A novel economical approach to gait analysis using locally available digital tools in a tertiary care hospital setting

BACKGROUND: Analysis of gait parameters is an essential component of diagnosing musculoskeletal/neuromuscular conditions. A standard approach to gait analysis involves sophisticated commercially available digital technologies, which are expensive and not affordable by under-resourced tertiary care hospitals. We aimed here to test the validity of a low-cost digital tool for gait analysis in a tertiary care hospital setting. MATERIALS AND METHODS: This study used a smartphone with a 12-megapixel camera mounted on a tripod with Kinovea software (version 0.9.5, Free Software Foundation, Inc., Boston, MA, USA, distributed under a GPL v2 license) for gait analysis in our tertiary care hospital. This novel indigenous tool was validated in 502 healthy individuals by comparing standard gait parameters evaluated between younger and healthy cohorts. RESULTS: Overall, the recording and analysis process was simple, quick, and easy to use. The validity of our novel indigenous gait analysis tool was established by observation of reduced stride length, foot clearance, and knee goniometry among elderly subjects compared to the younger cohorts. Our observation of the elderly subjects showing altered gait parameters compared to the younger age group was consistent with previous studies measuring the same parameters using commercially available standard gait analysis tools. CONCLUSION: The novel indigenous approach validated in this study offers a low-cost and effective tool for routine gait analysis in any under-resourced tertiary or primary care hospital setting.

Aim: Among the disabilities in India, locomotor disability ranks the highest among others due to a growing number of road traffic accidents, diabetic foot, peripheral vascular disease, and cancers that have led to the loss of one or the other limb; the primary aim is to study about how an idea of an orthopedic surgeon Professor S Vetrivel Chezian of creating an artificial limb production center at Coimbatore Medical College and Hospital has changed the lives of locomotor disabled persons and also provided monetary benefit to the government.Background: The idea of an orthopedic surgeon Professor S Vetrivel Chezian at Coimbatore Medical College and Hospital to start a lightweight artificial limb prosthesis production center with Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (AB-PMJAY), Tamil Nadu Chief Minister's Comprehensive Health Insurance Scheme (TNCMCHIS) funds without expenditure to the government but creating revenue to the government at the Institute of Orthopaedics and Traumatology, Coimbatore Medical College and Hospital to benefit locomotor disabled persons.Results: Lightweight artificial limb prosthesis production center started at the Institute of Orthopaedics and Traumatology, Coimbatore Medical College and Hospital, benefiting about 101 locomotor disabled persons to lead their life independently within 2 years of its establishment without any expenditure to the government rather making monetary benefit to the government under AB-PMJAY-TNCMCHIS.Discussion: An innovative idea of Professor S Vetrivel Chezian in using AB-PMJAY and TNCMCHIS fund generated by the Institute of Orthopaedics and Traumatology, Coimbatore, Tamil Nadu, India to establish a lightweight artificial limb prosthesis production center at Coimbatore Medical College and Hospital which gave not only new lives to locomotor disabled persons of very low socioeconomic status but also generated monetary benefit to the government for the infrastructure development of the institution.Conclusion: An idea by orthopedic surgeon Professor S Vetrivel Chezian is to be followed in all government institutions in India, which will lead locomotor disabled persons of the country to lead their life independently with confidence and also generate monetary benefit to the institution to improve health care in India.

Validity of percutaneous external fixator for calcaneal fractures to allow early weight bearing

Calcaneum, the most common tarsal bone to fracture affecting manual labourers who were sole breadwinners of the family. Generally treated with below knee pop and ORIF, commonly associated with complications and delayed weight bearing. This study analyses functional outcome of early weight bearing with minimally invasive skeletal stabilisation of calcaneal fractures. This study was conducted at Institute of orthopaedics and traumatology, Coimbatore medical college and Meenakshi medical college hospital and research institute, Kanchipuram between July 2019 to October 2021. 15 patients in age group 20-65 years with 19 calcaneal fractures less than 4 weeks duration are included in study. Surgery averaged 1.6 days after injury. Full weight-bearing was achieved in 11.7 days for unilateral calcaneal fractures (range 9 to 15 days) and 43.5 days for bilateral fractures (40 to 49 days). Patients returned to work in 35.6 days on average (range 24 to 52 days). All 19 calcaneal fractures healed in 52.63 days. External fixator removal averaged 56.57 days. Two patients had postoperative pin site infections. No bones were infected. Six-month AOFAS scores averaged 86.15. In our study calcaneal fractures are fixed with mini external fixator percutaneously and it can be used in presence of compound fractures and with skin blisters. Though anatomical reduction cannot be attained completely weight bearing is possible from 2nd day after surgery and full weight bearing was possible in 2nd week. The limitations of our study include a smaller number of cases. A large multi-centric study may be necessary to confirm our results.
