Parietal and occipital hair loss patterns in initial stages of androgenic alopecia in men

Background. The initial stages of androgenetic alopecia in men are characterized by a variety of clinical manifestation in the parietal or occipital scalp regions. However, the differences in the pathogenesis of hair loss patterns are not well understood, selective treatment has not been developed. Aims. Trichological characteristics of patients with initial stages of androgenetic alopecia with identification of genetic and non-genetic factors that determine the hair loss in the different scalp regions, and their response to conservative therapy. Materials and methods: Trichograms were photodocumented using an AramoSG microcamera (Republic of Korea). The genetic factor was analyzed by mini-sequencing of single nucleotide polymorphisms rs929626, rs5919324, rs1998076, rs12565727, rs756853. The non-genetic factors study included hormonal status (total and free testosterone, dihydrotestosterone, 17OH-progesterone, dehydroepiandrostenone, SHBG) and blood content of trace elements (Mg, Ca, Zn, Cu, Se, Fe) and vitamins (B12, D, E, folic acid). Conservative treatment was topical application of a minoxidil 5% (twice-a-day, 4 months) and a micronutrient deficiencies personalized correction. Results. The study included 47 man with initial stages of androgenetic alopecia. Their trichological examination showed two patterns, consisting a predominant decrease in the hair density and diameter in the parietal and occipital scalp regions, which were subgroups formation criteria. An intergroup comparison revealed similar genetic risk, while hormonal parameters (increased dihydrotestosterone levels, a decrease in free testosterone) characterized the subgroup with parietal hair loss pattern. Also, a multiple deficiency of Zn, Cu, Se and vitamins B12, D, folic acid was detected in all patients. Subsequent conservative treatment had a positive effect in patients with parietal hair loss pattern, while in patients with occipital pattern no significant response was observed. Conclusions. The study develops ideas about differences between androgen-dependent parietal and androgen-independent occipital hair loss patterns in the initial stages of androgenetic alopecia, which requires different approaches to their conservative therapy.

The impact of mitigation measures against COVID-19 on the incidence of pertussis and its evaluation using the ARIMA model

The aim of the study was to determine the impact of the novel coronavirus disease 2019 (COVID-19) mitigation measures on the spread of pertussis and identify which measures were most effective in preventing the spread of this disease. In order to analyze pertussis incidence dynamics in Moscow (Russian Federation), the quasiexperimental interrupted time series (ITS) approach was used. The mathematical modeling of the incidence time series was conducted with regression with autoregressive integrated moving average (ARIMA) errors. COVID-19 mitigation measures reduced the incidence of pertussis infection compared with the predicted incidence without restrictive measures of 42.7 %. Our analysis has shown that the most efficient COVID-19 mitigation measures influencing pertussis incidence were «restriction of mass events», «self-isolation regime for the elderly persons over 65 years of age and persons with chronic diseases» and «transfer to remote work of at least 30 % of the total number of employees». The obtained results of this research can be used for the selection of the most efficient mitigation measures to reduce the incidence and limit the spread of pertussis outbreaks.

The role of infectious agents and the gut microbiome in the development of the autistic spectrum in children

The article presents the results of studies of microbiome features in patients with autism spectrum disorders. The role of the microbiota as a part of the enteral nervous system influencing the dynamics of autism symptoms is considered. The links between eating behavior and the severity of autistic disorders, increased permeability of the gastrointestinal mucosa and hypoxia of brain cells, the nature of intestinal dysbiotic disorders, the severity and clinical manifestations of autism are shown. Taking into account the presented data, optimal approaches to the diagnosis and therapy of autism spectrum disorders are shown, which can significantly improve the quality of life of patients.

[Oral cavity microbiocenosis assessment on the basis of bacterial endotoxin and plasmalogens in a saliva by method GAS-liquid chromatography-mass spectrometry.]

The concentration of plasmalogen bacterial and endotoxin levels in the saliva of patients with different severity of periodontal disease, injury prosthetic bed and with various degrees of the oral cavity microbiocenosis violations was studied. Determination of the presence of the pathological process was carried out clinically, according to the condition of periodontal tissues. The degree of microbiological disorders was assessed by the quantitative ratio of the types of microorganisms isolated from the smear taken from the gingival groove. It was found that the concentration of plasmalogen for normal microbiocenosis is not less than 0.7 µg/g. For the intermediate type of microbiocenosis, the concentration of 1.82 µg/g was determined; for dysbiosis - 5.64 µg/g, and for the expressed violation of the microbial composition accompanied by inflammatory processes - 6.54 µg/g. An increase in the concentration of bacterial endotoxin (be) more than 6.25 nanomole/g indicates the pronounced inflammatory process, regardless of the determined intensity of contamination of opportunistic gram-negative microflora.

Plasma Zinc Levels in Males with Androgenetic Alopecia as Possible Predictors of the Subsequent Conservative Therapy’s Effectiveness

Androgenic alopecia (AGA) is the most common type of progressive hair loss in man. The search for reliable predictors of the conservative treatment's effectiveness is an urgent problem today. Forty-eight patients with AGA, stages I-IV by the Norwood-Hamilton scale, were treated for 4 months with 5% topical minoxidil joints with corrections for trace element and vitamin imbalances. In most cases, the positive therapy's effect was shown in the parietal but not in the occipital area, whereas that effect was observed in others. The attempts to associate the therapy's effectiveness with initially defined genetic, hormonal, and metabolic parameters showed the absence of differences between groups with positive and negative outcomes. Among the studied nutrient parameters (Zn, Cu, Mg, Ca, Fe, and Se, as well as vitamins B12, E, D, and folic acid), differences between these groups was shown in zinc content only. The starting point from a zinc plasma level above 10 µmol/L likely provides the success of the subsequent conservative therapy and correlates with an increase in the hair density and diameter in the parietal area. The integral predictive value of the Zn plasma level was assessed as 72.3% (positive predictive value: -88%; and negative predictive value: -55%).

Clinical and microbiological features and metabolic activity of fecal microorganisms in the rehabilitation of children with post-COVID-19 syndrome

The significant occurrence of a cohort of children who do not achieve complete recovery after the acute period of coronavirus infection has drawn the attention of researchers to elucidate the etiopathogenetic reasons for the lack of process termination. Objective. To study clinical-anamnestic and microbiologic parameters of fecal samples as possible predictors of the prolongation of the post-COVID state on the basis of laboratory tests performed at admission to the sanatorium at different time periods after COVID-19 infection. Patients and methods. In 61 children aged 4 to 17 years, a comprehensive clinical and laboratory investigation was performed in the rehabilitation of the post-COVID-19 condition. Results. The main complaints on admission: rapid fatigability, decreased appetite, decreased tolerance of habitual physical and mental exertion, tendency to constipation, increased frequency of episodes of acute respiratory diseases. In the microbiota of fecal excretions the reduced level of high-grade Escherichia coli, bifido- and lactobacilli was established. Inhibition of functional activity of microbiota was noted. Abnormal amount of fatty components was detected at scatoscopy. General health-improving measures promoted clinical and laboratory recovery processes. Conclusion. Commonly accepted recreational sanatorium activities contribute to the rehabilitation of children with post-COVID condition and can be recommended as a mandatory program. Characteristics of TLR reactions to the urogenital infections pathogens in pregnancy

Simultaneous TLR gene expression estimation (the prognosis of abortion and miscarriage), mucosal microbiocenoses types, mucosal immunity state estimation, presence or absence of clinical manifestation of miscarriage, pregnancy course (unfavorable prognosis-immunological rejection of the fetus or complicated pregnancy course and miscarriage, favorable prognosis-immunological irritation or normal fetal development, abortion-miscarriage), presence or absence of infection and/or clinical manifestation of infectious and inflammatory conditions can be objective additional criteria for estimation and prognosis of pregnancy course as well as to personalize the adequate treatment and prevention of abortion and miscarriage. Pathogenetic (concomitant increased reaction of mucosal epithelial cells receptors for developing fetus and infectious process leads to abortion and miscarriage) and TLR diagnostic role in pregnancy is established. In obstetric infections in pregnant women levels of gene expression of TLR-2 – 21.2 and above, TLR-4 – 23.0 and above, TLR-8 – 25.7 and above indicate a high degree of infection with clinical manifestations of urogenital infections, and in the levels of genes expression of TLR-8 - 26 and below verifies chronic urogenital infections, while levels of gene expression TLR-8 above 26 verifies acute course of urogenital infections TLR-8 level above 28 is a predictor of pregnancy termination and miscarriage.
