Effect of vanillin on lipid profile in a model of hyperlipidemia, a preliminary study.

To evaluate the effect of vanillin on the lipid profile of high fat diet induced hyperlipidemia in rats, the hyperlipidemia was induced by feeding cholesterol-rich high fat diet for 45 days in wistar rats of either sex. The reduction in the triglycerides and VLDL-C was significant at 200 & 400 mg/kg dose of vanillin compared to atorvastatin group. Reduction in total cholesterol was significant at 200 and 400 mg/kg doses compared to hyperlipidemic control. The results demonstrate that vanillin at a dose of 200 and 400 mg/kg body weight lowers the serum triglyceride, VLDL-C and total cholesterol level significantly in high fat diet induced hyperlipidemic rats. However there was no significant effect on the lipid profile at 100 mg/kg dose. There were no statistically significant changes in the HDL-C and LDL-C levels at any of the given doses.

Patterns of Persistence with Antihypertensive Medications Among Newly Diagnosed Hypertensive Patients

Persistence and adherence to treatment are essential to reach the treatment goals of hypertension. This study was conducted to investigate the antihypertensive drug persistence in hypertensive patients in the routine clinical settings. Patients above 20 years of age with stage 1 hypertension, who have received the first prescription for hypertension, were included in the study. Patients were followed up for one year. During each follow-up, the parameters noted were systolic and diastolic blood pressure, changes in the prescription by the treating physician, the number of days patient missed the medication and the probable reason for missing the dose, and total doses of anti-hypertensive medications received. Based on patient behavior of persistence with first-line single treatment, they were categorized as continuers, combiners, switchers, discontinuers. A total of 77 patients were included in the study, among them 51(66.2%) were males and 26 (33.8%) were females. Among these, 67 (87.1%) showed 100% adherence to medication schedule. Around 10 (12.99%) patients missed their antihypertensive medication for more than a day (2-24 days, mean 9.89±7.24 days). Amlodipine was the most common antihypertensive used, followed by atenolol and then losartan. There were no significant differences among the types of antihypertensives used among males and females as well as among diabetics and nondiabetics. Around 14 (18.2%) patients needed an add-on antihypertensive after 4-11 months. Most commonly used add on drug was atenolol. The target blood pressure goal (<140/90mmHg) was reached in 70 (90.9%) patients at the end of 1 year.

The prevalence of chronic kidney disease (CKD) in Brunei is reported to be 12.2%. At stage 3 and later, CKD is associated with mortality rates that are 3–13 times higher than the general population, along with significant comorbidities and increases in healthcare costs. Consequently, there may be significant healthcare and economic benefits to be gained from halting or delaying CKD progression. Dapagliflozin has been shown to delay CKD progression to kidney failure and cardiovascular (CV) or renal death in the Dapagliflozin and Prevention of Adverse Outcomes in CKD (DAPA-CKD) trial.

A Critical Evaluation and Comparison of Antimicrobial Prophylaxis in Elective Surgeries Across three Hospitals

1. To evaluate and compare the pattern and rationality of prophylactic antimicrobial therapy in elective surgeries. 2. To evaluate and compare the adherence of hospitals to the standard guidelines on prophylactic antimicrobial therapy.A total of 150 patients each from a government hospital (group I), medical college teaching hospital (group II), and corporate hospital attached to the institution (group III) who had undergone elective, non-complicated surgery were included. The number & types of antimicrobials used along with duration were noted. Rationality was assessed on the basis of Kunin's criteria, ASHP guidelines & SIGN guidelines. Statistical analysis was done using appropriate tests.Cephalosporins were the most commonly used antimicrobials in all three groups (52.6%,85.7% & 84.8% respectively) followed by nitroimidazoles. Antimicrobial prophylaxis was appropriate in only 14.1%, 23.3% & 32.9% cases in the three groups respectively (p<0.01).The most common problem was prolonged duration of administration in 50.3%,58% & 45% respectively. The implementation of prophylaxis with respect to regimen & duration was erroneous in 20.1%, 12.7% & 4.7% respectively (p<0.01).The rationality & regimen of antimicrobial prophylaxis was comparatively better in the corporate hospital followed by medical college hospital and the government hospital. The present study calls for an urgent review on rational use of antimicrobials for prophylaxis in all the 3 hospitals.

A Comprehensive Overview of the Role of Azacitidine in the Management of Myelodysplastic Syndromes and Acute Myeloid Leukaemia

Myelodysplastic syndromes (MDS) are a heterogeneous group of haematological disorders marked by progressive cytopenias and risk of transformation to acute myeloid leukaemia (AML). Best supportive care (BSC) remains the mainstay of therapy. The pyrimidine analogue azacitidine was the first drug to demonstrate that a change in the natural history of MDS is possible and was the first chemotherapeutic agent approved by the US Food and Drug Administration for the treatment of MDS. Azacitidine, a hypomethylating agent, has shown to reduce proliferation and induce apoptosis of dysplastic cells. The Cancer and Leukemia Group B (CALGB) 9221 and AZA-001 trials demonstrated superiority of azacitidine over BSC and conventional care regimen, respectively. Various studies have demonstrated that azacitidine has better tolerability with comparable efficacy in terms of conventional therapy in older AML patients. Azacitidine is well tolerated even in the elderly patients. Cytopenias were the most commonly occurring haematological adverse events. Gastrointestinal adverse events (AEs) (e.g. nausea, vomiting and diarrhoea) and injection-site reactions were the most commonly occurring nonhaematological AEs. The review summarises the current role of azacitidine in the management of MDS and AML.

Evaluation of Anxiolytic Effect of Chronic Administration ofMucuna Pruriens In Wistar Albino Rats

Mucuna pruriens commonly known as cowhage plant has been claimed to possess various beneficial effects like anti-parkinsonian, anti-tumor, neuroprotective, antioxidant, anti-diabetic and antidepressant activities. Previous studies have reported that Mucuna pruriens contains LDOPA and 5-hydroxy tryptophan (5-HT) as a major constituent with higher concentration in seeds. However, literature search revealed no scientific data on its anxiolytic activity. So the present study was designed to evaluate the anxiolytic activity of Mucuna pruriens in a murine model. Wistar albino rats were divided into five groups (n=6). Mucuna pruriens administered at doses of 250,500,750mg/kg/day orally, was compared with the standard drug Diazepam (1.0mg/kg/day, oral) fed for 14 days. Three pharmacologically validated models elevated plus maze, bright and dark arena and open field test were used. The data presented was analyzed using one way ANOVA followed by Dunnett’s post hoc test. A value of p<0.05 was considered as statistically significant. Mucuna pruriensat all three doses used significantly reduced the time spent in closed arm, increased the entries into open arm in elevated plus maze (p < 0.05). In bright & dark arena model, there was an increase in number of entries, time spent in bright chamber (p < 0.05) and in open field test, the time spent and squares crossed in central chambers were increased (p < 0.05). To conclude, the present study demonstrates the anxiolytic activity of chronic administration of Mucuna pruriens in Wistar Albino rats.

Evaluation of Tribulus Terrestris in Depression Models of Albino Mice

Depression is a heterogeneous disorder that affects a person’s mood, physical health and behaviour. Despite progress in pharmacotherapy, in majority of patients depression goes undiagnosed and untreated. Ayurveda, the Indian traditional system of medicine, mentions a number of single and compound drug formulations of plant origin that are used in the treatment of psychiatric disorders. Tribulus terrestris (Gokshura) is used in Indian and Chinese system of medicine for treating various male reproductive disorders. The present study was undertaken to evaluate the antidepressant potential of acute and chronic administration of Tribulus terrestris in forced swim test (FST) and tail suspension test (TST). Inbred Swiss Albino mice weighing 20-30g were used in the study. The vehicle distilled water ( (10ml/kg, p.o), imipramine (20mg/kg, p.o) and Tribulus terrestris (100mg/kg, 200mg/kg, 400mg/kg, p.o. respectively) were administered 1hour prior to acute study. In chronic study, all drugs were given once a day for 10 days and the last dose was given 1hour before the experiment. Duration of immobility was noted in both the models. In our study, both imipramine and Tribulus terrestris significantly reduced the duration of immobility in both experimental models as compared to the animals in the control group. The antidepressant activity of Tribulus terrestris was comparable to that of standard drug imipramine. ACUTE ORAL TOXICITY OF VTH PILLS IN WISTAR RATS

Objectives: To evaluate the acute oral toxicity of VTH pills in wistar rats Methods: The study was conducted in two phases: initial and continuation phase. In the initial phase there were two groups: control and test, with ten rats (5 male and 5 female) in each group. The control group received 2% gum acacia (vehicle) 10mg/kg; the test group received 5000mg/kg of VTH pillssuspended in 2% gum acacia,orally as a single dose on day 1. All the animals were observed for clinical signs of toxicity and/or death at 1hour (±10min), 2hours (±10min), 4hours (±10min) and 6hours (±10min) on day 1 and thereafter once daily for 14 days. On day 15, the animals were subjected to necropsy. In the continuation study, 30 rats were divided into three groups receiving 2000, 1000 and 500mg/kg VTH pills respectively. Behavioural toxicity analysis was done at the end of the continuation study. Results:There was no mortality andthere wasalso no biochemical, hematological or histopathological changes at any of the doses. However, VTH pills showed significant behavioral toxicity at 5000mg/kg which was reduced at 2000mg/kg. No behavioral toxicity was noted at 500 or 1000mg/kg body weight. Conclusion: The test item, VTH pill is non-toxic upto and at 1000 mg/kg when administered by oral gavage in a single dose to wistar rats.

Therapeutic Place of Fulvestrant in the Management of Hormone-receptor Positive Breast Cancer

Fulvestrant is an oestrogen-receptor antagonist that exerts selective oestrogen receptor downregulation, antiproliferative activity and induction of apoptosis. It is indicated for the treatment of postmenopausal women with locally advanced or metastatic breast cancer for disease relapse or progression on or after adjuvant anti-oestrogen therapy. Fulvestrant was initially approved at a dose of 250 mg, however, the results of the CONFIRM trial led to approval of 500 mg dose (i.e. 500 mg on days 0, 14 and 28, then 500 mg every 28 days). Fulvestrant has also shown superiority over anastrozole as first-line therapy in the phase II trial. There are contrasting data for its efficacy when used in combination with anastrozole. It is well tolerated, with no significant difference with respect to the toxicity profile of other hormonal therapies. Treatment with fulvestrant is not associated with any clinically significant effects on sex hormone levels, bone-specific turnover markers or endometrial thickening. Fulvestrant has been recommended by the National Comprehensive Cancer Network and the European School of Oncology guidelines as a treatment option in first- and second-line management of hormone-receptor positive breast cancer.

Acute and Repeated Dose (28 Day) Oral Toxicity Study of a Polyherbal Drug (Smartlean®) In Wistar Rats.

The study was conductedto evaluate the acute and repeated dose 28-day oral toxicity study of Smartlean®. According to OECD Test No. 420 guideline the acute oral toxicity study was conducted in four groups of five animals each. Control group received 2% gum acacia 10ml/kg; second, third & fourth groups received one dose each of 54, 540 & 1080mg/kg smartlean®. The animals were observed for clinical signs of toxicity and/or mortality at 10-15min, 30-40min, 1, 2, 4 and 6hours on day one and thereafter once daily for 14 days. Four groups of animals with ten animals each were used for repeated dose 28-day oral toxicity study. The groups received 2% gum acacia 10ml/kg, 54, 540 & 1080mg/kg of smartlean®respectively,daily for 28 days. On 29th day blood was sent for biochemical, haematological investigation and histopathological examination of organs was done.None of the animals revealed any clinical signs of toxicity/mortality nor biochemical, haematopoetic or histopathological abnormalities in both acute and repeated dose 28-day oral toxicity study.The test item smartlean® is non-toxic up to and at 1080 mg/kg body weight which is 20 times higher than the recommended dose, in acute and repeated dose 28-day oral toxicity study.
