Rivastigmine Adverse Effects: Complete Heart Block

Context: Rivastigmine (Exelon), a brain selective cholinesterase inhibitor, is used in the treatment of Alzheimer’s Disease (AD) to improve cognitive functions. Case Report: We report a case in which the addition of rivastigmine to memantine and metoprolol resulted in complete heart block and possibly predisposing a patient to syncope. Conclusion: Rivastigmine should be used with caution, especially in patients with such structural heart diseases as aortic stenosis and those taking other medications that may contribute to bradycardia. Acute Pulmonary Embolism Masquerading as Acute Inferior Myocardial Infarction

It is always important to maintain a high index of suspicion in the diagnosis of acute pulmonary embolism (PE). Death in patients with undiagnosed PE was shown to be 4 - 6 times higher than in those with a definite diagnosis and treatment. The ECG as much as it has great utility, unfortunately lacks sensitivity and specificity, providing findings that might serve as red herrings to the unsuspecting physician by suggesting an alternative diagnosis. This report describes a woman whose EKG showed ST segment elevations suggestive of acute inferior wall myocardial infarction (MI), but eventually was found to have a sub-massive PE.

After coronary artery bypass graft (CABG) surgery, patients can present with challenging lesions in the setting of acute coronary syndrome, particularly with high clot burden. Techniques including thrombectomy and local delivery of glycoprotein IIb/IIIa inhibitors have been utilized for these saphenous venous graft lesions. We report a case involving a post-CABG patient presenting with an acute ST-segment elevation myocardial infarction due to great clot burden in his bypass graft and successful restoration of thrombolysis in myocardial infarction (TIMI) grade 3 flow after rheolytic thrombectomy followed by localized IIb/IIIa administration.

Lemierre’s Syndrome: A Rare Case of Pulmonic Valve Vegetation

Lemierre’s syndrome is an uncommon complication of pharyngitis commonly associated with an anaerobic gram negative bacterium, Fusobacterium necrophorum . The syndrome usually affects young healthy adults with the mean age of 20 and is characterized by recent pharyngitis followed by ipsilateral internal jugular vein thrombosis and septic thromboembolism. The treatment is at least 6 weeks of antibiotics; the role of anticoagulation is unclear. The following presentation is a case of Lemierre’s syndrome in a 23-year-old healthy individual who is infected by a rare species: Fusobacterium nucleatum . The case is complicated by septic emboli to the lungs and impressive seeding vegetation to the right ventricular outflow tract (RVOT) at the pulmonic valve of the heart.
