To the Editor.

—I noted with interest 3 articles^1-3dealing with adverse drug reactions (ADRs) and their consequences. Since the hospital to which I admit my patients has started what they describe as a mandatory ADR reporting system, I have 2 points. First, in our institution, trivial events are required to be reported as an ADR. If a cancer patient receiving chemotherapy has nausea or loses his or her hair, that is an ADR; if an asthmatic patient develops mild hyperglycemia from receiving steroids, that is a reportable ADR; if a patient is given antacids for an upset stomach and develops a mild case of selflimited diarrhea, that is a reportable ADR. There seems to be little rational consensus on what constitutes an ADR. The second point is that physicians are in a double bind. If we do not make a selective report to the pharmacy about a

Introduction

Recreational drug use (RDU) in a sexualised context (“chemsex”) is increasing amongst MSM regardless of HIV status and is associated with poor sexual health outcomes. National guidelines for both PEP and HIV management suggest regular sexual health screens (SHS) and screening for RDU and alcohol use.

Aim(s)/objectives

To audit the documentation of SHS and RDU history from HIV positive MSM attending routine outpatient clinics compared to HIV negative MSM accessing PEP.

Methods

Retrospective review of 45 randomly selected MSM attending routine HIV care (HIV+ group) OR receiving PEP (PEP group) at each of 2 London clinics during 2014/15.

Results

Discussion

High levels of STIs and RDU were seen in both groups but most significantly in the PEP group. This highlights the importance of identifying RDU/chemsex in PEP patients, which may be an opportunity for intervention to reduce risk of acquisition of HIV through risky sex.

Introduction of a nurse-led sexual health service for HIV-positive men

A nurse-led clinic offering screening for sexual infections to men within an existing HIV outpatient service was created. A retrospective case-note review was undertaken of those having a sexual health (SH) screen between May and December 2007. A total of 125 men had an SH screen, 117 identified as men who have sex with men and 84 were asymptomatic. Six (7%) asymptomatic men had chlamydial and two (2%) had gonococcal infection. Six new diagnoses of syphilis and two of hepatitis C virus infection were made. We suggest that introducing a nurse-led clinic offering SH screening within HIV services has helped to achieve national standards and improved the SH of our patients.

Summary Cases of neurological deficit following cannulation of the internal jugular vein in adults are reported. From a consideration of the techniques and topography the possibility of damage to the ansa subclavia and recurrent laryngeal nerve is highlighted.

Use of Foam Bolsters for Securing Facial Skin Grafts

Many methods of securing skin grafts to their recipient beds have been described. A review of the senior author's experience with facial skin cancers over a two-year period revealed that reconstruction with full-thickness skin grafts was employed in 60 of more than 500 cases. This series of 60 cases was reviewed. Uses of foam as a bolster has proven to be a simple, time-conserving and reliable technique that produces consistently excellent results with a low complication rate. The technique is reviewed, along with its advantages and disadvantages.
