To review our experience in treating thoracolumbar fractures with Moss Miami System and vertebra anterior distraction device(ADD).Twenty-eight patients with thoracolumbar fractures admitted in the Department of Trauma and Reconstructive Surgery (Unfallkrankenhause, Berlin, Germany) were treated by posterior fixation with Moss Miami system and anterior decompression, followed by autologous bone grafting and ADD stabilizing implant.Death occurred in none of the cases and no patients exhibited signs of neurologic damages both intraoperatively and postoperatively, with the only exception that disruption of the dura mater of the spinal cord occurred in 1 case intraoperatively. Follow-up study lasting for a mean period of 11.8 months was conducted in all the cases, which found that the position of Moss Miami system and implanted ADD remained unchanged in 24 cases, in comparison with their position as shown by postoperative imaging data. In 3 cases, however, ADD was inlaid in the vertebral body, with an angle correction loss of which was less than 5 degrees. Lateral obliquity and displacement of ADD was found in 1 case. Of the 5 incomplete paraplegic patients, 3 achieved complete habilitation of neurologic function and 2 had partial improvement.Moss Miami system and ADD constitute good modalities in treating thoracolumbar fractures, with such merits as thorough decompression of the spinal canal, reliable fixation to allow early force loading with less correction loss of the angle, and are therefore suitable for treating unstable thoracolumbal fractures.
The efficacy of application of Hilus pulmonic in guiding of kyphosis correction was explored by radiographic parameters and the quality of life scores of AS kyphosis patients. A retrospective analysis was performed with patients that were diagnosed as ankylosing spondylitis with kyphosis and using the hilus pulmonic to guide kyphosis correction. A total of 16 cases, 15 males and 1 female, were enrolled. The spine-pelvic imaging parameter, the SRS-22 score and ODI dysfunction score were recorded and analyzed at pre-operation and post-operation. The post-operation spine-pelvic parameters PT, SS, GK, TLK, LL, SVA and TPA were all improved compared with preoperative parameters (P < 0.05). SRS-22 score and ODI score were significantly improved after osteotomy (P < 0.05). After using the Hilus pulmonic to guide kyphosis correction, patients with AS kyphosis can obtain good radiographic parameters and physical activity ability score, and can meet the functions of head-up, standing, sitting and lying in patients after orthopedic surgery. PT、SVA、GK and TPA have significant influences on the quality of life of patients with AS kyphosis. Correcting spine-pelvic imaging parameters is important guarantees of higher quality of life after surgery.
Mesenchymal stem cells are multipotent adult stem cells,which can be chemoattracted to tumor region and metastatic regions,experiments show that mesenchymal stem cells can promote the malignant behavior of breast cancer,such as malignant proliferation,metastasis and recurrence,and they can actively participate in blood vessel formation or provide immune protection and so on,their research in breast cancer were reviewed as below.
To study the therapeutic effect of open reduction and internal fixation in the management of old vertical unstable pelvic fractures.In the 9 cases with surgical treatment for pelvic fractures, combination of the anterior and posterior approaches was adopted.In the follow-up lasting for 6-30 months, all pelvic fractures were reduced satisfactorily. The 9 patients were able to walk and 8 of them gained good clinical recovery without complications.In the management of old vertical unstable pelvic fractures, a stable pelvis can be reconstructed by effective open reduction and internal fixation through combination of the anterior and posterior approaches, so that further sequelae can be reduced.
AIM: To investigate the effects of Chinese medicine, Jinan injection, on ultrastructure and mitochondria in cultured lung cancer cell lines. METHODS: The cultured lung cancer cell lines PG and PAa were used and divided into 4 groups: control (C), cisplatin (DDP), Jinan (JA) and Jinan in combination with cisplatin (DJ), respectively. The changes of morphology and mitochondria membrane potential, intracellular Ca~(2+) and pH in every group were observed by inverted microscope and electronic microscope as well as by using flow cytometry, staining by rhodamine, Fluo-3 and BCECF, respectively. RESULTS: Degeneration cells showed chromatin condensation and peripheral congregation. In cytoplasm autophage lysosome increased and myelinoid body was seen easily. In mitochondria structure, where the space between the inner and outer membranes of these organelles expanded as the matrix was compressed. The electron-dense or swelled was observed as vacuole degeneration and its matrix showed electron-lucent. Compared to control, mitochondria membrane potential increased in every group after 24 h and 48 h treatment. DDP increased intracellular calcium ion in PG cells, however, in PAa cells, JA and DJ decreased it. Intracellular pH got lower at 24 h and higher at 48 h in PG and PAa cells. There were significance in every group vs control in PG and PAa by statistic t-test (P0.01). CONCLUSION: Apoptosis was induced in PG and PAa cell lines by Jinan injection and DJ. Mitochondria matrix displayed electron-dense, mitochondrial potential, intracellular calcium ion and pH showed an increasing trend. Mitochondria damages may play an important role in apoptosis induced by Jinan and DJ.
OBJECTIVES: To discuss the methods and outcome of shorting proximal femoral and total hip arthroplasty for Crowe IV dysplastic hip of adults. METHODS: From July 2000 to February 2006, 13 cases of osteoarthritis secondary to severe development dysplastic hip were treated by total hip replacement and the shorting proximal femoral. RESULTS: The duration of follow-up ranged from 4 months to 55 months. The average score increased from 36.9 to 84.1 points after the surgery according to Harris. All the patients could walk independently. Their paces were improved obviously and the function of their hips was satisfactory. CONCLUSIONS: The treatment by total hip arthroplasty and the shorting of posterior femoral is effective and efficient for osteoarthritis secondary to Crowe IV development dysplastic hip in adults. The long-term followup is necessary for further study.
With the aging of the global population, osteoporosis has become a widespread phenomenon in the society. With the rapid increase of the number of osteoporotic fractures, a huge social and economic pressure has been caused. At the same time, it has attracted extensive attention. Bisphosphonates (BPs) can effectively prevent bone loss, improve bone density, and effectively reduce the incidence of osteoporotic fracture in the vertebral body and hip, which is the first choice for the treatment of osteoporosis. However, after tens of thousands of people take bisphosphonates for a long time, there are more and more reports of atypical femoral fractures (AFFs) caused by bisphosphonates, which cause the concerns of patients and doctors. The long-term use of bisphosphonates can promote the apoptosis of osteoclasts, inhibit the osteoclast's dissolution and absorption of bone trabecula, then reduce bone turnover, inhibit bone remodeling, lead to the accumulation of bone micro damage, and finally lead to fracture AFFs. We present an 83 year old woman who has been taking bisphosphonates for more than 10 years. The images indicate that there is nonunion in the middle and upper segments of both femurs. She was diagnosed as bilateral AFFS and was treated with plate and screw internal fixation for the left femur fracture and temporary conservative treatment for the right femur fracture. She was treated with anti infection, analgesia and circulation improvement after operation and the symptoms were relieved obviously and the effect was satisfactory. The case was reported as follows and reviews the related literature.
VACTERL association is an acronym that includes vertebral anomalies (V), anal atresia (A), cardiac defects (C), tracheoesophageal fistula (TEF) or esophageal atresia (EA), renal anomalies (R), and limb defects (L). Rib abnormalities have rarely been reported with the VACTERL association. The second case of VACTERL association complicated with multiple rib abnormalities will be reported in this case report. A 4-year-old girl who had been diagnosed with congenital cardiovascular disease and received surgical corrections soon after birth consulted our institution by complaining congenital scoliosis. The little girl was diagnosed with VACTERL association (congenital cardiovascular disease, scoliosis with hemivertebra and butterfly vertebra, and nephrolithiasis) and congenital multiple rib abnormalities. The Cobb angle of the main curve was 29.3° before surgery, 19.9° after surgery, and 23° at last follow-up. Multiple rib abnormalities may be seen in the VACTERL association. Clinicians should have a high index of suspicion when evaluating patients with rib abnormalities associated with VACTERL. It is extremely necessary for careful physical examination and detailed auxiliary examination to each system (including echocardiography, computed tomography, magnetic resonance imaging, and so on) when diagnosing VACTERL association. Also, recognizing and understanding the congenital multiple system malformation is important, which aids in the diagnosis of disease and accordingly plan the therapeutic interventions. Early diagnosis of congenital scoliosis with appropriate surgical intervention decreases long-term morbidity.
Recently,cohort study has widely been applied to clinical tumor research of traditional Chinese medicine(TCM),which characterizes as requiring a long follow up and is easy to loss the person followed up.Follow-up study plays an important role in the prospective cohort study.The significance,contents,ways of follow up and the measures to improve the follow-up quality,method to avoid the loss of the material of follow up and records of studied outcomes are discussed in this article.
Huangqi powder injection's positive rate of skin-test was 12.3%. Qingkailing powder injection was 3.0%. Qingkailing injection was 7.6%. Shuanghuanglian injection was 6.3%. Penicillin's rate of allergic reactions was 0.7%-10%. Different form of a drug (power or injection) and different drug consistency could influence the positive rate of skin-test. We don't use drug in positive group, and we use drug in negative group. Some trial subjects still happened allergic reactions in negative group of skin-test. In negative group of skin-test, Huangqi power injection's rate of allergic reactions was 2.1%. Qingkailing injection was 0.4%. Shuanghuanglian injection was 0.9%-2.6%. Shuanghuanglian injection's rate of allergic reactions was 8.6% in all subjects (31/360 include the subjects with positive skin-test and allergic reactions). Qingkailing powder injection's rate of allergic reactions was 4.5% (6/132). Qingkailing injection' s rate of allergic reactions was 9.1% (12/132). Huangqi power injection's rate of allergic reactions was 15.4% (62/402). The total rate of allergic reactions was 10.8%. The main appearance of Penicillin's skin-test was welling under skin, and with some blush. But the main appearance of traditional Chinese medicine skin-test was blush, and with a little welling under skin. Skin-test can reduce the allergic reactions of Qingkailing powder injection, Shuanghuangiian injection, Huangqi power injection. It can be the one measure of reducing adverse reactions.
