Structural and functional characteristics of the heart before and after CRT in patients with heart failure

The aim of the study was to evaluate the clinical and hemodynamic effects of resynchronization therapy in patients with congestive heart failure. Materials and methods: Seventy-six consecutive patients underwent echocardiography, NYHA classification, 6-minute walk test and clinical assessment scale modified by Mareev, before and after cardiac resynchronization therapy. All had complete left bundle branch block, with a QRS complex duration ≥ 130 ms. and left ventricular ejection fraction ≤ 35%. Also, all patients had received optimal medical therapy for at least 3 months before inclusion to the study. Results: We observed significant increase in left ventricular ejection fraction (35.4±3.7%, p < 0.001, compared with baseline) and decrease in end-systolic volume of the left ventricle (20.2 ±3.0 %, p < 0.001 compared with baseline). Improvement in functional class of congestive heart failure by NYHA classification by > 1 was observed in 68.4% of individuals, in 26.3% of participants demonstrated no change and 5.3% of patients had worsening of CHF symptoms. Conclusions: The response of patients with congestive heart failure to cardiac resynchronization therapy is heterogeneous. The relationship between left ventricular reverse remodeling and the functional class of the congestive heart failure was not significant. Распространенность факторов риска основных сердечно - сосудистых заболеваний среди жителей Южно - казахстанской области

Изучены некоторые социально-демографические показатели и распространенность факторов риска основных ССЗ в ходе проведения комплексного скрининга состояния здоровья взрослого неорганизованного населения различных регионов ЮКО (2000 человек). Использовался анкетный метод, проводилось физикальное обследование, определялись ОХС и глюкоза в крови экспресс-методом с помощью тест-полосок на аппарате Acutrend. Для оценки 10-летнего риска развития фатальных сердечно-сосудистых событий нами была использована шкала SCORE. Результаты исследования показали, что самым распространенным фактором риска ССЗ у жителей ЮКО является избыточная масса тела (51,8%). Частота АГ составляет в среднем 32,6%, повышенный уровень ОХС и гипергликемия обнаружены у 17,2% и 10,3% лиц соответственно. Среднее число курящих, в целом, составило 14,2%. При сравнительном анализе результатов скринингового обследования населения различных регионов ЮКО установлено, что в с. Сайрам по подавляющему числу изученных показателей, отражающих здоровье населения, отмечается наиболее благоприятная ситуация по сравнению с остальными обследованными населенными пунктами ЮКО.Женщин (в целом по ЮКО) с низким риском развития фатальных сердечно - сосудистых событий (48,2%) в 4 раза больше, чем мужчин (12,9%), тогда как мужчин с высоким и очень высоким риском (28,5%) было существенно больше, чем женщин (4,2%).

DRUG AND CARDIAC RESYNCHRONIZATION THERAPY IN THE TREATMENT OF CHRONIC HEART FAILURE (LITERATURE REVIEW)

The article deals with the role of pharmacological agents and implantable devices in the treatment of chronic heart failure (CHF) with different ejection fraction: preserved ejection fraction, mildly reduced ejection fraction and reduced ejection fraction (HFpEF, CHFmrEFV, CHFrFV). The authors emphasise that despite the widespread introduction of angiotensin receptor and neprilysin inhibitor (ARNI), beta-blockers, mineralocorticoid receptor antagonists (MCRAs) and sodium-glucose cotransporter inhibitors (SGLT2) into practice, there is still a high residual risk of unfavorable outcomes in patients with both HFpEF and CHFmrEFV, CHFrFV. Therefore, therapies using implantable cardioverter-defibrillators (ICDs) and cardiac resynchronisation therapy (CRT) are evolving. The review is based on data from PubMed and Google Scholar databases for the last 5 years.
