Satisfaction Rate of the Families of Patients Hospitalized in Intensive Care Unit with the Quality of Care: A Cross-Sectional Descriptive Study

Abstract Background Satisfaction of patients and their families is a major indicator of healthcare quality improvement. This study aimed to investigate the satisfaction of the families of the hospitalized patients and its relationship with the demographic variables of the patient and family in the intensive care unit. Methods This cross-sectional descriptive study was conducted in 2022 with the participation of 500 family members of patients hospitalized in the intensive care unit of four hospitals of Tehran. Data was collected via the completion of the modified FS-ICU24 questionnaire by a family member on third day of the patients hospitalization. Stepwise multiple regression was used to investigate the demographic factors influencing the satisfaction of the patients families. Results Families’ level of satisfaction was good with the quality of care in the intensive care unit (71%). Families were the least satisfied with meeting religious needs with an average of 3.07 (.86) and the most satisfied with respect to the patient with an average of 3.37 (.65). Also, the history of the patient’s hospitalization in the intensive care unit (P < .001), number of visits to the unit (P < .001) and the relationship with the patient (P = .024) were significantly related to family satisfaction. Conclusion There has not been enough attention to spiritual, religious, educational needs and communication with the families of patients hospitalized in the intensive care unit. Therefore, appropriate interventions are recommended to enhance satisfaction.

Psychometric properties of self-assessment clinical competency questionnaire in baccalaureate nursing students

One of the main challenges of nursing student education is the lack of clinical competence that reduces the quality of patients care. Measuring the clinical competency of undergraduate nursing students is effective in curriculum developing, planning, and learning evaluation.To validate self-assessment clinical competence questionnaire of undergraduate nursing students.This was a cross-sectional study at Baqiyatallah University of Medical Sciences in Tehran, Iran, 2015-2016. The original version of the self-assessment questionnaire for baccalaureate nursing graduates was translated to Persian by the World Health Organization's (WHO) method. The psychometric evaluation of the questionnaire was conducted on 300 nursing students. The participants were selected by convenience sampling. The tool validity was confirmed by exploratory factor analyses. The reliability was investigated by internal consistency and test-retest.Cronbach's alpha coefficient was 0.96 (0.82-0.91). Intraclass correlation coefficient (ICC) was 0.98 (0.88-0.99). Five factors were extracted by analysis of clinical competency tool of undergraduate nursing students. The factors included technical competence, advanced competence, ethical competence, care management and safety competence.The Persian version of the clinical competence assessment tool has acceptable psychometric characteristics in the Iranian nursing students' community. This questionnaire can be used as a valid tool in planning and research management areas.

The Effect of Education and Implementation of Evidence-Based Nursing Guidelines on Infants’ Weight Gaining in NICU

Background: Educating evidence-based guidelines influences increased quality of nursing cares effectively. Infant’s weight gaining is one of the most important indicators for measuring quality of nursing care in NICU. The research is conducted with the aim of surveying the effect of education and implementation of educating evidence-based guidelines on infants’ weight gaining in NICU. Methods: This two-group clinical trial study was conducted in 2013 on one hundred infants in Baqiyatallah (AJ) hospital of Tehran. It was performed by using non-probable and convenient sampling. Data collection tools included; infants’ demographic questionnaire and a researcher-made checklist to record infants’ weight by using a weighing scale. Infants’ weight was recorded before intervention and two months after implementation of the guidelines, then data were analyzed by using SPSS19 statistical software. Findings: Mean weight of the infants in the control group on admission and on discharge was respectively; 1771(41.71) and 1712(42.68), and mean weight of the infants in intervention group on admission and on discharge was respectively; 1697(37.63) and 1793(40.71). After two months, infants’ weight gaining in intervention group was more than control group and it was statistically significant (P = 0.001). Conclusion: results of the present study showed that implementation of evidence-based instruction an effective and economical method regarding infants’ weight gaining. Therefore it is recommended to the authorities and managers of the hospitals and educational centers of the healthcare services to put education and implementation of educating evidence-based instruction the priority of their work plans.

Experiences of role model instructors and nursing students about facilitator factors of role-modeling process: A qualitative research.

One of the key strategies in students' learning is the influence of models on them. Understanding the factors affecting the implementation of role-modeling process in education will help to make greater use of this training strategy. This study aimed to understand the experiences of role model instructors and nursing students about the facilitator factors in the role-modeling process.This qualitative study was carried out by using thematic analysis method and purposeful sampling. Data were collected until saturation by using three focus group discussions (n = 20) and two individual interviews with nursing instructors, as well as six semi-structured face-to-face interviews with role model instructors from five nursing faculties of Tehran universities in 2011.Six themes, "effort for humanistic and professional growth of students," "individual and managerial empowerment of instructor," "instructor and student's modeling," "motivation and effort of student," "strategies governing the education system," and "appropriate facilities and equipment," were extracted as the facilitating factors.For development of role-modeling process in nursing education, paying attention to personal and environmental factors, especially effort for professional and humanistic growth of nursing students is necessary.

PROFESSIONAL CONCERNS OF ROLE-MODEL NURSING INSTRUCTORS: A QUALITATIVE RESEARCH

Background The present study was conducted to perceive the role-model nursing instructors’ experience in terms of professional challenges and identification of occupational concerns in the education of nursing students. Nursing instructors have a critical and decisive role in the training of skilled and specialized manpower and subsequent growth and development of the communities. So, the present study was conducted to address the role model nursing instructor’s experience in terms of professional challenges of nursing students’ education. Methods The present qualitative study was carried out using the inductive thematic analysis method and the objective-based sampling. Data were collected until saturation, using three focus-group discussions (n=22) and two individual interviews with nursing instructors, as well as eight semi-structured face-to-face interviews with role-model instructors from five nursing faculties of the universities in Tehran. Results The three main themes were extracted, indicating the occupational challenges perceived by the instructors as follows: “Inefficient education-research system, ” “Inappropriate professional position,” and “Ineffective interpersonal interactions”. Conclusion Regarding the concerns of successful model instructors, professional development, prevention of job dissatisfaction, and implementation of role modeling strategies are recommended to be emphasized. Taken together, reducing the clinical training-associated problems in order to modify the education-research system was recognized as the main professional concern of the role-model nursing instructors.
