Topiramate-Induced Somnambulism in a Migraineur: A Probable Idiosyncratic Adverse Effect

Somnambulism (sleepwalking) is a disorder of arousal that falls under "parasomnia" group and is more common in children. These phenomena occur as primary sleep events or secondary to systemic disease or can be drug induced. Medications that can cause sleepwalking include neuroleptics, hypnotics, lithium, amitriptyline, and β-blockers. This report presents an unusual adverse effect of topiramate on sleep in a patient with migraine.

Triad of cluster-like headaches, hypertension and hypercoagulability a pointer to pheochromocytoma

We report the case of a middle-aged hypertensive woman presenting to the neurology department with short-lasting episodic headaches for 4 years. She was initially diagnosed and treated with cluster headaches for one year. Following this, she presented with right lower limb arterial claudication. Arterial Doppler of lower limbs showed thrombosis of the bilateral common femoral arteries. Further computed tomography (CT) angiogram of the lower limbs confirmed extensive arterial thrombosis in bilateral lower limbs. The CT angiogram incidentally detected a left adrenal lesion. She had elevated urinary vanillylmandelic Acid and 24-hour metanephrines suggesting the presence of a pheochromocytoma. She was initially medically managed and later underwent left open adrenalectomy. Histopathology examination of the sections proved pheochromocytoma. Postsurgery, the patient's symptoms improved remarkably. This case highlights the importance of diagnosing pheochromocytoma when you encounter a patient with refractory short-lasting headaches, hypertension and hypercoagulability.

A Case of ANCA Negative Pauci-immune Crescentic Glomerulonephritis in Mixed Connective Tissue Disease

Pauci-immune crescentic glomerulonephritis(CrGN) is one of the most common causes of rapidly progressive glomerulonephritis.The majority of patients with pauciimmune CrGN have circulating antineutrophil cytoplasmic autoantibodies(ANCA). Approximately 10% of systemic vasculitides patients test negative for ANCA. Majority of the ANCA negative pauciimmune CrGN described in the literature have not shown associations with Connective Tissue Diseases. Only isolated case reports of associations with SLE, Scleroderma have been described. We report the case of a 24 yr lady who was diagnosed to have a pauciimmune crescentic glomerulonephritis on renal biopsy ,and was found to be ANCA negative. Subsequently her collagen profile revealed a Mixed Connective Tissue Disease,even though she did not have any other systemic manifestations of the disease at presentation. We presume that antiendothelial antibodies(AECA) may have a role in the pathogenesis of the disease.

T. N. Krishnan writing in a previous issue of “Economic and Political Weekly” put forward a hospitalization insurance plan that is also expected to act indirectly as an income protection plan for the poor. However the ability of this scheme to reach its objectives has to be examined. Medical insurance does not cover even a small proportion of the costs of treatment in the case of many serious illnesses. The alternative therefore is to provide free medical care to the poor. This approach would lead to income protection and poverty reduction much more effectively than a mere hospitalization insurance plan. However this free provision should be restricted only to the impoverished persons. Thus a two-tier system of health care finance consisting of two distinct categories is considered: 1) free care for the poor; and 2) paying care for the non-poor. Unfortunately though health care in India is not the priority of any political party. It is also not feasible given the country’s fiscal constraints of the country. However the two-tier system would ensure equity and efficiency and would promote a just social order.

Early clinical exposure through a vertical integration programme in physiology

Early clinical exposure is likely to be beneficial during the preclinical year. This pilot programme aimed to define a learning framework of introducing first year medical students to early clinical exposure through a vertical integration programme in the physiology course. The intent was to enhance the understanding of theoretical concepts and practical applications of physiology. Student and faculty perceptions were evaluated.First year MBBS students (n = 60) had bedside clinics conducted by 5 clinical departments, where the clinical context and applied physiological concepts were emphasized. Clinical sessions were synchronized with pedagogic highlights on related physiological concepts. The student's perceptions were recorded through a semi-structured questionnaire, while qualitative feedback was obtained from the faculty.All students ( 100%) reported that the programme was relevant and did not interfere with their physiology course. Most (91%) appreciated the synchronization of classroom knowledge with clinical exposures, and thought that integrated teaching helped in better understanding of practical applications of physiology (94%) with adequate discussions during the sessions (91 %). Fifty-nine (98%) students preferred integrated teaching over the isolated traditional classroom teaching. The clinicians involved also felt that the early clinical exposure was relevant. The students achieved a mean (SD) score of 13 (2.98) of 25 in the quiz consisting of multiple-choice questions that attempted to test their learning through early clinical exposures.This pilot exercise showed the utility of early clinical exposure integrated into the physiology course. It will be worthwhile to replicate this exercise at other institutions and among a larger student strength.

Medical Research and the ‘Sugar Pill’ Debate

Sugar pill is the popular term for Placebo, an entity which has been the subject of much debate over the last several decades. Well known is the ‘Placebo Effect’ where a treatment with no demonstrated therapeutic effect ends up alleviating the symptoms of the illness and at times even resulting in cure. Placebo treatments are most commonly employed by clinicians when they perceive the cause of the symptoms to be more psychosomatic than actual pathology. When the placebos work, everyone’s happy, when they don’t, usually no harm is done and the clinicians switch to standard regimes.
