Onlay bone grafting simultaneous with facial soft tissue augmentation in a hemifacial microsomia patient using de-epithelialized orthograde submental flap: a technical note

Soft tissue augmentation in hemifacial microsomia patients is a challenging procedure. Free microvascular flap transfer is considered usually as the most accepted choice. On the other hand, bone grafting, simultaneous with facial soft tissue augmentation using de-epithelialized orthograde submental flap, is a suggested procedure. Moreover, preoperative evaluation of facial artery and anterior belly of the digastric muscle are essential steps for success in such flaps. Furthermore, bone suture technique helps achieve more predictable results and reduces the need for postoperative bulky dressing.

Reconstruction of the maxilla by submental flap

Abstract Background Palate separates the oral and nasal cavities, hence its reconstruction after pathologic resections or trauma avulsions is important for speech, degulation, mastication and aesthetic facial results. Methods In a retrospective study, the cases that were treated by submental flap for palatomaxillary reconstruction during 2007–2012 were selected from the archive files. Size of the skin paddle, type of the flap and the pathology responsible for resection were recorded. Results In the search of the files, there were 10 patients in which submental flap were used for palatomaxillary reconstruction. The most common complication was fistula at the junction of the palate (20%). All flaps survived and donor site healed without any complication. Treatment for dental rehabilitation was performed in 80% of the patients (60% with conventional removable prostheses, 20% with implant‐supported dentures). Conclusion Submental flap in pedicled form (orthograde or retrograde) can be used for reconstruction of the maxilla in Brown class 2a maxillectomy.

Platelet-rich plasma as filling material in open sinus lift surgery

Background Many biomaterials are used during the open sinus lift surgery, beneath the elevated scheniderian membrane and maxillary sinus bony floor but the search for Ideal material continues. Methods In the maxillary posterior atrophic ridges with the bone height less than four millimeters the open sinus lift surgery performed. The autologous Platelet-rich plasma (PRF) was used as the sole grafting material. Dental implant insertion was not attempted at the same cession. Results Three month after surgery, good density of the bone was observed in OPG. Conclusion Use of Platelet-rich plasma as grafting material in open sinus lift should be considered. It has benefit of omitting the need for donor site for bone grafting. Keywords: Platelet-rich, plasma, sinus

Summary: Delayed reconstruction of large lower lip defects after traumatic avulsion is a challenge in medically compromised patients with concomitant cheek skin burn. Combination of orthograde submental transposition flap and anteriorly based ventral rectangular myomucosal tongue flap is useful. The former reconstructs the body of the defect, and the latter masks the red lip, resembling vermilion. In this article, the detailed surgical technique is explained. Literature review of tongue flap and submental flap in the lower lip reconstruction is presented.

Posteriorly based lateral tongue flap for reconstruction of large palatal-alveolar fistulas in cleft patients

Large palatal fistula in cleft patients is a difficult situation, especially with previous multiple surgeries, which have led to severe scars in the palatal mucosa. Tongue flaps are useful aids in such situations.Seven cleft patients who were reconstructed by posteriorly based lateral tongue flap between 2005 and 2012 were studied. Variables such as flap-ability to close the fistula, remaining tongue shape at least 1 year after operation, and speech improvement (patients' self-assessment) were evaluated.Age range of the patients was 14‒45 years. The male-to-female ratio was 2/7. Posteriorly based lateral tongue flap effectively closed the large fistula in 6/7 of patients. The largest dimensions of fistula closed by this flap was 5 cm × 1.5 cm. Follow-up of 2‒7 years showed that the tongue never returned to the original size and remained asymmetrical. In addition, the nasal speech did not improve dramatically after the closure of large palatal/alveolar fistulas in this age group.Posteriorly based lateral tongue flap is an effective method to solve the problem of large palatal fistulas in adult cleft patients. The most useful indication for this flap is a large longitudinal palatal fistula, extending to the alveolar process. Asymmetrical tongue shape after surgery is the rule and speech improvement depends on patient's age and location of fistula.

Open Sinus Lift Surgery and the Importance of Preoperative Cone-Beam Computed Tomography Scan: A Review.

Open sinus lift surgery is a form of pre-prosthetic surgery for increasing the quality and quantity of bone in the posterior region of the maxilla. Pre-operative assessment of the maxillary sinus is essential for the success of this surgery. PubMed search was carried out in English language literature for open sinus lift surgery and cone-beam computed tomography (CBCT). The results focused on anatomic variants, vascular anatomy, complications, osteotomy/ostectomy window dimensions and thickness of the Schneiderian Membrane. 59 articles were included in this review. Features other than the height and the width of the residual alveolar ridge that should be evaluated in preoperative CBCT scan include the thickness of the lateral maxillary sinus wall, the presence of the alveolar antral artery and its diameter, the maxillary sinus floor width and angulation, irregularity of sinus floor, intimate relation of Schneiderian membrane with the roots of the adjacent teeth, sinus septum, and the quality of subantral bone. Other conditions that occasionally may be observed in special situations are also explained. More than ten parameters should be checked in evaluating CBCT images of paranasal sinuses other than the width and the length of the residual ridge in the posterior region of the maxilla. Each of them may have a significant impact on the results of the open sinus lift surgery.
