Scrub typhus infection presenting as acute heart failure: A case report and systematic review of literature of cardiopulmonary involvement in scrub typhus infection

We describe a middle aged previoulsy healthy female patient who presented with clinical features suggestive of acute heart failure. Investigations revealed very high NT pro-BNP, right heart enlargement, bilateral pulmonary alveolar edema and bilateral pleural effusion. In view of falling platelet counts and exudative pleural effusion inflammatory/infective causes were considered. Her Weil Felix test was strongly positive and IgM for scrub typhus also returned positive. She was started on doxycycline to which there was dramatic improvement. Thus in this case scrub typhus infection presented as acute right heart failure and the cause seemed elusive at the outset. We also systematically reviewed the existing literature on cardio-pulmonary manifestations of scrub typhus infection.

Evaluation of kit-based loop-mediated isothermal amplification (TB-LAMP) assay in the diagnosis of tubercular lymphadenitis

Background: The rapid and accurate diagnosis of tubercular lymphadenitis remains a challenging task today.WHO endorsed LoopAMP™ MTBC kit (TB-LAMP) as a replacement for sputum-smear microscopy in the diagnosis of pulmonary tuberculosis (PTB). However, no prospective diagnostic accuracy study of TB-LAMP in tubercular lymphadenitis in adults has been done yet. Current study evaluated the diagnostic performance of TB-LAMP in tubercular lymphadenitis (LNTB).Material & Methods: In a prospective observational study conducted at a tertiary care hospital in India, 90 subjects (age>18years) suspected of LNTB were consecutively recruited and followed up till six months between January 2019 and December 2020. Samples were processed for microscopy, culture, GeneXpert®, histopathology and TB-LAMP. Sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of TB-LAMP against composite reference standard (CRS) and culture were determined.Results : TB-LAMP showed a sensitivity of 83.78% (95% CI, 73.76-90.47) and specificity of 81.25% (95% CI, 56.99-93.41) respectively against CRS. The PPV and NPV were 95.38% (95% CI, 87.29-98.42) and 52.00% (95% CI, 33.50-69.97) respectively. TB-LAMP showed a sensitivity of 88.89% (95% CI, 71.94-96.15) and specificity of 36.17% (95% CI, 23.97-50.46) respectively against culture. The PPV and NPV were 44.44% (95% CI, 32-57.62) and 85% (95% CI, 63.96-94.76) respectively.Conclusion: TB-LAMP can be used over conventional microscopy for the diagnosis of TB in lymph node specimens at primary health care centres. This is the rapid and cost-effective diagnosis of LNTB in resource-limited settings due to good sensitivity and NPV.

Weaning difficulty in a near hanging patient: An unusual cause

Suicidal hanging causes damage to the airways, neck blood vessels as well as soft tissue injuries. We report the development of tracheo-esophageal fistula in such a patient. Recurrent soiling of the airways and the resultant lung infection led to weaning failure. We highlight the approach to diagnosis and appropriate management in such a patient.

ACE2 Expression in Lungs of Severe COVID-19 Infection: A Study on Minimally Invasive Post- mortem Tissue Samples

Abstract Angiotensin-converting enzyme 2 (ACE2) is a key host protein by which severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) enters and multiplies within cells. The level of ACE2 expression in the lung is hypothesised to correlate with an increased risk of severe infection and complications in COVID-19 (COrona VIrus Disease 2019). To test this hypothesis, we compared the protein expression status of ACE2 by immunohistochemistry (IHC) in post-mortem lung samples of patients who died of severe COVID-19 and lung samples obtained from non-COVID-9 patients for other indications. IHC for CD61 and CD163 were performed for assessment of platelet-rich microthrombi and macrophages, respectively. IHC for SARS-CoV-2 viral antigen was also performed. Quantification of immunostaining, random sampling, and correlation analysis was used to substantiate the morphologic findings. Our results show that among a total of 44 COVID-19 post-mortem lung tissues and 15 lung biopsies in non-COVID-19 patients included, ACE2 protein expression was significantly higher in COVID-19 patients than in controls, regardless of sample size. Histomorphology in COVID-19 lungs showed diffuse alveolar damage (DAD), acute bronchopneumonia, and acute lung injury with SARS-CoV-2 viral protein detected in a subset of cases. ACE2 expression levels positively correlated with increased expression levels of CD61 and CD163. In conclusion, our results show significantly higher ACE2 protein expression in severe COVID-19 disease, correlating with increased macrophage infiltration and microthrombi, suggesting a pathobiological role in disease severity.

A study on the morbid histopathological changes in COVID-19 patients with or without comorbidities using minimally invasive tissue sampling

Introduction: COVID-19 causes morbid pathological changes in different organs including lungs, kidney, liver,etc especially in those who succumb. Though clinical outcomes in those with comorbidities are known to be different from those without – not much is known about the differences at histopathological level. Aims: It was to compare the morbid histopathological changes in COVID-19 patients between those who were immunocompromised(Gr 1), malignancy(Gr 2) or had cardiometabolic conditions (hypertension, diabetes or coronary artery disease)(Gr 3). Methods: Post-mortem tissue sampling (MITS) was done from the lungs, kidney, heart, and liver using biopsy gun within two hours of death. Routine (H & E stain) and special stains (AFB, SM, PAS) were done besides immunohistochemistry. Results: A total of 100 patients underwent MITS and data of 92 were included (immunocompromised: 27, maligancy:18, cardiometabolic conditions:71). Within lung histopathology, capillary congestion was more in those with malignancy while others like diffuse alveolar damage, microthrombi, pneumocyte hyperplasia etc was equally distributed. Within liver, architecture distortion was significantly different in immunocompromised while steatosis, portal inflammation, Kupffer cell hypertrophy, confluent necrosis were equally distributed. There was a trend towards higher acute tubular injury in those with cardiometabolic conditions as compared to the other groups. No significant histopathological differences in heart was discerned. Conclusion: Certain histopathological features are markedly different in different groups (Gr 1,2 and3)of COVID-19 patients with fatal outcome.
