Umbilical catheters (UC) have become the most common way to access neonatal veins and arteries especially after the increasing the number of preterm newborns. Significant clinical complications have been reported with this increased usage of UC. We present a rare complication of umbilical catheterization in a newborn with asphyxia that had meconium discharge after UC.

Effectiveness of Permanent Implantable Catheter (Polysite) in Children with Cancer

Abstract Background Totally implantable central venous access devices (ports) have been available for over 10 years, but have not been achieved widespread use in pediatric oncology patients. Ports facilitate the administration of chemotherapy in children with cancer. Materials and Methods In this study, we investigated early complications of implantable central venous access devices in children who suffered from different type of cancer. All of the complications were recorded by staff nurses by checklist for one week. This study included 68 patients with different cancer (lymphoma-leukemia-sarcoma and wilms’ tumor) who were treated between April 2007 and November 2011 in oncology department of Dr Sheikh hospital, Mashhad University of medical science. Results Venous ports were placed in 26 (38.2%) girls and 42 (61.8%) boys aged between 2 and 12 years (mean: 6 years).We implanted all of the venous ports in patients for chemotherapy, and port implantation procedures were performed by one experienced Pediatric Surgery. 3 cases (4.4%) have needle access site infections which were controlled with starting of antibiotics. Catheter leakage in 3 cases (4.4%), port-catheter disconnection in 4(5.8%) cases, Occlusion of the system in 5 cases (7.4%). In this period, there were no major complications. Conclusion With proper placement technique and adequate nursing care, they represent a definite improvement in child cancer therapy. Ports can provide satisfactory for the majority of pediatric oncology patients, with a low risk of line-related complications and a high degree of acceptability to children and their parents.

Genetic Background of Hirschsprung Disease: A Bridge Between Basic Science and Clinical Application

ABSTRACT Hirschsprung's disease (HSCR) is a congenital disorder, defined by partial or complete loss of the neuronal ganglion cells in the intestinal tract, which is caused by the failure of neural crest cells to migrate completely during intestinal development during fetal life. HSCR has a multifactorial etiology, and genetic factors play a key role in its pathogenesis; these include mutations within several gene loci. These have been identified by screening candidate genes, or by conducting genome wide association (GWAS) studies. However, only a small portion of them have been proposed as major genetic risk factors for the HSCR. In this review, we focus on those genes that have been identified as either low penetrant or high penetrant variants that determine the risk of Hirschsprung's disease. J. Cell. Removal of foreign body (bottom of pen) in bronchus: An innovative technique

Foreign body (FB) aspiration in children is still a global medical problem. It is responsible for the accidental mortality in children. Non-food FB such as pen components is more frequent in the older children. In this case report, we present a challenging case with a bottom of a pen aspiration and describe a method for bronchoscopic extraction using similar aspirated foreign body.

Ultrasound- guided Hydrostatic (Hydrocolonic) Treatment of Meconium Ileus: A Preliminary Report

Background Meconium ileus (MI) is one type intestinal obstruction due to inspissated meconium at the level of the terminal ileum. The purpose of this study is to present the non-ionizing ultrasound-guided approach for the treatment of meconium ileus. Materials and Methods This study was performed, prospectively, at Dr. Sheikh and Akbar children's Hospitals, Mashhad University of Medical Science (Mashhad city, Iran) from March 2017 to March 2018. The 21 newborn patients with suspicious meconium ileus were referred to Radiology department for contrast enema and ultrasound examination. The hydro colonic ultrasound-guided treatment was considered for four cases with simple form of meconium ileus. Ultimately, plain abdominal radiography was taken to documentation of entrance of solution to dilated proximal loop and evaluation of probable complications. Results: In ultrasound exam, air bubbles are floating in fluids on proximal dilated loops, and no air is seen in bowel loops contain inspissated meconium. Ultrasound showed a collapsed microcolon in addition to inspissated meconium as a hypoechoic beaded intraluminal material in comparison to adjacent bowel mucosa. During ultrasound-guided enema, overt distention of unused colon and ileum was seen with coating and detachment of meconium plaques from bowel wall and floating and movement of the plaques toward the dilated proximal loops. The successful treatment in our study was occurred in two-third of cases. Conclusion Ultrasound-guided hydrostatic (hydro colonic) treatment can be a non-ionizing nonsurgical procedure for successful treatment of newborns with uncomplicated meconium ileus.

Circumcision, can this procedure be dangerous for the patient?

Circumcision is one of the most common surgical operations, which is usually a safe and simple procedure with rare complications. Amputation of glans is an uncommon condition but has definitive risk of complication. It's crucial to consider patients based on their age and anatomical conditions, as well as undertaking well sterile surgery for preventing any future complications of circumcision. In this study we have investigated a complete glans amputation during the circumcision that was initially repaired and the result was good.

Is the perianal fistula a variation of anal duplication? Case reports and review

Anorectal malformations are birth defects which include a wide range of diseases. With this defect, the anus and rectum don't develop properly. They can involve both genders. The involved organs in such malformations are anus, rectum, urinary and genital systems. The defects vary from very easy to very difficult curable. The incidence of these birth defects is about 1 in 5000 live births. On the other hand, perianal fistula and abscess formation are common perianal disease in children. The symptoms of perianal fistula are: secretion in the perianal area, pruritus and bleeding. Among alimentary tract duplications, anal canal duplication is the rarest and the more distal one, which has been defined as duplication along the posterior side of the anal canal, with a perineal orifice located behind the anus at 6 o'clock position and its physical aspect is duplicated anus. In this study, three cases were reported with anal duplication presenting a condition similar to perianal fistula. Reviewing these cases guide us to a new horizon that some fistulas or abscess formation in early age or in infancy might be a complication of underlying anal duplication. The proposed management for perianal fistula is fistulectomy.

The genetic factors contributing to hypospadias and their clinical utility in its diagnosis

Abstract Hypospadias is among the most common congenital malformations in male neonates. It results from abnormal penile and urethral development, but is a multifactorial disorder that is highly heterogeneous, with several genetic and environmental determinants. Monogenic and chromosomal abnormalities are present in approximately 30% of cases, although the genetic factors contributing to hypospadias remain unknown in 70% of cases. While defects in androgen synthesis can lead to this malformation, mutational analyses have shown several genes, such as sonic hedgehog, fibroblast growth factors, bone morphogenetic proteins, homeobox genes, and the Wnt family, are involved in the normal development of male external genitalia. Mutations in the genes of penile development (e.g., HOX, FGF, Shh) and testicular determination (e.g., WT1, SRY), luteinizing hormone receptor, and androgen receptor have also been proposed to be implicated in hypospadias. Here we review the recent advances in this field and discuss the potential genes that could determine the risk of hypospadias.
