A qualified decision-support mechanism—even when integrated into the ordering workflow of an electronic health record (EHR)—may be constructed so that the path of least resistance for the ordering provider encourages entry of information that does not optimally reflect the patient's condition or, worse, contradicts the patient's true presentation.
