Diabetic retinopathy in patients with newly diagnosed type 2 diabetes mellitus

Introduction: Diabetic retinopathy (DR) is the leading cause of visual impairment worldwide including Nepal and studies from populations have shown that up to 10% of diabetic patients have vision-threatening DR, with up to 5% requiring treatment at this initial visit. Within this context, this study aims to evaluate the presence of DR in newly diagnosed diabetic patients, presenting in the outpatient department of ophthalmology, at Patan Academy of Health Sciences. Method: It is a retrospective cross-sectional study carried out in referred cases of newly diagnosed type 2 Diabetes Mellitus (DM) from January 2021 to December 2022. Information regarding age, gender, Body Mass Index (BMI), blood pressure (BP), Triglyceride (TG), proteinuria, and findings of fundus evaluation were extracted from the record maintained in the Department of Ophthalmology. Result: Out of 102 patients with mean age of 49.1±13.6 y, 14(13.73%) had DR, 11(10.8%) had mild Non-proliferative Diabetic Retinopathy (NPDR) and three (2.9%) had moderate NPDR. There was female predominance 56(54.9%), 50(49%) patients were overweight or obese (BMI≥25 kg/m2), 15(14.7%) patients had proteinuria and 28(27.5%) had fasting hypertriglyceridemia and additional ocular anomaly was identified in 33(32.4%) patients. Conclusion: Diabetic retinopathy is already present in significant proportion of newly diagnosed type 2 DM patients. Routine screening for DR at the time of diagnosis can improve ocular outcomes in newly diagnosed diabetics with targeted early treatment. Future research should evaluate this in wider population to generate guidelines for need of mandatory eye evaluation at diagnosis.

To Assess the Oral Health Status among Adolescent Ghutkha Chewers of Selected Schools

Background: Oral health is a critical part of general health and every individual's valuable asset.Oral diseases are among the most common non communicable diseases that affect a number of populations.Ghutkha behaviors that coexist in a large proportion of male adolescents will have a major impact on oral diseases.Objectives: 1) To assess the oral health status among adolescent ghutkha chewers of selected schools. 2) To associate oral health status among adolescent ghutkha chewers of selected schools with selected demographic variable. Method -Research approach: Exploratory approach. Setting of the study: Study conducted in primary school Wardha Sample: In this study the sample adolescent Sampling technique: -by using non probability purposive sampling technique, Sample size: 150 Results: In this study purposive sample of 150 adolescents according to the level oral health status among adolescent ghutkha chewers of were seen into 5 categories, poor, average, good, VeryGood and excellent.Majority of ghutkha chewer adolescents 15.33% (23) had average, 63.33% (95) had good, 21.33% (32) had very good level of health status score.Mean score was health status 50.37± 9.45.Conclusion: In this study the level of oral health adolescent ghutkha chewers status, association of oral health status levels with adolescent ghutkha chewers' demographic characteristics.The table values are significantly higher than the measured values at a significance rate of 5%.Also the p values calculated that were much higher than the acceptable meaning level, i. e. ' p'=0.05.

A Review on Preventing Tragedy: Strategies to Combat the Devastating Effects of Adolescent Drug Overdoses

Adolescent drug overdose deaths are a growing public health crisis, with significant consequences for individuals, families, and communities. This review article provides a comprehensive overview of prevention strategies to combat the devastating effects of adolescent drug overdose. Drawing on a comprehensive literature search of electronic databases, the article evaluates the effectiveness of prevention strategies and identifies risk factors associated with overdose deaths. The review outlines three key prevention strategies, including education and awareness programs, access to treatment and support services, and policy changes and regulations. The article also discusses the limitations and challenges of prevention efforts, including limited access to treatment and support services, the need for more research on effective prevention strategies, and the ongoing challenges posed by the opioid epidemic and the emergence of new synthetic drugs. Overall, this review highlights the urgent need for continued research, innovative prevention strategies, and effective policies to prevent adolescent drug use and overdose deaths and promote healthier communities for all.

Association of dry eye syndrome with oral Isotretinoin therapy for severe nodulocystic acne and recalcitrant acne vulgaris

Introduction: Oral isotretinoin was registered in Nepal almost three decades later its first introduction in USA. It remains the most clinically effective medication for acne. Dry eye syndrome is a ‘certain’ adverse effect of oral isotretinoin therapy which can be validated by tear film break up time and Schirmer I tests.Methods: Fifty patients aged above 12 years presenting to dermatology outpatient department of Dhulikhel Hospital with severe nodulocystic acne or recalcitrant acne vulgaris not responding to three months of systemic antibiotics along with topical agents were included. Tear film break up time and Schirmer I tests before, at 45 and 90 days, and one month after taking oral isotretinoin were performed to study association of dry eye syndrome due to oral isotretinoin.Results: Both eyes of fifty selected patients were sampled. Baseline mean tear film break up time of 12.37±4.17 sec decreased to 9.69±3.70 sec, 9.09±3.77 sec and 10.67±3.50 sec at 45 and 90 days and on follow up, respectively. Likewise, baseline mean Schirmer I value of 16.68±8.73 mm decreased to 12.26±7.64 mm, 11.49±8.07 mm and 11.76±8.11 mm respectively at 45 and 90 days and on follow up. The differences between the mean values were analyzed using paired samples T test and all were statistically significant except between the mean Schirmer values at 90 days and on follow up.Conclusions: Abnormal mean tear break up time at 45 and 90 days in this study suggests that dry eye syndrome was caused by oral isotretinoin during acne therapy but tends to revert to normal on cessation.Journal of Patan Academy of Health Sciences, Vol. 2, No. 1, 2015. page:12-16

To Assess the Parents Reported Causes of Physically Abuse Children the Age Group of 6 to 12 Years

Introduction: Child maltreatment and neglect are considered important issues in the US. However, parental-child violence is not studied enough to provide a comprehensive understanding of the underlying causes. Aim: To determine the parent's reported causes of actual children who are abusing the age bracket of 6 to 12 years. Methods and Materials: The descriptive survey study design was used with a quantitative research approach conducted in a selected community area of Wardha town. The sample size was 200 parents with children aged 6 to 12. Pre-tested and validated structured questionnaires were used. The data gathered was registered into the Microsoft Excel sheet. The statistical analysis ended up being done SPSS. The frequencies and percentages of categorical variables had been provided. Results: The parents reported causes for physical abuse among children of 6 to 12 years were seen into 4 categories, once in a week, sometimes, twice in a week, and never respectively. 37.5% of the parents reported twice in a week and 62.5% of them never reported causes. The mean score of parents who reported causes of physically abuse children in the age group of 6 to 12 years was 44.58 ± 4.66 and the range of the score was 32 to 56. Demographic variables such as age in years, gender, monthly income, education, area of residence, and relationship with parents were statistically associated with their reported causes. Conclusion: Community-based methods, such as house visits, are incredibly efficient in changing the behavior of parents at risk of misuse. Targeted programs for moms with mental disorders and substance abuse are also shown to be effective in stopping emotional abuse [1].

Effectiveness of Mechanical Hydrotherapy on Pain Management among Patients with Arthritis Pain

Introduction: Hydrotherapy is a warm water therapy used in relieving pain such as muscle pain, back pain and inflammation associated with the arthritis pain. In arthritic condition hydrotherapy helps to improving blood circulation. It is commonly used for treating muscle injuries and stroke and brain injuries. Objectives: To determine the effectiveness of mechanical hydrotherapy on pain management among patients with arthritis pain. Materials and Methods: In this study a quantitative research approach with an experimental pre-test post-test research design was used for the present study. Purposive sampling was the sampling method used to collect data from arthritis patients on the basis of standardized numerical pain rating scale (NPRS). The sample size was 70, for experimental group 35 samples and control group was 35 samples. For experimental group the mechanical hydrotherapy was given along with prescribed medicines and for control group only prescribed medicine was monitored. Results: The majority of participants were male 41 (58.57%) and female 29 (41.43%) as compared to male. In experimental group male were 22 (62.85%) and female were 13 (37.15%) and in control group male were19 (54.28%) and female were 16 (45.72%). Before intervention on day-1 pre assessment pain mean value was 7.51 and standard deviation was 1.46 in experimental group and In control group mean value was 7.91 and standard deviation was 1.12. On day-7 post assessment pain the outcome of an experimental group after mechanical hydrotherapy with reducing pain, mean value was1.80 and standard deviation was 1.07 and in control group mean value was 7.97 and 1.27. Statistically significant improvement seen in the level of pain regarding effectiveness of mechanical hydrotherapy on pain management among patients with arthritis pain. Conclusion: Hence it is statistically clear that, the mechanical hydrotherapy to arthritis patients is helpful in managing the pain effectively.

This comprehensive review explored the evolving landscape of biomarkers of acute kidney injury (AKI), emphasizing their critical role in early detection, monitoring, and prognostication. The examination included functional biomarkers (serum cystatin C, NGAL, KIM-1), tubular injury biomarkers (NAG, L-FABP, and IL-18), inflammatory biomarkers (CRP, TNF-α, and IL-6), and oxidative stress biomarkers (MDA, SOD, and catalase). These biomarkers offer nuanced insights into specific facets of renal injury, aiding in differentiating prerenal and intrinsic renal injury and providing a more holistic understanding of AKI pathophysiology. The implications for clinical practice are profound, with biomarkers presenting valuable tools for personalized patient management, early intervention, and improved outcomes. Future research directions should focus on identifying emerging biomarkers, technological advancements, refining integration strategies, and addressing standardization and cost-effectiveness for seamless integration into routine clinical practice. Carcinoma of Tail of Pancreas: A Case Report

Background: A typical manifestation of pancreatic tail cancer is large intestinal obstruction with perforation. Clinically the cancer of pancreas is usually complicated to diagnose. Most cancer patients are not having symptoms throughout during the initial stages of the cancer, which often leads to a delay in diagnosis. Treatment choices include surgery, chemotherapy, and palliative care. It is more common in African-Americans, slightly more common in men. Case Presentation: A female patient of 40 years from Wardha was admitted to Female surgery Ward, Unit-3, AVBRH on 18th December with a chief complaint of pain in epigastric region since 2weeks. Patient was apparently all right 2 months back then she was complaining of pain in the epigastric region which was insidious in onset, gradually progressive in nature, burning type of pain with radiating to left upper back. No history of fever, nausea vomiting, clay-coloured stools. After that patient was undergone on routine investigation in that total WBC count was increased i.e., 13000/cu mm and haemoglobin level were decreased i.e., 9.7gm%, liver biopsy revealed that metastasis of Adenocarcinoma probably of pancreatic origin, Computed tomography and ultrasound and it revealed that heterogenous iso-echoic mass in the tail of pancreas based on investigation she was diagnosed as a case of Carcinoma tail of pancreas and she was undergone on treatment of antibiotic before chemotherapy .after that chemotherapy treatment was done for management of pain. Conclusion: Pancreatic adenocarcinoma presents differently from large intestinal cancer and should be explored in the differential diagnosis of large intestinal obstruction.

Carcinoma of Breast: A Case Report

Background: In Indian women, breast cancer is the most frequent type of cancer. Breast cancer affects 30 out of every 100,000 women in India. Case Report: A female patient of 70 years old was admitted to Female surgery Ward-33, Unit-3, AVBRH on 19th January with a chief complaint of lump in upper outer quadrant of the right breast, hardness and non-tenderness of right breast since 2 months. The patient was ok. She had complained of a lump in the upper outer quadrant of her breast for 2 months, and it had appeared gradually. During a physical examination, some clinical findings were obtained i.e., lump in a breast was gradually progressive, and non-tenderness and a nonmobile lump in upper outer quadrant of right breast, no history of fever, nausea vomiting, bloody discharge from a nipple. After that patient was undergone on the routine investigation in that Haemoglobin- 8.4gm% was decreased, Total Red Blood Cell count 2.88 cumm decreased, Total Leucocyte Count -17800 cumm increased. Fine needle aspiration cytology show metastasis of breast and ultrasonography shows an irregular strangulated hypoechoic mass of approximately 3.5*1.9 cm at the outer upper quadrant of the right breast. After histopathology report revealed that patient having metastasis of right breast and she was undergone on treatment of chemotherapy. Conclusion: This case report was of great relevance for academics, its provided to improve knowledge about breast cancer, to encourage the practice of BSE making women aware of the importance of this exam.

Initially it is in small quantities, causing human harm to a level until it falls into the category of contamination, but as soon as this limit is violated, it becomes pollution rather than contamination.In the Hindu scripture "Varaha Purana" it is said that the five Mahayagas are the benefactors of trees. They treat planets by giving them shade and rest to fuel pilgrims, birds to nests and medicine to all living beings with leaves, roots and ulcers. विश्व प्रकृति निधि भारत का हमेशा ही यह उद्देश्य रहा है कि हम प्रकृति और पर्यावरण संरक्षण के साथ दीर्घकालिक तथा न्याय संगत विकास करने में सहभागी बनें।1जब तक हमारे पर्यावरण में बाह्य पदार्थ आकर मिलते हैं संदृषण होता है। प्रारम्भ में यह अल्प मात्रा में होता है, जिससे एक स्तर तक मनुष्य को कोई हानि नहीं पहुँचती तब तक यह संदूषण की श्रेणी में, लेकिन जैसे ही इस सीमा का उल्लंघन होता है तो यह दूषण संदूषण न रहकर प्रदूषण बन जाता है। हिन्दू धर्म ग्रन्थ ‘‘वाराह पुराण’’ में लिखा है कि वृक्षों के उपकार पाँच महायज्ञ हैं। वे ग्रहस्थों को ईंधन पथिकों को छाया तथा विश्राम, पक्षियों को घोंसले तथा पत्ते, जड़ एवं छालों से सारे जीवों को औषधि देकर उनका उपचार करते हैं।
