Activation of Complement System during Ship Voyage and Winter-over Expedition in Antarctica

The Antarctic milieu comes with extreme cold, intense UV radiation, white-outs, varying magnetic fields, sleep deprivation, altered circadian biorhythms, memory loss, isolation,-causing significant physiological and psychological stresses. These conditions closely resemble space travel, with striking similarity in character of peculiar climate, remoteness, constrained living spaces, disturbed sleep cycles, and environmental stress. Due to its extreme weather conditions, and unique geo-location around the South Pole, it is one of the last frontiers for the human race. Thus the geographic isolation and prolonged stressful stay at Antarctica makes it interesting to study the body responses of expeditioners under extreme environmental conditions. Two teams participated in the study, in 28th Indian Scientific Expedition to Antarctica. The first team included members for short "summer" expedition, while the second team as "Winter team", included members exposed for long term to Antarctic environment. The present study was performed to study the changes occurring in proteomic profile related to immune system. Blood samples were collected in chronological sequence, including i) baseline control ii) during ship journey and iii) at Maitri (Antarctica), during the months of March, May, August and November. The sera samples collected were subjected to differential expression of protein analysis by 2D Gel electrophoresis followed by MALDI-TOF analysis. Catecholamine was also estimated as a measure of stress. Results of 2D gel and MALDI-TOF-TOF analysis revealed significantly increased level of Complement Factor 3 during ship journey and during peak winter in August. Further detailed analysis of complement pathway by ELISA revealed activation of complement system during the acute and chronic stress of Antarctica.

Solid Lipid Nanoparticles Embedded Hydrogels as a Promising Carrier for Retarding Irritation of Leflunomide

Leflunomide (LEF), a disease-modifying anti-rheumatic drug, has been widely explored for its anti-inflammatory potential in skin disorders such as psoriasis and melanoma. However, its poor stability and skin irritation pose challenges for topical delivery. To surmount these issues, LEF-loaded solid lipid nanoparticles (SLNs) integrated with hydrogels have been developed in the present investigation. SLNs developed by microemulsion techniques were found ellipsoidal with 273.1 nm particle size and -0.15 mV zeta potential. Entrapment and total drug content of LEF-SLNs were obtained as 65.25 ± 0.95% and 93.12 ± 1.72%, respectively. FTIR and XRD validated the successful fabrication of LEF-SLNs. The higher stability of LEF-SLNs (p < 0.001) compared to pure drug solution was observed in photostability studies. Additionally, in vitro anti-inflammatory activity of LEF-SLNs showed good potential in comparison to pure drugs. Further, prepared LEF-SLNs loaded hydrogel showed ideal rheology, texture, occlusion, and spreadability for topical drug delivery. In vitro release from LEF-SLN hydrogel was found to follow the Korsmeyer-Peppas model. To assess the skin safety of fabricated lipidic formulation, irritation potential was performed employing the HET-CAM technique. In conclusion, the findings of this investigation demonstrated that LEF-SLN hydrogel is capable of enhancing the photostability of the entrapped drug while reducing its skin irritation with improved topical delivery characteristics.

Unravelling the Oral–Gut Axis: Interconnection Between Periodontitis and Inflammatory Bowel Disease, Current Challenges, and Future Perspective

As the opposite ends of the orodigestive tract, the oral cavity and the intestine share anatomical, microbial, and immunological ties that have bidirectional health implications. A growing body of evidence suggests an interconnection between oral pathologies and inflammatory bowel disease [IBD], implying a shift from the traditional concept of independent diseases to a complex, reciprocal cycle. This review outlines the evidence supporting an 'oral-gut' axis, marked by a higher prevalence of periodontitis and other oral conditions in IBD patients and vice versa. We present an in-depth examination of the interconnection between oral pathologies and IBD, highlighting the shared microbiological and immunological pathways, and proposing a 'multi-hit' hypothesis in the pathogenesis of periodontitis-mediated intestinal inflammation. Furthermore, the review underscores the critical need for a collaborative approach between dentists and gastroenterologists to provide holistic oral-systemic healthcare.

A transdermal patch is a medicated adhesive patch that is placed on the skin to deliver a specific dose of medication through the skin and into the bloodstream.Often, this promotes healing to an injured area of the body.An advantage of a transdermal drug delivery route over other types of medication delivery such as oral, topical, intravenous, intramuscular, etc. is that the patch provides a controlled release of the medication into the patient, usually through either a porous membrane covering a reservoir of medication or through body heat melting thin layers of medication embedded in the adhesive.Transdermal drug delivery offers controlled release of the drug into the patient, it enables a steady blood level profile, resulting in reduced systemic side effects and, sometimes, improved efficacy over other dosage forms.The main objective of transdermal drug delivery system is to deliver drugs into systemic circulation through skin at predetermined rate with minimal inter and intrapatient variations. INTRODUCTION:During the past few years, interest in the development of novel drug delivery systems for existing drug molecules has been renewed.The development of a novel delivery system for existing drug molecules not only improves the drug's performance in terms of efficacy and safety but also improves patient compliance and overall therapeutic benefit to a significant extent. 1 Transdermal Drug Delivery System (TDDS) are defined as self contained, discrete dosage forms which are also known as "patches" 2, 3 when patches are applied to the intact skin, deliver the drug through the skin at a controlled rate to the systemic circulation. 4TDDS are dosage forms designed to deliver a therapeutically effective amount of drug across a patient'sskin. 5

Unraveling the Link between Periodontitis and Inflammatory Bowel Disease: Challenges and Outlook

Periodontitis and Inflammatory Bowel Disease (IBD) are chronic inflammatory conditions, characterized by microbial dysbiosis and hyper-immunoinflammatory responses. Growing evidence suggest an interconnection between periodontitis and IBD, implying a shift from the traditional concept of independent diseases to a complex, reciprocal cycle. This review outlines the evidence supporting an Oral-Gut axis, marked by a higher prevalence of periodontitis in IBD patients and vice versa. The specific mechanisms linking periodontitis and IBD remain to be fully elucidated, but emerging evidence points to the ectopic colonization of the gut by oral bacteria, which promote intestinal inflammation by activating host immune responses. This review presents an in-depth examination of the interconnection between periodontitis and IBD, highlighting the shared microbiological and immunological pathways, and proposing a multi-hit hypothesis in the pathogenesis of periodontitis-mediated intestinal inflammation. Furthermore, the review underscores the critical need for a collaborative approach between dentists and gastroenterologists to provide holistic oral-systemic healthcare.

Impact of the harsh Antarctic environment on mucosal immunity

Abstract Mucosal immunity of Indian Antarctic personnel was analysed during the 34th Indian Scientific Expedition to Antarctica (ISEA) by ship voyage. Serum and salivary IgA, IgA1 and IgA2 levels along with salivary cortisol and TGF-β were quantified by enzyme-linked immunosorbent assay. Samples were collected at three different time points (T1, T2 and T3) during the expedition. Serum and salivary IgA, IgA1 and IgA2 concentrations incrementally increased towards the end of the expedition as compared to the beginning of the expedition. Salivary IgA and TGF-β levels were significantly altered during the expedition. Levels of IgA1 ( P = 0.0007) and IgA2 ( P = 0.0135) increased significantly at T3 as compared to T1. Additionally, significant changes in serum IgA were observed, with peak levels at T3 ( P = 0.0015) and T2 ( P < 0.001). However, the level of serum IgA2 was also significantly altered at T3 ( P < 0.05) and T2 ( P = 0.0006) in comparison with T1. The exact cause of the changes in serum and salivary IgA, IgA1, IgA2 and TGF-β levels during the summer expedition are unknown; however, the changes are evident in mucosal immunity.
