Topic: 13. Myeloma and other monoclonal gammopathies - Biology & Translational Research Background: The immunomodulatory drugs (IMiDs) have made a profound difference in the treatment of multiple myeloma (MM). Lenalidomide (LND) is chiral compound with two enantiomers and one enantiomer may have different pharmacologic action than the other. For a given chiral drug, until proven otherwise, it is right to think of the two enantiomers as two different drugs with unique properties. Aims: This study aimed to explore efficacy of lenalidomide (R) - (+) and (S) - (-) enantiomers in human multiple myeloma cell line. Methods: H929 myeloma cell line model was designed to understand the potential differences in chirality of the R and S form of LND enantiomers and racemate in multiple myeloma cell line. We report the first biological evaluation of lenalidomide in racemic and in both (R)- and (S)-enantiomerically pure forms against (in vitro) H929 cells of multiple myeloma (MM) using an annexin V assay. Results: It was observed that at 48 hours S-LND treated cells demonstrated significant reduction in the metabolic activity at 10 and 20 µg/mL and profound morphological changes. Cell cycle analysis showed cells in S and G2/M phase were decreased suggesting that all of the drugs can arrest cells at G1 cell cycle phase. We demonstrate (S) - (-) enantiomers of lenalidomide inhibited the growth of H929 MM cells without any in-vivo activation. Summary/Conclusion: The study concludes that (S) - (-) enantiomers of lenalidomide noted more potent agent which could have both the anti-cancerous and immunomodulatory activity.Keywords: Imids, Cell line, Multiple myeloma

Association Between Aspirin Resistance and Blood Lipid Profile in Patients of Ischemic Stroke: A Meta-Analysis

In ischemic stroke patients, aspirin resistance has been linked to an altered lipid profile. This study was undertaken to explore the association of blood lipid profiles with aspirin resistance in ischemic stroke patients. Relevant studies published before November 2022 in PubMed, Medline, EMBASE, and Google Scholar were chosen for this study. Random effects models were used to assess pooled standardized mean and standard deviation (SD) with a 95% confidence interval (CI). RevMan 5 and STATA version 13.0 software were used for statistical analysis. Subjects from eight studies consisting of aspirin resistance and sensitive data were considered in this meta-analysis. Our findings suggested a significant association between the elevated levels of TG in Aspirin resistance groups as compared to non-resistance (SMD 0.83, 95% CI 0.30 to 1.95, p-value < 0.001). However, the meta-influence analysis suggested no significant association when a study with a large effect size was excluded. The results of our comprehensive meta-analysis suggest none of the lipid parameters were significantly associated with aspirin resistance. Further studies might be required to support the findings of this meta-analysis.

AI driven ECG arrhythmia diagnosis

The accurate and timely diagnosis of cardiac arrhythmias is crucial for effective patient management and improved health outcomes. However, the precise identification of arrhythmias in electrocardiogram (ECG) data often requires specialized medical expertise, leading to potential delays and errors in diagnosis. To address these challenges, this project introduces an AI-driven system for ECG arrhythmia diagnosis. Employing advanced deep learning techniques, the proposed system leverages a comprehensive dataset of annotated ECG recordings to train a robust model capable of detecting and classifying various types of arrhythmias. The model is designed to process raw ECG signals, extract relevant features, and generate clinically meaningful insights, enabling automated and rapid identification of arrhythmic patterns. Through a user-friendly interface, medical professionals can upload ECG data for real-time analysis, allowing for prompt decision-making and personalized patient care. Furthermore, the system offers interpretable results, highlighting key indicators and providing detailed explanations to aid clinicians in understanding the diagnostic outcomes.

Design of Reasoning Agent for Cognitive Radio Networks

The main task for a cognitive radio system is to adapt to the transmission parameters in a dynamically changing environment.The nature of wireless channels and the number of transmission parameters to be optimized in various layers add more complexity to the adaptation process.In these situations Case Based Reasoning (CBR) agent along with optimization algorithm provides a robust solution.CBR Agent develops cases as the changes occur in the environment and acquires an understanding of the system.As new situations occur, experience from the previously developed cases is taken into consideration and a solution to the system is provided.In real time with training, the Cognitive Engine (CE) learns the variations occurring in the environment and utilizes the previously existing cases to provide new solution.As well as if abruptly some unexpected situation is encountered then the optimization algorithm is initiated and a solution is designed as per the need of the environment and this case is also added to the case list for future reference.Hence variations in the environment are mapped extensively and the optimization process becomes more efficient.In the present work case based reasoning along with particle swarm optimization is implemented.

A review of potential pharmacological treatments of COVID-19: an evidence-based perspective

Coronaviruses (CoVs) typically manifest as mild to severe respiratory tract infections. No drug is approved by US food and drug administration (FDA) for the treatment of patients with coronaviruses infection. With growing COVID-19 pandemic globally, need of hour is to work on potential prophylactic and therapeutic drugs to prevent local and community transmission. A literature search for eligible studies published till March 2020 was conducted in the PubMed, Medline, EMBASE, OVID, and Google Scholar databases by two reviewers. Therapeutic efficacy and safety of different drug regimens targeting treatment pathway acting against corona virus-2019 (COVID-19) were reviewed. Possible mechanism of actions of these potential repurposed drugs against COVID-19 were reviewed to develop effective prevention and treatment strategies. Many potential pharmacological therapies are being studied in various clinical trials. No FDA-approved repurposed drugs have shown safety and efficacy in randomized controlled trials for patients with COVID-19. Vaccines are under development and only few vaccines are under clinical evaluation. This review highlights potential drug actions against COVID-19 and their safety issues. It could help researchers and physicians to use these potential agents judiciously in clinical trials as well as in treatment protocols.

Background: COVID-19 has been declared as a pandemic recently and has caused many deaths worldwide. Till date, no effective drug or vaccine is available against SARS-CoV-2. There is an urgent need to find effective alternative preventive and treatment strategies to deal with the SARS-CoV- 2 outbreak. Objective: This communication proposes a new potential drug combination (repurposed) for prophylaxis and treatment of SARS-CoV-2. Methods and Materials: We performed a brief review of literature on a combination of Hydroxychloroquine, Melatonin and Mercaptopurine for prophylaxis and treatment of Novel COVID-19 infection and also assessed their possible mechanism of action against SARS-CoV-2. Observation: Proposed combination seems to be safe and target is unlikely to develop resistance to this combination. Conclusion: This scientific review proposes potential repurposed drugs and their combination targeting SARS-CoV-2. Conclusion: This scientific review proposes potential candidate repurposed drugs and potential drug combinations targeting 2019-nCoV/SARS-CoV-2.
