We report a case of recurring foreign bodies in the urinary bladder. A 67-year-old male inserted a foreign body into the urinary bladder during masturbation. Eight months later, he experienced a fever and went to a hospital where ultrasonography revealed a foreign body in his urinary bladder. Then, he was referred to our hospital for surgical treatment. The patient's surgical record indicated that he had undergone the same operation 3 times in the past thirty years. The inserted foreign body was successfully removed by suprapubic cystotomy, and he was discharged 13 days after the operation. He was also evaluated by psychiatrists, but they diagnosed that he had no mental disorder. To our knowledge, this is the first report on the removal of a foreign body in the urinary bladder four times in the same patient.
A 65-year-old man had been hospitalized for examination of melena at a separate facility. Computed tomography (CT) showed left renal mass and he was referred to our hospital. Magnetic resonance imaging (MRI) revealed a tumor without adipose components in the perirenal space. Preoperative diagnosis was retroperitoneal malignant tumor. Tumor excision and left nephrectomy were performed. Pathologic evaluation revealed inflammatory liposarcoma. He has been followed up for 10 months with no additional treatment and no evidence of local recurrence was seen.
Background/Objectives: The accurate prediction of postoperative renal function (post-RF) in living kidney donors is essential for optimizing donor safety and long-term health. After nephrectomy, renal function can be significantly altered, owing to the functional adaptation of the remaining kidney; however, the extent of this has not been investigated. This study aimed to examine how various donor factors affect functional adaptation after nephrectomy, and to develop a new predictive model. Methods: In total, 310 patients who underwent donor nephrectomy were included. Preoperative split renal function (pre-SRF) of the remaining kidney was measured. Post-RF was measured 1 month after surgery. The functional adaptation rate was calculated from the difference between pre-SRF and post-RF. Multiple regression analysis was performed to develop a predictive formula for post-RF, incorporating donor age and pre-SRF. Results: The median age of the donors was 60 years, and 38.7% were men. The median pre-SRF was 36.4 mL/min/1.73 m2. The median functional adaptation rate was 26.8%, with donor age, pre-SRF, and a history of hyperuricemia (HUA) being significant predictors of the functional adaptation rate. The equation for post-RF was established as 0.94 × pre-SRF − 0.12 × age + 18.87 mL/min/1.73 m2. The estimated post-RF showed a high coefficient of determination (R2 = 0.76), with a mean bias of –0.01 mL/min/1.73 m2. Conclusions: Donor age, pre-SRF, and HUA are key predictors of renal functional adaptation after nephrectomy. The developed formula accurately estimates post-RF, supporting clinical decision-making and donor counseling for living kidney donations.
A 96-year-old woman was diagnosed with colitis in our hospital. Her past history was breast cancer and lung cancer but no diabetes mellitus. She suddenly complained of lower abdominal pain. Abdominal computed tomography (CT) revealed intramural gas in the bladder wall and multiple poor contrasting area in her right kidney, which suggested emphysematous cystitis and acute focal bacterial nephritis. Indwelling urethral catheter was performed for bladder drainage and the treatment with antibiotics started. Urine culture revealed Klebsiella pneumoniae and blood culture was negative. After 3 weeks, the abdominal CT confirmed the decrease of gas within the bladder wall and improvement of contrasting area in the right kidney.
Organ ischemia reperfusion injury (IRI) is unavoidable in kidney transplantation and induces reactive oxygen species and causes organ damage. Although the effectiveness of molecular hydrogen for IRI has been reported, oral ingestion of hydrogen rich water and inhalation of hydrogen gas are still not widely used in clinical settings because of the lack of efficiency or difficulty in handling. We succeeded in generating a large amount of hydrogen molecules by crushing silicon (Si) to nano-sized particles and reacting with water. We orally administered the nano-sized silicon particles or relatively large-sized silicon particles to renal IRI rats. Rats were divided into four groups: sham-operated, IRI, IRI + nano Si, and IRI + large Si group. We observed that serum creatinine and urine protein significantly decreased 72 hours following IRI in rats that were administered nano Si particles. Serum malondialdehyde and urinary 8-hydroxydeoxyguanosine, which are oxidative stress markers, were also significantly decreased with the nano Si treatment. Transcriptome and gene ontology enrichment analysis showed that the oral intake of nano Si downregulated the biological processes related to oxidative stress, such as immune response, cytokine production, and extrinsic apoptotic signaling. Regulation of some genes in these pathways was validated by quantitative polymerase chain reaction. Furthermore, immunohistochemistry staining demonstrated that the nano Si treatment alleviated the interstitial macrophage infiltration and the tubular apoptosis, suggesting anti-inflammatory and anti-apoptotic effects. In conclusion, renal IRI was attenuated by oral administration of the nano-sized silicon particles, which is a novel hydrogen administration method. Grants-in-aid for Scientific Research (18K16697). A 34-year-old male came to us with congenital spina bifida, vesicoureteral reflux and scoliosis. He had been on hemodialysis for chronic renal failure caused by reflux nephropathy since the age of 23 years. At the age of 24, he received bilateral nephrectomy and underwent living renal transplantation from his mother. Hemodialysis was started again at the age of 26, because the renal graft was not functioning. At the age of 34, the patient developed a fever that persisted for a few days. He received antibiotic medication from his physician, but since his condition did not improve he was refered to our hospital. A computed tomography scan examination revealed abscess formation in the left retroperitoneum. Magnetic resonance imaging findings also showed the abscess in the left retroperitoneum. The patient was diagnosed with empyema of the residual ureter and underwent a left ureterectomy procedure.
To evaluate the influence of prior abdominal surgery on the outcomes after robotic-assisted laparoscopic radical prostatectomy (RALP).We retrospectively analyzed patients with prostate cancer who underwent RALP between June 2012 and February 2015 at our institution. Patients with prior abdominal surgery were compared with those without prior surgery while considering the mean total operating, console, and port-insertion times; mean estimated blood loss; positive surgical margin rate; mean duration of catheterization; and rate of complications.A total of 203 patients who underwent RALP during the study period were included in this study. In all, 65 patients (32%) had a prior history of abdominal surgery, whereas 138 patients (68%) had no prior history. The total operating, console, and port-insertion times were 328 and 308 (P=0.06), 252 and 242 (P=0.28), and 22 and 17 minutes (P=0.01), respectively, for patients with prior and no prior surgery. The estimated blood losses, positive surgical margin rates, mean durations of catheterization, and complication rates were 197 and 170 mL (P=0.29), 26.2% and 20.2% (P=0.32), 7.1 and 6.8 days (P=0.74), and 12.3% and 8.7% (P=0.42), respectively. Furthermore, whether prior abdominal surgery was performed above or below the umbilicus or whether single or multiple surgeries were performed did not further affect the perioperative outcomes.Our results suggest that RALP can be performed safely in patients with prior abdominal surgery, without increasing the risk of complications.
Steroid sulfatase (STS) deficiency is one of the causes of ichthyoses. STS genes on the X chromosome is responsible for this disease. Therefore, STS deficiency is also called X-linked ichthyosis. Herein we report a case of STS deficiency complicated by bilateral undescended testis. A5-year-old-boy with STS deficiency was referred to our hospital because of bilateral undescended testis. We performed bilateral orchiopexy.
