The relationship between Cormic Index and uterine contractions’ pattern in the active phase of the first stage of labour

Uterine contractions are indicators of labour progress. The relationship between anthropometric indices and types of childbirth has been investigated. One of these indices is referred to as the Cormic Index, which provides an estimation of the sitting height and leg length. This cross-sectional study was performed to determine the relationship between the Cormic Index and the uterine contractions' pattern in the active phase of the first stage of labour. The standing and sitting heights of 150 pregnant women were measured to calculate the Cormic Index. Then, two Cardiotocographic traces were recorded during the 3 to 5 and 6 to 8 centimetres' dilations, by which the uterine contractions' pattern were determined. The mean Cormic Index was 51.84 ± 2.62. The results indicated that the Cormic Index had a positive linear correlation with MTIME (frequency of contractions) in the first monitoring episode and SDTIME (regularity of contractions) in the first and the second monitoring episode, which was significant (p < .05). However, there was no significant correlation between the Cormic Index and F:R ratio (shape of contractions) in both monitoring episodes. With the rise of the mother's Cormic Index, the frequency of contractions in the acceleration phase, and also their regularity in the acceleration phase and the phase of the maximum slope, were decreased during the active phase of labour.IMPACT STATEMENTWhat is already known on this subject? Uterine Contraction as an indicator of labour progress enforces cervical dilation. If uterine activity is analysed through electronic methods, one could assess labour outcome through some patterns of the contractions. Anthropometry provides a quick determination of the body proportions. The Cormic Index provides an estimation of the upper and lower body length. Several studies have investigated the relationship between the anthropometric indices and the types of childbirth. Despite the various approaches available, there is no objective means of precisely distinguishing the fact that whether labour can be successful in effecting vaginal delivery or not and diagnosis of dystocia still relies on the trial of labour.What do the results of this study add? The results of this study add to the growing body of research on the progression of labour that Cormic Index has a positive linear correlation with the frequency of contractions in acceleration phase, and with the regularity of contractions in the acceleration phase and the phase of maximum slope.What are the implications of these findings for clinical practice and/or further research? The results can help professionals to evaluate the progress of labour based on the type of uterine contractions in the latent phase or the early active phase of labour using the Cormic Index.

The Relationship of Emotional intelligence with Women's Post-abortion Grief and Bereavement

Background & aim: Emotional intelligence (EQ) is one of the factors influencing post-abortion grief and bereavement. This study aimed to determine the relationship between the level of EQ and post-abortion grief and bereavement in women referred to the hospitals affiliated to Mashhad University of Medical Sciences. Methods: This correlational study was conducted on 60 women with spontaneous abortion admitted in Department of Obstetrics and Gynecology of Imam Reza, Ghaem, and Omolbanin hospitals, Mashhad, Iran in 2016. The subjects were selected through convenience sampling and data were collected using a self-structured questionnaire, Bar-On model of socio-emotional intelligence, grief intensity scale, and perinatal Grief and Bereavement Scale. Data analysis was performed using descriptive statistics, simple linear regression, and Pearson correlation coefficient in SPSS software version 11.5. Results: The subjects’ mean age was 28.6±5.7 years old and 73.3% (n=44) of them were housewives. The mean scores of post-abortion grief, bereavement, and EQ were 126.5±26.7, 42.7±9.4, and 188.4±17.9, respectively. According to the results of linear regression analysis, the EQ adversely and significantly predicts the post-abortion grief (P=0.001, β=-43.22) and bereavement (p <0.001, β=-462/0). Given the results of Pearson correlation coefficient, there was an indirect and significant relationship between the EQ and post-abortion grief (P=0.001, r=-432) and bereavement (p <0.001, r=-0.462). Conclusion: Considering the effect of EQ on post-abortion grief and bereavement, it is recommended to provide an educational program to promote the level of EQ in women.

Effect of cream containing green tea extract and pedophylline solution on the process of reducing genital wart lesions

Introduction: Genital wart is a common sexually transmitted infection which impairs the patient's quality of life. One of the plants which is mentioned in traditional medicine for the treatment of genital warts is green tea extract. Considering the need for new and improved treatments for external anogenital warts, as well as the emphasis on the use of home and herbal medicines and its prevalence in families and its use by the patient and saving the health costs due to the lack of need to refer to physician, this study was performed with aim to compare the effect of green tea extract cream with pedophylline solution on the reduction of genital wart lesions. Methods: This clinical trial study was performed in 2018-2019 on 76 women with genital warts. The subjects were randomly divided into the intervention and control groups. In the intervention group, green tea extract cream was used 3 times a day and in the control group, pedophiline solution was used once a week. The patients in both groups referred every two weeks until 12 weeks or complete recovery of lesions and the number of dimensions and percentage of reduction of lesions were recorded in the lesion assessment form. The validity and reliability of the tool were confirmed by the views of the experts and Cronbach's alpha coefficient r=89. Data were analyzed by SPSS software (version 22). P Results: The number and dimensions of lesions were significant between the two groups of green tea extract and pedophylline in weeks 4, 6 and 8 after the intervention compared with before the intervention (p <0.001) and showed better results in the pedophylline group. But, according to the results of Mann-Whitney test, no statistically significant difference was found between the two groups in the number and size of lesions during the 10 weeks after treatment (p <0.001). The number and dimensions of lesions were significant between the two groups during 10 weeks of intervention compared to before the intervention (p <0.001). Conclusion: The rate of healing of lesions after the end of treatment in the green tea group was the same as pedophylline, so considering the success of the results of the present study, it is recommended to use this product as a suitable alternative to pedophylline in the treatment of genital warts

Comparison of Sexual Problems in Fertile and Infertile Couples

Introduction: Infertility is known to have a negative effect on couple's life and in most cases it has a profound impact on sexual relations. Sexual problems may be the cause of infertility or may arise as a result of infertility. The aim of this study was to compare the sexual problems in fertile and infertile couples. Methods: This cross-sectional study was performed on 110 infertile and 110 fertile couples referring to Montaserieh infertility center and five health centers in Mashhad which were selected as class clustering method and easy method. Data collection tools included demographic questionnaires and Golombok-Rust Inventory. The collected information was analyzed by SPSS software and descriptive and inferential statistics. Results: No significant difference was found between fertile 26 (17, 37) and infertile 26(18, 37) women in terms of total score of sexual problems and other aspects of sexual problems (except infrequency). The women in the fertile group had higher infrequency than infertile women. Total score of sexual problems in fertile men was 18.5 (13, 27) and in infertile men 25 (19, 31) and the difference was statistically significant. Infertile men reported more problems in no relation, impotency and premature ejaculation compared to fertile men. Men in both fertile and infertile group reported more sexual problems than women. Conclusion: In view of the more frequent sexual problems in infertile men than infertile women, it seems that it is necessary to pay more attention to sexual aspects of infertility in men and design the training programs for sexual and marital skills in infertility centers.

Comparison of the Impacts of Auriculotherapy with Electrical Stimulation and Vaccaria Seeds on Colposcopy Pain: A Randomized Clinical Trial

Background: Colposcopy is an invasive medical procedure, which involves pain and discomfort. Auriculotherapy is a medical technique that can facilitate the reduction of pain. Aim: The aim of this study was to compare the impacts of auriculotherapy with electrical stimulation and Vaccaria seeds on colposcopy pain. Method: This controlled randomized, clinical trial was conducted on 93 females undergoing colposcopy in Ghaem Hospital, Mashhad, Iran, in 2017. The study population was selected through convenience sampling technique, and then randomly assigned into three groups, namely transcutaneous auricular stimulation (TAS), Vaccaria seeds, and placebo. The TAS group was subjected to electrical stimulation at four auricular acupuncture points 40 min before colposcopy, using a Pointer Excel stimulator (2 Hz). In the Vaccaria seed group, the seeds were fixed on the same acupoints three days prior to colposcopy. The participants were asked to press each point for 1 min, three times a day. The placebo group were provided with three adhesive patches without any seeds. After colposcopy, pain intensity was monitored using the visual analog scale. Data analysis was performed using Kruskal-Wallis and exact Chi-square tests in SPSS software, version 22. Results: The three groups were comparable in terms of the demographic data (P>0.05). The mean pain intensities in theTAS, seed, and placebo groups were 49.0±24.2, 49.5±26.0, and 65.1±26.6, respectively. A significant difference was observed among the three groups regarding the pain intensity (P=0.02). There were also significant differences between the TAS and placebo groups (P=0.01) and between seed and placebo groups in this respect (P=0.02). However, the TAS and seed groups showed no significant difference in this regard (P=0.94). Implications for Practice: The use ofauriculotherapy with TAS and seeds could effectively reduce the patient’s pain during colposcopy.

مقدمه: خونریزی پس از زایمان، یک وضعیت اورژانسی مامایی و یکی از شایع‌ترین علل مرگ‌و‌میر مادران در سراسر جهان می‌باشد که پیشگیری از آن می‌تواند بر کاهش مرگ‌و‌میر و عوارض تأثیرگذار باشد. یکی از گیاهانی که در طب سنتی به‌عنوان بندآورنده خونریزی به آن اشاره شده است، بارهنگ می‌باشد. مطالعه حاضر با هدف تأثیر شیاف رکتال بارهنگ بر میزان خونریزی پس از زایمان زنان در معرض خطر خونریزی انجام شد. روش‌کار: این مطالعه کارآزمایی بالینی تصادفی یک سوکور در سال 1397 بر روی 70 زن باردار واجد شرایط پژوهش مراجعه‌کننده جهت زایمان واژینال به بیمارستان ام‌البنین (س) شهرستان مشهد انجام شد. در این مطالعه، واحدهای پژوهش شامل زنان پرخطر زایمان کرده بودند که طبق نوموگرام پیشگویی احتمال خونریزی پس از زایمان، نمره مساوی یا بیشتر از 10 را کسب کرده بودند. افراد به‌طور تصادفی در دو گروه 35 نفری بارهنگ و کنترل قرار گرفتند. در هر دو گروه بعد از تولد نوزاد، اقدامات مطابق روتین بیمارستان و انفوزیون 30 واحد اکسی‌توسین در هزار سی‌سی کریستالوئید صورت گرفت. در گروه مداخله، بلافاصله پس از خروج جفت و پرده‌های جنینی و ماساژ رحمی، اولین دوز شیاف رکتال بارهنگ 120 میلیگرمی و سپس دوزهای بعدی با فاصله هر نیم ساعت تا 5 دوز توسط پژوهشگر قرار داده شد. میزان خونریزی پس از زایمان با استفاده از توزین کیسه جمع‌آوری خون و پد به مدت 4 ساعت پس از زایمان، توسط پژوهشگر اندازه‌گیری و ثبت شد. تجزیه و تحلیل داده‌ها با استفاده از نرم‌افزار آماری SPSS (نسخه 16) و آزمون‌های تی مستقل، من‌ویتنی و کایاسکوئر انجام گرفت. میزان p کمتر از 05/0 معنی‌دار در نظر گرفته شد. یافته‌ها: میانگین خونریزی در پایان 4 ساعت اول پس از زایمان در گروه کنترل 2/11±2/306 سیسی و در گروه بارهنگ 2/14±3/253 سیسی بود که بین دو گروه اختلاف آماری معنی‌داری وجود داشت (001/0=p). نتیجه‌گیری: شیاف رکتال بارهنگ می‌تواند به‌عنوان یک داروی مؤثر بر کاهش میزان خونریزی بعد از زایمان در زنان در معرض خطر خونریزی توصیه شود و می‌توان از آن به‌عنوان یک روش کمکی در کاهش خونریزی در زنان در معرض خونریزی بعد از زایمان استفاده نمود.

Sexual dysfunction in infertile women

Sexual problems have different effects on the life of people by influencing their interpersonal and marital relationships and satisfaction. Relationship between sexual dysfunctions and infertility can be mutual. Sexual dysfunction may cause difficulty conceiving but also attempts to conceive, may cause sexual dysfunction.This paper compares sexual dysfunction in fertile and infertile women.In this cross-sectional study, 110 infertile couples referring to Montasarieh Infertility Clinic and 110 fertile couples referring to five healthcare centers in Mashhad were selected by class cluster sampling method. Data collection tools included demographic questionnaire and Glombok-Rust Inventory of Sexual Satisfaction. Data were analyzed through descriptive and analytical statistical methods by SPSS.There was no significant difference in total score of sexual problems and other dimensions of sexual problems (except infrequency) in fertile 28.9 (15.5) and infertile 29.0 (15.4) women. Fertile women had more infrequency than infertile women (p=0.002).There was no significant difference between fertile and infertile women in terms of sexual problems. Paying attention to sexual aspects of infertility and presence of programs for training of sexual skills seems necessary for couples.

The Relationship between Sexual Assertiveness and Positive Feelings towards Spouse in Married Women

Background & aim: Stability and survival of marital life depends on the love and positive feelings towards spouse in married couples. Sexual assertiveness is considered as a person's ability to have intercourse to meet his/her sexual needs and promote sexual relationships with the spouse. The more couples are aware of each other's sexual needs and can satisfy those needs, the more desirably their emotional bond are developed. This study was conducted to determine the relationship between sexual assertiveness and positive feelings towards spouse in married women. Methods: This correlational study was conducted in 2016 on 60 women visiting Imam Reza Medical Center in Mashhad, Iran. The subjects who were selected through convenience sampling were married and involved in a heterosexual marriage for a minimum of one year. Data was collected using Halbert Index of Sexual Assertiveness and Positive Feelings Questionnaire. To analyze the data, Pearson correlation coefficient, multivariate regression, Mann-Whitney U test, and Chi-squared test were run in SPSS, version 16. Results: Higher sexual assertiveness scores were associated with more positive feelings towards spouse. The multivariate regression analysis reflected a significant positive correlation between sexual assertiveness and positive feeling toward spouse (r=0.5, p <0.01). Conclusion: Sexual assertiveness and positive feelings towards spouse are significantly associated, and they affect couples’ emotional and sexual relationship. Considering mutual agreement and sexual rights among couple, providing sexual care and consultancy services are recommended for married women to improve their sexual assertiveness and positive feelings towards their husband.
