Musculoskeletal complaints may be the initial manifestation of childhood leukaemias. When these symptoms predominate at the onset, a diagnosis of one of several rheumatic diseases may be entertained. Where blood tests are normal, no bone marrow examination would normally be indicated. The use of immune-suppressing medication, such as steroids, may lead to diagnostic delay or misdiagnosis.

KNOWLEDGE AND AWARENESS OF INFORMED CONSENT AMONG DENTAL PATIENTS

Obtaining informed consent from the patient is fundamental. The consent document should be in local language and should be simple and easy to understand. The awareness and knowledge on informed consent among both patients and the physician is essential in order to practice an efficient process as it is a continuous, two- way communication where transparency and partnership plays an important role.This study was undertaken to examine patients knowledge and awareness about informed consent process. 100 patients attending the Out Patient department were interviewed through questionnaire to assess the awareness and knowledge of the informed consent process. The present study revealed the percentage of patients who were aware of the informed consent process and the information provided in it.

How well are we prescribing medications to our children?

Introduction: Prescription errors are common and every step in the process of writing a prescription has the potential for error. These errors can be broadly attributed to the lack of knowledge or lack of skill. Poor legibility, inaccuracy in writing, incomplete prescriptions, omissions, and use of abbreviations are the most common type of errors. Objectives: The primary objective of our study was to assess the completeness and legibility of the prescriptions. This is to estimate the rates of skill-based errors. Secondary objective of this study was to assess the appropriateness of the prescriptions using the pediatrics: Omission of Prescriptions and Inappropriate prescriptions (POPI) tool. Methods: This was a cross-sectional study, conducted in the pediatric outpatient department of a tertiary teaching hospital in New Delhi, India. Prescriptions received by outpatients attending both the general and special pediatric clinics were collected and evaluated using the World Health Organization Guide to Good Prescribing and the Medical Council of India guidelines. Results: A total of 343 prescriptions were assessed, in which 753 drugs were prescribed. A total of 1709 errors were uncovered pertaining to the completeness of the prescription. A majority of the prescriptions were written by postgraduates trainees (206), followed by senior residents (61), consultants (42), and junior residents (20). Abbreviations were used while mentioning the drug names in 53.64% prescriptions. The mean legibility score of all prescriptions was 3.35. Forty (11.66%) prescriptions were found inappropriate as per the International POPI tool. Conclusion: The legibility and completeness are largely overlooked yet crucial to the “Art of Prescription Writing.” They deserve as much awareness and education as the appropriateness of the drugs prescribed. The international POPI tool is a promising tool to analyze the appropriateness of pediatric prescriptions.

Cerebral Vasculitis in a Case of Henoch–Schönlein Purpura

Background: Henoch–Schönlein purpura (HSP) is the most common vasculitis seen in children that rarely involve central nervous system (CNS) (0.65%–8%). Patients with CNS vasculitis may present with symptoms such as headache, seizures, focal neurological deficit, or behavior abnormalities. We describe a case of HSP where the patient presented with developmental delay along with skin rash and renal involvement. Clinical Description: A 15-month-old boy presented with a history of developmental delay and skin rashes for the past 2 months. There was no significant antenatal or birth history. At the age of 15 months, the child could not sit without support, could not transfer objects from one hand to other, could only speak bisyllables, and could not wave “bye-bye.” On examination, the patient had normal vitals with equal volume peripheral pulses and normal blood pressure in all four limbs. Urine examination showed microscopic hematuria, and kidney functions were mildly deranged. Skin biopsy showed immunoglobulin A vasculitis. Magnetic resonance imaging brain was done which revealed multifocal areas of altered signal intensity in periventricular deep white matter in bilateral frontal and parieto-occipital lobes, appearing hyperintense on T2-weighted and fluid-attenuated inversion recovery sequences, showing evidence of diffusion restriction, suggestive of areas of acute ischemia. The findings were consistent with manifestations of CNS vasculitis. Management: The patient was managed conservatively and advised physiotherapy and follow-up for developmental delay. Conclusion: HSP can rarely involve CNS, however, developmental delay in HSP has never been reported before.

Tuberculosis Presenting as Thrombotic Thrombocytopenic Purpura

Background: Thrombotic thrombocytopenic purpura (TTP) is a rare disorder characterized by microangiopathic hemolytic anemia, thrombocytopenia, acute renal failure, and central nervous system impairment. Clinical Description: Although most cases of TTP are idiopathic, there are some case reports describing its association with certain infections. We report a rare case of TTP associated with tuberculosis which was confirmed by ADAMTS13 levels. Management: This child was managed with high-dose plasma infusion. Conclusion: To the best of our knowledge, this is the first case in a pediatric patient with this association and treated successfully by high-dose plasma infusion.

Clinical Profile and Outcome of Burn Injuries amongst Children in a Tertiary Care Center of North India

Objectives: The study aims to describe the clinical characteristics and outcome of pediatric burn patients admitted to a tertiary care hospital. Patients and methods: This was a cross-sectional, retrospective study that reviewed the records of all burn victims aged 0 to 12 who were admitted in a tertiary care hospital in North India over one year. Collected data included patient demographics such as age, gender, burn causal agents, discharge time, Total body surface area (TBSA) burnt by using Lund and Brauder’s Rule, the duration of stay, the season and month of admission that burn occurred, and burn characteristics, such as region of body involved, percentage of area burnt, complications and outcome. Results: 129 paediatric burn patients under 12 were included with median age of 4 years. Scald caused the majority of burns (81.4%), followed by dry burns (14.7%) and electric injuries (3.9%). The Median burn percentage was 20 and most burn patients were encountered during the winter season. The face and neck (56.6%) were involved in majority of cases, with a 4.6% overall mortality rate. Conclusion: This retrospective study provides valuable insights into the clinical profile of burn patients admitted to a tertiary care center in North India. The study's findings can help guide the development of targeted interventions and resource allocation to improve burn care in this region, ultimately reducing morbidity and mortality among burn patients.

This article aims to describe some of the potential risks of orthodontic treatment which when neglected can lead to a malpractice lawsuit.In orthodontic practice,categories vary from informed consent or negligent non-disclosure,failure to diagnose,to gross negligence.This study focuses on creating awareness about the importance of informed consent before orthodontic procedure to avoid legal issues.Orthodontic failures do occur sometimes but it should be identified earlier and rectified.Consent should be seen as a process and any discussions should be fully and legibly recorded in the patient records. Key words: Malpractice, informed consent, hazards of orthodontic treatment, Negligence.
