We assessed the nitrous oxide (N2O) gas-barrier properties of a new endotracheal tube cuff, the Profile Soft-Seal Cuff (Resistant: R) (Sims Portex, Kent, UK).The tracheas of randomly selected patients were intubated with the Profile Cuff (Permeable: P) (Sims Portex) tuble or with Portex Soft-Seal Cuff (R) (n=20 each) endotracheal tube. Cuffs were inflated with air, and intracuff pressure was measured during anesthesia using 67% N2O. Postoperative sore throat was assessed. In addition, the volume-pressure relationship of the cuff was determined in vitro.Cuff pressure increased gradually during anesthesia in both groups. The mean cuff pressure of the group R was significantly lower than that of the group P from 10 minutes to 230 minutes. The inflated gas and the deflated gas were not significantly different in both groups. The incidence of postoperative sore throat was not significantly different between the two groups. In vitro, the mean cuff pressure of the group R was significantly lower than that of the group P.The difference of cuff pressure is considered due to the difference in cuff compliance.
Data on statin for patients with aortic stenosis (AS) who underwent transcatheter aortic valve implantation (TAVI) are limited. The present study aimed to evaluate the impact of statin on midterm mortality of TAVI patients.
Postoperative delirium (POD) is a complication after surgery which leads to worse outcomes. The frequency of this syndrome is increasing as more elderly patients undergo major surgery. The frequency is around 10-25% but reaches as high as 50% for cardiac surgery. Although intranasal insulin (INI) administration of up to 160 units in patients with cognitive dysfunction and delirium has been shown to improve memory function and brain metabolism without complications such as hypoglycemia, it remains unknown whether INI prevents POD after cardiac surgery METHODS: A multicenter, double-blind, randomized, controlled trial will be conducted at University of Tsukuba Hospital and Tsukuba Medical Center Hospital, Japan, from July 1, 2023, to December 31, 2025. A total of 110 elderly patients (65 years old or older) undergoing cardiac surgery requiring cardiopulmonary bypass will be enrolled and randomly allocated to intranasal insulin or intranasal saline groups. The primary outcome is the incidence of POD within 7 days after surgery. Secondary outcomes include days and times of delirium, screening tests of cognitive function, pain scores, duration of postoperative tracheal intubation, and length of ICU stay.The present objective is to assess whether 80 IU INI administration during surgery prevents POD after cardiac surgery. The results may provide strategic choices to prevent POD in patients with cardiac surgery requiring cardiopulmonary bypass.The trial was registered with the Japan Registry for Clinical Trials with identifier jRCTs031230047 on April 21, 2023.
O desenvolvimento e a evolucao da automacao para aumentar a interatividade com os clientes e maximizar os lucros em estabelecimentos comerciais vem ganhando espaco rapidamente. O presente trabalho tem como objetivo o desenvolvimento de uma chopeira self service, voltada a bares e restaurantes que possuem o consumo de chope como um de seus servicos. Para o trabalho, utilizou-se ferramentas como sensor de temperatura, sensor de vazao e microcontrolador para se ter a automatizacao do equipamento. Os resultados gerados que serviram como base de dados foram obtidos durante o teste com a passagem de agua na chopeira, sendo apos isso, utilizados nos ajustes finais para o teste com cerveja, devido a este ultimo ter como caracteristica, a presenca de CO₂. Cada etapa de montagem e preparacao do projeto foi separada em fases, onde primeiramente tivemos o levantamento do material necessario e custos, apos isso, a construcao da base de resfriamento e por ultimo a programacao, onde os sensores e microcontroladores foram dispostos para realizarem um trabalho em comum. Observou-se apos os ensaios, que para o nao congelamento do chope na serpentina, e necessario que um liquido anticongelante seja distribuido em volta dela, nao havendo formacao de camada de gelo e melhorando o controle de temperatura pelo sensor. Atraves do liquido coletado na saida do equipamento e os dados registrados pelos controladores referentes a temperatura e vazao, observou-se que os resultados gerados foram satisfatorios para uma baixa vazao. Como os instrumentos que dispomos no momento em que o trabalho se desenvolveu nao sao todos os mais indicados para se trabalhar com o chope, se torna dificil o avanco no rendimento do equipamento. Para que o processo de retirada da bebida se concretizasse totalmente, houve a necessidade de adaptacoes de algumas pecas para suprir a demanda do trabalho, como por exemplo, conexoes, sensor de vazao e disposicao da torneira na montagem de certa forma que nao haja perda de temperatura ao ocorrer a transferencia do liquido entre a serpentina.
