Corticosteroids in Croup: Further Testimony

Like Doctor Coffin, whose letter you printed in the September issue, we have practiced for 10 plus years in a three-man group which has together treated all of the croup cases in a very busy practice. The difference is we do not use corticosteroids in croup. There have been no fatalities and the only tracheostomies have been in cases of epiglottitis (possibly three).

Microwave-Induced Hyperthermia Dose Definition

In vitro thermal data on cytotoxicity are consistent with the simple picture of chemical reaction kinetics as governed by an activation energy. These kinetics are used to calculate, for any arbitrary heating profile used in clinical hyperthermia, the corresponding percentage of cells killed by such treatment in in vitro tissue cultures. The quantity calculated, which incorporates biological response to thermodynamic parameters, is suggested as a measure of hyperthermal dosage. Alternative dosage measures are discussed. Doses, defined by thermal cytokinetics, are derived for current clinical practice in whole-body and Iocal hyperthermia. Both types of treatment, although superficially very different, are shown to employ comparable dose magnitudes, and these magnitudes are found to be in quantitative accord with the thermal cytotoxic basis for dosage measurement.

Assessment of current hyperthermia technology.

The current state of the art in techniques for the production and control of whole-body and local hyperthermia is discussed. Techniques of radio-frequency, microwave, and ultrasonic local hyperthermia are considered and compared. Problems of thermometry and control system integration are defined, and requisite biophysical data requirements are established. A bibliography is provided to serve both as an introduction to the hyperthermia literature for researchers from other fields and as a convenient compendium of original sources for scientists currently engaged in hyperthermia research.

Peter was born by normal vaginal delivery at 32 weeks gestational age weighing 2.49 kg. This was the first pregnancy for his 22-year-old mother who had no significant past medical history and had been well throughout pregnancy. Antenatal ultrasound at 20 weeks showed no fetal abnormality, a well grown fetus and mild polyhydramnios. The pregnancy was unremarkable until the onset of spontaneous labour at 32 weeks. Amniotic membranes remained intact until just before delivery. Cardiotocographic monitoring was within normal limits prior to delivery and there was no history of vaginal bleeding. Peter cried immediately following cephalic vaginal delivery; his heart rate was more than 100 beats/min and he did not require any resuscitation. By 10 min of age Peter was noted to be dusky and was grunting. He was given intermittent positive pressure ventilation (IPPV) and his colour improved. He was transferred from the delivery suite to the special care baby unit in facial oxygen and on arrival was saturating at 95% in 30% oxygen with a respiratory rate of 40 breaths/min and a temperature of 36.9°C. On examination the admitting senior house officer (SHO) noted that despite being pink with an oxygen saturation of 95% Peter continued to grunt and had subcostal recession and nasal flaring while in 30% ambient oxygen. His systolic blood pressure (BP) was 60 mm Hg, capillary refill time (CRT) was <2 s and his heart sounds were normal with no murmur audible; abdominal examination was also normal. A capillary blood gas at 40 min of age showed: pH 7.099, pCO2 8.75 kPa, BE −9.5, HCO3 20.6 mmol/l. The SHO advised continuing oxygen and repeating the capillary blood gas in 1 h. Can a social service register of children with disabilities be used for planning services?

ABSTRACT Objective To compare a health and a social service register of children with disability in order to assess their potential for planning services. Design Descriptive. Setting Children with disability resident in North Tyneside, a Local Government Authority in England. Main variables studied Disability and its severity. Results There were 1692 children on the health register and 83 on the social service register. Only 20% of the children with the most severe disability were on the social service register. There was good agreement between parents' and paediatricians' rating of severity for the children with the most severe disability. Although the health register had details of all children with disability, it did not record details of current needs. Conclusions The social service register did not provide data about all children with disability, and in particular only listed 20% of the children with the most severe disability. Therefore, the social service register does not appear suitable for providing data for overall planning of services, although it is helpful in describing and co‐ordinating the care of a smaller number of children and families whose problems were active, complex and unresolved, and where social services had an active role.

Induced hyperthermia in sedated humans and the concept of critical thermal maximum

The concept of critical thermal maximum (CTM) has been defined in the literature as the minimal high deep-body temperature that is lethal to an animal. In man the CTM has been estimated at 41.6--42.0 degrees C. Data are presented for sedated, unacclimatized, well-hydrated men heated 1 h at esophageal temperatures of 41.6--42.0 degrees C, without sequelae, except for modest elevation of serum enzymes in two of five patients. These data when combined with other observations in the literature suggest that CTM be redefined as the particular combination of exposure time at elevated body temperatures that results in either subclinical (CTM)s) or clinical (CTMc) injuries. Also presented is a mathematical technique, equivalent time at 42 degrees C (Teq 42 degrees), for expressing hyperthermia in terms of body temperature and exposure time.
