Recurrent Intracranial Bleed in 3 Siblings: Short of a Shot of Vitamin K!

We present a family who suffered recurrent sibling losses due to vitamin K deficiency bleed. The index child was asymptomatic at presentation, had normal clinical examination, and was investigated for coagulation disorders in view of previous 3 sibling losses as a result of intracranial hemorrhage. His investigations showed deranged coagulogram and clotting factors’ assay. The baby was given vitamin K 1 1 mg intramuscularly following which his coagulogram and clotting factors’ assay returned to normal. The genetic analysis did not identify any inherited cause of bleeding tendency. The significant family history, exclusive breastfeeding, no diarrhea, failure to thrive or drug use, no prophylaxis with vitamin K at birth, recovery of clotting factors on vitamin K administration, and a corroborative molecular analysis confirmed diagnosis of vitamin K deficiency in the index child. This case gives a strong reminder not to miss birth dose of vitamin K in any neonate.

Peripheral nerve sheath tumours--a short series with some uncommon variants.

Peripheral nerve sheath tumours are rarely malignant (0.001%), such malignant peripheral nerve sheath tumours (MPNST) are more common in upper extremities than in head and neck. Chondroid differentiation in benign peripheral nerve sheath tumours and melanotic schwannoma are very uncommon. In a retrospective analysis of 25 peripheral nerve sheath tumours over a period of two years, we reported two MPNST one of which was in a parapharyngeal location while the other MPNST showed melanotic differentiation. Similar melanotic differentiation was also seen in another benign melanotic schwannoma. Chondroid differentiation in a schwannoma was also observed which is usually documented in MPNST.

Haematological profile of dengue fever

Background: Dengue is a viral illness that is increasingly becoming endemic in India. This study aimed to study the haematological profile of patients diagnosed with dengue infection in a tertiary care hospital.Methods: 89 patients suspected of having dengue illness were followed. Out of which those confirmed by positive serology were followed and studied in detail (n=46).Results: Common clinical symptoms were fever, vomiting, and abdominal pain. Common haematological abnormalities were thrombocytopenia and leucopoenia. All patients improved clinically with improvement of biochemical and hematological parameters. None of the patients died in this series.Conclusions: Dengue Fever continues to be a significant health problem especially in Northern region of India. A sharp vigilance is required by concerned authorities to prevent and minimize any future outbreak. It is extremely important to implement and maintain an effective, sustainable and community based disease prevention program.

Pulmonary Alveolar Microlithiasis: A Case Report

Pulmonary alveolar microlithiasis is a rare disease of unknown etiology, characterized by intra-alveolar deposition of calcific concretions. Chest radiographic findings are characteristic, showing multiple nodules less than 1 mm diameter scattered diffusely throughout the lung parenchyma. Pulmonary alveolar microlithiasis is a slowly progressive disease leading to respiratory insufficiency associated with cyanosis, clubbing, and pulmonary hypertension. There is no satisfactory therapy for this condition.

Prevalence of vascular thrombosis in patients with liver abscess

Thrombotic complications in liver abscess are less commonly identified in the literature. In this prospective observational study, vascular thrombosis was detected in 21.9% cases. However, there was no significant difference in outcome with or without vascular thrombosis, properly treated.
