Impact of concomitant aortic valve replacement in patients with mild‐to‐moderate aortic valve regurgitation undergoing left ventricular assist device implantation: EUROMACS analysis

Abstract Introduction Left ventricular assist device (LVAD) therapy may lead to an aortic regurgitation, limiting left ventricular unloading and causing adverse events. Whether concomitant aortic valve replacement may improve outcomes in patients with preoperative mild‐to‐moderate aortic regurgitation remains unclear. Methods A retrospective propensity score‐matched analysis of adult patients with preoperative mild‐to‐moderate aortic regurgitation undergoing durable LVAD implantation between 01/01/2011 and 30/11/2021 was performed. Patients undergoing concomitant valve surgery other than biological aortic valve replacement were excluded, resulting in 77 with concomitant biological aortic valve replacement and 385 without. Results Following 1:1 propensity score matching, two groups of 55 patients with and without biological aortic valve replacement were obtained, (mean age 59 ± 11 years, 92% male, 59.1% HeartWare). Aortic regurgitation was mild in 72.7% and 76.4% and moderate in 27.3% and 23.6% in non‐replacement and replacement cohorts respectively. The 30‐day survival was 89.1% vs. 85.5% ( p = 0.59), 1‐year survival 69.1% vs. 56.4% ( p = 0.19), and 2‐year survival 61.8% vs. 47.3% ( p = 0.10) in the non‐replacement and replacement groups, respectively. After a mean follow‐up of 1.2 years, non‐replacement patients had a higher incidence of pump thrombosis (11 [20%] vs. 3 [5.5%], p = 0.022) and fewer major bleedings (2 [3.6%] vs. 11 [20%], p = 0.008). Conclusion Compared with those treated conservatively, patients with mild‐to‐moderate aortic regurgitation undergoing concomitant aortic valve replacement during LVAD implantation have a similar survival up to 2 years on support. Patients with concomitant valve replacement had a higher risk of bleeding complications but fewer pump thromboses.

Bioprosthetic Total Artificial Heart Implantation Does Not Induce Chronic Inflammation

The Aeson total artificial heart (A-TAH) has been developed for patients at risk of death from biventricular failure. We aimed to assess the inflammatory status in nine subjects implanted with the A-TAH in kinetics over one year. Laboratory assessment of leukocyte counts, inflammatory cytokines assay, and peripheral blood mononuclear cell collection before and after A-TAH implantation. Leukocyte counts were not significantly modulated according to time after A-TAH implantation (coefficient of the linear mixed effect model with 95% CI, −0.05 (−0.71 to −0.61); p = 0.44). We explored inflammatory cytokine after A-TAH and did not observe, at any time, a modified profile compared to pre-implantation values (all p -values > 0.05). Finally, we compared the distribution of circulating immune cell subpopulations identified based on sequential expression patterns for multiple clusters of differentiation. None of the population explored had significant modulation during the 12-month follow-up (all p -values > 0.05). In conclusion, using a cytokine multiplex assay combined with a flow cytometry approach, we demonstrated the absence of inflammatory signals in peripheral blood over a period of 12 months following A-TAH implantation.

Platelet activation with the Aeson Bioprosthetic Total Artificial Heart: insight from aspirin treatment and outcomes

Abstract Introduction The Aeson bioprosthetic total artificial heart (A-TAH) is a pulsatile and autoregulated device. The aim of this study was to evaluate the level of platelet activation secondary to A-TAH implantation. Methods We examined the level of platelet activation markers in adult patients receiving A-TAH support (n=16) during clinical follow-up by quantifying sP-selectin (sP-sel) and sCD40L in plasma. Results The cumulative duration of A-TAH support was 3587 days. Before implantation, sCD40L was 2684.6 pg/mL (954.0-16706.1) and remained steady after implantation [3305.8 pg/mL (1234.2-12327.5), 3300.5 pg/mL (1041.5-8370.1), and 2560.0 pg/mL (1325.5-14039.5), respectively, at <3 months, 3–6 months and >6 months; non-significant difference along time-period]. sP-sel was 33997.0 pg/mL (16019.6-73377.6) and remained steady after implantation [33580.1 pg/mL (13979.8-53395.2), 33204.9 pg/mL (15332.6-67263.4), and 34684.5 pg/mL (14084.9-49206.0), respectively, at <3 months, 3–6 months and >6 months; non-significant difference along time-period]. Levels sP-sel and sCD40L analysed according to aspirin or heparin use did not change. Finally, no relation existed between pericardial effusion and aspirin use according to timing of aspirin start and drain removal. Conclusions We demonstrated that A-TAH does not induce significant platelet activation. Absence of relationship between aspirin and platelet activation levels or pericardial effusion may support potential for withdrawal of antiplatelet therapy after A-TAH implantation in the future.

The European Registry for Patients with Mechanical Circulatory Support of the European Association for Cardio-Thoracic Surgery: third report

In the third report of the European Registry for Patients with Mechanical Circulatory Support of the European Association for Cardio-Thoracic Surgery, outcomes of patients receiving mechanical circulatory support are reviewed in relation to implant era.Procedures in adult patients (January 2011-June 2020) were included. Patients from centres with <60% follow-ups completed were excluded. Outcomes were stratified into 3 eras (2011-2013, 2014-2017 and 2018-2020). Adverse event rates (AERs) were calculated and stratified into early phase (<3 months) and late phase (>3 months). Risk factors for death were explored using univariable Cox regression with a stepwise time-varying hazard ratio (<3 vs >3 months).In total, 4834 procedures in 4486 individual patients (72 hospitals) were included, with a median follow-up of 1.1 (interquartile range: 0.3-2.6) years. The annual number of implants (range: 346-600) did not significantly change (P = 0.41). Both Interagency Registry for Mechanically Assisted Circulatory Support class (classes 4-7: 23, 25 and 33%; P < 0.001) and in-hospital deaths (18.5, 17.2 and 11.2; P < 0.001) decreased significantly between eras. Overall, mortality, transplants and the probability of weaning were 55, 25 and 2% at 5 years after the implant, respectively. Major infections were mainly noted early after the implant occurred (AER<3 months: 1.44 vs AER>3 months: 0.45). Bilirubin and creatinine levels were significant risk factors in the early phase but not in the late phase after the implant.In its 10 years of existence, EUROMACS has become a point of reference enabling benchmarking and outcome monitoring. Patient characteristics and outcomes changed between implant eras. In addition, both occurrence of outcomes and risk factor weights are time dependent.

Percutaneous CoreValve aortic valve implantation-initial clinical experience in high-risk surgical patients at IKEM

Úvod: Perkutánní implantace aortální chlopně (PAVI) představuje možnost léčby pro nemocné s významnou aortální stenózou, kteří mají neúměrně vysoké operační riziko. Popisujeme naše první zkušenosti s technikou perkutánní implantace samoexpandabilní bioprotézy CoreValve a krátkodobé (třicetidenní) sledování nemocných.Metody a výsledky: V období prosinec 2008-červen 2009 bylo provedeno 21 perkutánních implantací aortální chlopně (systém CoreValve ReValving® třetí generace) u nemocných s těsnou aortální stenózou. Pacienti (8 mužů a 13 žen) měli průměrný věk 80,9 ± 4,8 let; průměrné hodnoty logistického EuroSCORE byly 17,5 ± 6,9 %, střední aortální gradient 53,6 ± 23 mm Hg, plocha aortálního ústí 0,43 ± 0,09 cm2/m2 a průměrná ejekční frakce 51,5 ± 9,4 %. Technická úspěšnost výkonu byla 100% a v průběhu prvních 30 dnů jsme neměli žádnou závažnou komplikaci (MACE - úmrtí, infarkt myokardu, mozková příhoda - 0 %). Došlo k významnému poklesu aortálního středního gradientu na konečných 3,0 ± 2 mm Hg. Významně se zlepšil klinický stav hodnocený klasifikací NYHA, z průměrných hodnot 3,0 ± 0,5 před výkonem na 1,4 ± 0,4 za 30 dní po výkonu.Závěry: U nemocných s významnou aortální stenózou a vysokým operačním rizikem představuje perkutánní implantace aortální chlopně systému CoreValve ReValving® výkon s vysokou úspěšností a přijatelným periprocedurálním rizikem s jednoznačným klinickým a hemodynamickým zlepšením. Nezbytný je při výkonu týmový přístup a zkušenost katetrizujícího.
