Information-seeking barriers and strategies in maternity care: a framework analysis of nurses’ goals, plans, and actions

Background There is limited understanding about informational barriers that nurses face when assessing obstetric patients and the strategies they use to obtain and elicit disclosure of potentially sensitive health information. Using framework analysis and goals-plans-actions theory, this study qualitatively explores the perspectives of nurses, nurse practitioners, and midwives on these processes and strategies.Methods This study utilizes framework analysis to analyze in-depth interviews with 22 nurses working in the field of obstetric and intrapartum care.Results Two overarching themes regarding barriers to disclosure and six themes regarding strategies to address these barriers emerged. Barriers included intentional self-concealment and unintentional omission of information. Strategies to overcome these barriers include education, rapport building, strategic questioning, creating a zone of privacy, seeking out information from other sources, and postnatal debriefing sessions.Conclusion This study provides greater insight into the types of barriers that may prevent pregnant people from disclosing their health information, as well as communicative strategies on how to elicit sensitive health information. Nurses’ responses highlight their reliance on patients, as well as on other sources of information when encountering patient reticence. Framework analysis points to four overarching strategic implications for communicative practices in health care settings: creating mutual understanding, creating rapport, creating privacy, and creating more empowered and informed patients.

The effect of an evidence-based intervention on women’s exposure to intimate partner violence.

This study tested the effectiveness of an evidence-based intervention for reducing women's exposure to intimate partner violence (IPV). One-hundred and twenty women who had experienced IPV in the last 2 years were assigned to a treatment or comparison group condition using a sequential assignment paradigm. Treatment group participants completed a 10-session evidence-based intervention, the Mom's Empowerment Program, designed to treat common mental health issues after exposure to IPV and assist women with access to resources. Women were interviewed at baseline and at 6- to 8-month follow-up regarding the level of IPV they had experienced. Violence victimization significantly decreased for women in both conditions between the baseline interview and the 6- to 8-month follow-up. Participation in treatment was related to an augmented effect of violence reduction such that women participating in the intervention experienced greater declines in violence than those who did not participate in the treatment program. These findings support the hypothesis that this treatment model, which combines mental health and advocacy services, is effective in reducing violence revictimization risk for women exposed to IPV.

The consequences of IPV extend beyond the adults in the relationship, as children witness a significant proportion of such violence. Exposure to IPV during childhood has devastating effects across multiple domains of functioning.

Citizens, consumers and the citizen-consumer: articulating the citizen interest in media and communications regulation

The Office of Communications (Ofcom), established by an Act of Parliament in 2003, is a new sector wide regulator in the UK, required to further the interests of what has been termed the ‘citizen-consumer’. Using a critical discursive approach, this article charts the unfolding debate among stakeholders in the new regulatory environment as they attempt to define the interests of citizens, consumers and the citizen-consumer. Ofcom has preferred to align the terms ‘citizen’ and ‘consumer’ so that the interests of both may be met, as far as possible, through an economic agenda of market regulation. Among civil society groups, there is growing concern that the citizen interest is becoming marginalized as the consumer discourse becomes more widespread. We conclude by advocating the development of a positive definition of the citizen interests, distinct from the consumer interests, for the media and communications environment.

“Who’s gonna love a junkie? But he does”: Exploring couples’ identity negotiations and dyadic coping in the context of opioid use disorder

Opioid use disorder (OUD) is a leading cause of accidental death in the U.S., and people with OUD may rely on romantic partners to dyadically cope with their illness. Guided by the communication theory of identity, this study examines the identity tensions experienced by couples managing OUD and how identity negotiations contribute to couples’ efforts to dyadically cope with OUD. Semi-structured interviews were conducted with 19 dyads and 6 individuals ( N = 44), all of whom were in relationships in which at least one partner was managing OUD. Analyses revealed that couples managing OUD experience three types of identity gaps: personal–relational, relational–enacted, and relational–communal identity gaps. These gaps coalesced around a central theme of couple identity paradoxes, which demonstrated the opportunities and potential downsides of dyadically coping with OUD. Findings deepen theoretical understanding of identity negotiation in couples and the role of relational identities in managing illness.

Swift role negotiations: Communicative strategies for enabling rapid role shifts in cross-functional teaming

This field study of a hospital emergency department (ED) examined role negotiations among ED care providers in teaming (i.e., the process of bringing individuals in distinct roles to collaborate as a temporary unit). A grounded qualitative analysis revealed that although a prescriptive role structure enables teaming through role-based coordination, ED providers negotiated one another’s role shifts via brief communicative acts to work through novel situations. Whereas role negotiation has traditionally been theorized as individuals’ attempts to (re)shape their roles throughout their organizational membership, this study showed how roles are negotiated in action quickly and as needed to adapt to emergent needs. Based on the findings, we reconceptualized role negotiation as swift communication situated in short-lived moments of collaboration.
