Severe Osteolysis of the Mandibular Angle and Total Condylolysis in Progressive Systemic Sclerosis

Progressive systemic sclerosis is an autoimmune disease characterized by gradual deposition of abnormal collagen which causes fibrous changes of the skin and visceral organs such as lung, heart, and kidney, which lead to serious complication. Microangiopathy and firm skin cause extrinsic pressure and obliteration of the vessels that lead to ischemia and destruction of the underlying bone. Salivary gland fibrosis causes xerostomia that increases risk of dental caries. Therefore, oral hygiene instruction (OHI) and preventive dentistry are recommended for these patients. We present a 36-year-old female that suffers from systemic sclerosis with total lysis of the mandibular condyle.

Evaluation of CBCT Accuracy in the Diagnosis of Horizontal Root Fractures in the Presence and Absence of Intracanal Metallic Post: An In Vitro Study

مقدمه: بیشتر تروماهای دندانی در ناحیه قدامی دهان و بیشتر در فک بالا اتفاق می‌افتند؛ که عوارض آن به شکل شکستگی‌های تاج و ریشه یا صدمات جابجایی می‌باشد. شکستگی ریشه بر شکستگی سمنتوم، عاج و درگیری پالپ دلالت دارد؛ که ممکن است به صورت عمودی یا افقی در عرض ریشه گسترش ‌یابد.هدف از این مطالعه بررسی دقت دو سایز وکسل (voxel) مختلف در CBCT و نیز تأثیر سیستم‌های فیلتراسیون و حذف آرتیفکت جهت تشخیص شکستگی افقی ریشه در حضور و یا فقدان پست فلزی بود. مواد و روش‌ها: شکستگی افقی به شکل مصنوعی در 15دندان درمان ریشه شده به صورت تصادفی در نواحی میانی و کرونال ریشه ایجاد شد؛ و 15 دندان نیز به عنوان گروه کنترل انتخاب شدند. برای هر 30 دندان قبل از ایجاد شکستگی پست ریختگی ساخته شد. دندان‌ها جهت تصویربرداری در کست ساختگی قرار داده شدند و تصاویر توسط سیستم CBCT در دو سایز وکسل 075/0 میلی‌متر و 15/0 میلی‌متر تهیه شد و تحت سیستم‌های فیلتراسیون و حذف آرتیفکت قرار گرفتند. تصاویر توسط یک رادیولوژیست با تجربه مورد ارزیابی قرار گرفت. حساسیت، ویژگی و دقت شرایط مختلف اعمال شده در تشخیص شکستگی افقی ریشه محاسبه شد. یافته‌ها: میزان حساسیت از 7/46 درصد، که مربوط به سایز وکسل 15/0 میلی‌متر باحضور پست فلزی داخل کانال بود، تا 100 درصد در سایز وکسل 075/0 میلی‌متر بدون حضور پست، بدست آمد. میزان ویژگی برای همه روش‌های مورد مطالعه برابر 100 درصد بوده است. بیشترین تاثیر منفی پست در تشخیص شکستگی‌های یک سوم کرونال ریشه و ناحیه سرویکال بود. نتیجه گیری: بیشترین میزان حساسیت و دقت در سیستم 3Dmax Promax مربوط به سایز وکسل 075/0 میلی‌متر، بدون حضور پست، بوده است که با نتایج واقعی نمونه‌ها یکسان بود (%100). سیستم‌های حذف آرتیفکت و فیلتراسیون در تشخیص شکستگی ریشه بی‌تأثیر بودند.

Root Length and Anatomy of Impacted Maxillary Canines in Patients with Unilateral Maxillary Canine Impaction

Introduction: Canine impaction is a common occurrence. In this study, we sought to investigate the root anatomy and length of impacted canines and lateral incisor adjacent to impacted maxillary canine. Materials and Methods: In this retrospective study, three-dimensional tomographic imaging was performed on 26 patients with unilateral maxillary canine impaction. In this study, we evaluated root length and anatomy of impacted canines, in terms of resorption intensity and curvature, with Planmeca Romexis Viewer 4.0. Furthermore, crown shape as well as root length and anatomy of the lateral incisors adjacent to impacted canines were investigated and compared with the other side on the dental arch, where canine eruption was normal. Results: Root length of impacted canines was significantly lower than that of normal canines (P=0.011). There were no significant differences between root length of lateral incisors adjacent to impacted canines and root length of lateral incisors adjacent to normal canines (P=0.221). Moreover, the resorption intensity of the adjacent lateral incisors was higher than that of the impacted canines. No significant differences were noted in root resorption intensity between the lateral incisors adjacent to the imacted canines and the lateral incisors adjacent to normal canines (P=0.36). In addition, resorption intensity was significantly higher in impacted canines than in normal canines (P=0.024). Root anatomy of impacted canines was not significantly different from that of normal canines (P=0.055). The crown shape of the lateral incisors adjacent to impacted canines was not significantly different from that of the lateral incisors adjacent to normal canines (P=0.052). Conclusion: Impaction can probably affect root length and canine resorption severity. However, root and crown shape of lateral incisors cannot always be associated with canine impaction.

Diagnostic Efficacy of Digital Waters' and Caldwell's Radiographic Views for Evaluation of Sinonasal Area.

New digital two-dimensional imaging systems are considered as an easily accessible modality for sinonasal evaluation. The aim of this study was to assess the diagnostic efficacy of digital Waters' and Caldwell's radiographic views for evaluation of sinonasal area.A total of 273 patients fulfilling the inclusion criteria were enrolled in this descriptive-analytical study. Complete opacification or mucosal thickening of frontal, ethmoidal and maxillary sinuses and nasal septum deviation were assessed on digital Waters and Caldwell images. Considering cone beam computed tomography (CBCT) as the gold standard, accuracy, sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) were obtained for the plain films.The sensitivity and specificity for the diagnosis of complete opacification were 89.47% and 95.66% (for frontal sinus), 100% and 97.59% (for ethmoidal air cells), and 100% and 96.42% (for maxillary sinuses), respectively. Regarding mucosal thickening, the sensitivity and specificity were 51.43% and 90.70% (for frontal sinus), 60.84% and 84.80% (for ethmoidal air cells) and 81% and 74.74% (for maxillary sinuses), respectively. The sensitivity of Waters and Caldwell images for detecting nasal septum deviation was 84.31% and 87.25%, respectively; whereas, their specificity was 100%.Digital Caldwell image performs well in detecting frontal and ethmoidal opacification and nasal septum deviation. It has moderate efficacy in assessing frontal and ethmoidal mucosal thickening while it has superior performance for detection of healthy cases. Digital Waters' view is a reliable modality for maxillary sinus evaluation.

Seckel syndrome associated dental anomaly: case report

espanolEl sindrome de Seckel (SS) es una rara enfermedad autosomica recesiva que cursa con un grave retraso del crecimiento, un perfil de cabeza de pajaro y microcefalia. Se caracteriza por baja estatura, defectos esqueleticos, retraso mental y rasgos faciales caracteristicos como microcefalia, micrognatia y apariencia de cabeza de pajaro. Los hallazgos dentales incluyen hipodoncia, hipoplasia del esmalte, apinamiento y maloclusion de clase II. Caracteristicas craneofaciales pueden incluir asimetria facial, fisuras palpebrales inclinadas hacia abajo, orejas sin lobulos y anormalidades dentales, incluyendo hipoplasia del esmalte, la hipodoncia y la microdoncia. Los pacientes con SS pueden tener un alto riesgo de desarrollar leucemia mieloide aguda. El proposito de este trabajo es informar de un caso de sindrome de Seckel con una anomalia dental. EnglishSeckel syndrome is a rare autosomal recessive with severe growth retardation, bird-headed profile, and microcephaly. It is characterized by short stature, skeletal defects, mental retardation, and characteristic facial features such as microcephaly, micrognathia, and a bird-head appearance. Dental findings include hypodontia, enamel hypoplasia, crowding, and Class II malocclusion. Craniofacial features may include facial asymmetry, down-slanting palpebral fissures, lobeless ears, and dental abnormalities, including enamel hypoplasia, hypodontia, and microdontia in the patients with SS may be at high risk of developing acute myeloid leukemia. The purpose of the paper is to report a case of the seckel syndrome with a dental anomaly.

Prevalence of accessory mental foramen and lateral lingual foramen using cone beam computed tomography: A single-center cross-sectional study.

Abstract Purpose This study aimed to assess the prevalence, dimensions, and positions of AMF and LLF in an Iranian population using CBCT imaging. Methods and materials: In this cross-sectional study, CBCT images of 2082 patients were retrospectively reviewed based on inclusion criteria, including images from the mandibular region taken in the past five years with accurate demographic data. Exclusion criteria included images with significant pathologies, fractures, or poor quality. CBCT images were captured, and AMF and LLF presence, dimensions, and positions were recorded relative to the mental foramen and adjacent teeth. Data analysis was performed using the Chi-square test with a 5% significance level. Results A total of 2082 CBCT samples (46.54% females and 53.46% males) with a mean age of 48.30 ± 12.7 years were examined. Among these, 246 AMFs were observed in 222 individuals, with 24 having two AMFs. AMF was mostly observed unilaterally, with bilateral occurrence noted in 10.81% of patients with AMF. There were no significant differences between genders in AMF and LLF prevalence. Additionally, 68 patients were found to have lateral lingual foramina (LLFs). Conclusion AMF and LLF were detectable using CBCT, with a prevalence of 11.81% for AMF and 5.91% for LLF in the selected Iranian population. AMF was commonly situated posterior and superior to the mental foramen, while LLF was often found near the first premolar. This knowledge is crucial for preventing nerve injury during surgery and ensuring effective nerve block procedures.

Assessment of the lateral wall thickness of the maxillary sinus is very important in decision making for many surgical interventions. The association between the thickness of the lateral wall of the maxillary sinus and the dental status is not well identified.To compare the thickness of the lateral wall of the maxillary sinus in individuals with and without teeth to determine if extraction of the teeth can lead to a significant reduction in the thickness of the maxillary sinus lateral wall or not.In a retrospective study on fifty patients with an edentulous space, the thickness of the lateral wall of the maxillary sinus,one centimeter above the sinus floor in the second premolar (P2), first molar (M1) and second molar (M2) areas was determined by cone beam computed tomography scans(CBCTs) and a digital ruler in Romexis F software (Planmeca Romexis 2.4.2.R) and it was compared with values measured in fifty dentated individuals. Three way analysis of variance was applied for comparison after confirmation of the normal distribution of data.The mean of the wall thickness in each of these points was lower in patients with edentulous spaces; however it was not significant. There was no association between gender and the thickness of the lateral wall of the maxillary sinus, but location was associated with different thicknesses.The differences in the thickness based on the location and dental status necessitates assessment of the wall thickness of the maxillary sinus in addition to the current evaluation of bone thickness between the sinus floor and the edentulous crest before maxillary sinus surgery.

Radiographic evaluation of Stafne Bone Cyst in patients referred to a radiology center in Mashhad, Iran: A 3-year survey

Introduction : The aim of this study was to evaluate the radiographic features of stafne bone defects in patients referred to a private radiology center from March 2013 to 2016 in Mashhad, Iran. Materials and Method: In this retrospective cross-sectional study 5000 CBCTs were evaluated with the definitive diagnosis of SBD. Radiolucencies between the molar region and angle of mandible were considered as SBD and if no increase in the size of the depressions was observed in the second panoramic view, the diagnosis of SBD was made. After selecting the radiographs with SBD, all their radiographic characteristics were recorded. Results : Among the 5000 CBCT radiographs, 10 (0.2%) SBD cases were diagnosed, 9 in males and only 1 case in a female. All SBDs were between the molar regions and the angle of mandible and were categorized as the posterior variant of SBD. In our studied cases SBD was near the inferior border of mandible and in 3 cases the depressions disrupted the continuity of the inferior mandibular border. The lesion’s shape was oval in 7 cases and round in the others. In total 5 cases were categorized as type I, 4 as type II and one case as type III of SBD. Conclusion : According to our findings, SBD is an uncommon lesion in the mandible and CBCT can be used as a non-invasive method to detect this defect.. Evaluation of the interpretation of bitewing radiographs in treating interproximal caries

Abstract Objective: Misinterpretation of bitewing radiographs may lead to the selection of surgical approach rather than medical approach in treating proximal caries lesions. We aimed to determine the sensitivity, specificity, and diagnostic accuracy of bitewing radiography interpretation by various groups of dentists for the detection of proximal caries and subsequent treatment decision-making. Materials and Methods: This in vitro study was performed using 60 extracted molar and premolar teeth. The target proximal carious surfaces were categorized and coded according to the International Caries Detection and Assessment System (ICDAS) as category 1: ICDAS = 0, category 2: ICDAS = 1 or 2, and category 3: ICDAS = 3, 4, or 5. All the teeth were randomly divided and mounted onto 15 quadrants made of two premolars and two molars, and a digital bitewing image was taken from each quadrant. A checklist was given to four groups of participants (dentistry students, dentists with a DDS degree, restorative dentistry specialists, and oral radiology specialists) to indicate for which lesion depth they would intervene restoratively. The data acquired through the checklists were compared with direct visual examination of target surfaces before mounting. Results: Sensitivity and accuracy of bitewing radiography showed no significant difference among the groups. However, specificity was significantly higher in Group D. Conclusions: According to our results, interpretation of bitewing radiographs was different among the groups. Although not significant, the radiologists had the highest diagnostic accuracy than the other groups of participants, and the students showed the weakest performance in the diagnosis of restorative treatment needed. Furthermore, the highest percentage of decision error occurred when lesions had ICDAS 1 or 2, followed by ICDAS 3, 4, or 5, and finally 0 in all the four groups.

Complaints against Dentists Working in Mashhad, Iran; 2012-2017

Background: The analysis of complaints against health professionals assists in improving health care services. The objective of this study was to evaluate complaints against dentists working in Mashhad, Iran during 2012-2017. Methods: In this study, all the available completed dental complaint cases from the archive of the Mashhad Medical Council addressed within a six-year time period (from 2012 through 2017) were reviewed. The required information was extracted based on a checklist including variables such as the gender of dentists and complainants, the age of dentists and complainants, dentists’ degree, the place of service provision, the type of services provided, the causes of complaints, and the sentences issued to dentists (conviction or non-conviction). Results: A total of 208 cases were evaluated, and in 67 (32.2%) cases, dentists had been found guilty. Most cases involving convicted dentists were associated with male dentists, female complainants, general dentists, and in-office services. In the cases of convicted dentists, the most common service provided was endodontic treatment and the most common cause of complaint was failed or incomplete treatment. Conclusion: To reduce the growing number of complaints against dentists, their skills should be improved, especially in endodontic treatment services. It is advisable for general dentists to consider referring patients to specialists either for more skilled treatments or to allow their patients’ expectations to be better met.
