Stenotrophomonas maltophilia is increasingly being isolated as a nosocomial pathogen. It is the third most common non-fermenting gram negative bacilli (NFGNB) isolated from the clinical samples especially from the respiratory specimens, blood, urine and pus samples. Though inherently resistant to many groups of antibiotics, its susceptibility to co-trimoxazole is a peculiar character, because of which Co-trimoxazole is considered as the drug of choice for treating these infections. Recently there has been an increase in the incidence of resistance against Co-trimoxazole with variable resistance towards other antibiotics. In the present study 14.2% of the isolates were resistant to Co-trimoxazole. Resistant to other antibiotics was also very high. Ciprofloxacin and Chloramphenicol have shown reasonably good activity against this organism making these drugs as therapeutic alternatives in Co-trimoxazole resistant cases
To report a case of a young female who presented with scotoma in the right eye for few days.Krill's disease or acute retinal pigment epithelitis (ARPE) is a self-limiting retinal disease with no specific treatment. Typical clinical and imaging features helped us to diagnose her with ARPE. Intravenous methylprednisolone (IVMP), which gives a rapid anti-inflammatory response, was advised. An SD-OCT scan post-injection showed a reduction in hyperreflectivity and height of lesion at day 3 and near total resolution by day 5.This case suggests rapid resolution of ARPE with the use of IVMP.
Pyogenic liver abscess (PLA) is a potentially fatal disease. Klebsiella pneumoniae (K. pneumoniae) has replaced Escherichia coli (E. coli) as the predominant causative organism for pyogenic liver abscess. Over the years a unique form of community-acquired invasive K. pneumoniae infection of the liver has been well described in Southeast Asia. This has recently been linked to a virulent hypermucoviscous K. pneumoniae phenotype and to a specific genotype, rmpA positive. To our knowledge, we report the first case of PLA with bacteremia and meningitis in a Guyanese patient with the presence of rmpA-positive K. pneumoniae with laboratory evidence in North America.
