more advanced cognitive deterioration.
