Recurrent Pulmonary Tuberculosis in Low Prevalence Settings: Case Report from Louisiana

Recurrence of Pulmonary tuberculosis (TB) after successful completion of supervised treatment may be due to relapse or reinfection.Recurrence is common with an incidence rate of 2.26 per 100 person years over a mean follow up of 2.3 years.However, it is uncommon for an immunocompetent individual to experience multiple episodes of recurrences.We describe a 67yo, Human Immunodeficiency Virus (HIV) seronegative man, seen in the public health system in Louisiana, USA with multiple relapses of TB due to drug susceptible MTB despite successful completion of guideline based direct observed treatment regimen in each episode.This case report highlights the need for increased awareness and recognition of risk factors contributing to the recurrence of TB even in countries with low prevalence of TB.Better understanding of the importance of drug penetration and action on varying mycobacterial populations within diseased lung tissue would also aid clinicians in the management of these relapses.

Homocysteine, Vitamins B6, B12, and Folate and the Risk of Ischemic and Hemorrhagic Stroke: A Case-control Study from Northeast India

Stroke is a major leading global health complication. Identification and management of risk factors associated with stroke can help in prior detection, prevention, and improvement in patient care.To investigate the prevalence of hyperhomocysteinemia (HHcy) and Vitamins B6, B12, and folate deficiency in stroke patients and also to assess other risk factors associated with ischemic and hemorrhagic stroke.Detail history of all the subjects in the study including history of hypertension, anemia, fasting glucose, carotid artery thickness, smoking, alcohol, and dietary intake was recorded. Standard assays for homocysteine (Hcy), Vitamins B6, B12, and folate estimation were done. Lipid and renal profile tests were also performed. The prevalence and odds of having HHcy, Vitamins B6, B12, and folate deficiency, and other risk factors in ischemic and hemorrhagic stroke patients were evaluated. Student's t-tests and chi-square tests were done for statistical validation of the data.Prevalence of HHcy and Vitamins B6, B12, and folate deficiency was not observed in ischemic cases. HHcy and folate deficiency was found to be prevalent in hemorrhagic stroke patients. The odds that a person with HHcy and folate deficiency has hemorrhagic stroke was found to be significantly high.In our study, high Hcy and low folate levels emerged as risk factors for hemorrhagic stroke.

Significance of Lyapunov Exponent in Parkinson's Disease Using Electroencephalography

A neurodegenerative disorder which is caused due to loss of a neurotransmitter called dopamine in the substania nigra of the mid brain is known as Parkinson's disease (PD). It is mostly affected to the people after the age of 50 years. The disease results in the loss of coordination between brain and muscle; hence the person is disturbed. Electroencephalography (EEG) is one of the primitive methods for studying the human brain function. It is used for the detection of cognitive decline in neurodegenerative disorders including PD. In this paper, we reported a study of the frontal and temporal EEG of Parkinson's disease patients using MATLAB platform. Lyapunov exponent and inverse Lyapunov exponent for both PD and healthy subjects were calculated within a given time frame. It was found that for PD subjects, the Lyapunov exponent for the temporal part of the brain is less than that of the frontal while the inverse Lyapunov exponent is reverse order of the brain. This result signifies that the correlations between neurons are stronger in frontal region for healthy subjects than PD.

A tale of two images: From mycobacterium avium complex-lung disease (MAC-LD) to mycobacterium avium complex-pleural disease

The typical radiographic presentation for Mycobacterium avium complex lung disease (MAC-LD) is either nodular bronchiectasis or cavitary lung disease. The former is seen most commonly in middle-aged or elderly Caucasian females with the characteristic asthenic phenotype, and the latter in middle-aged male smokers with COPD. We present the case of a young, otherwise healthy woman, with no significant risk factors, who was incidentally found to have MAC-LD with associated bronchiectasis. The patient's treatment and clinical course over a period of 5 years was marred by erratic follow up, intermittent treatment and poor adherence to guideline-based antibiotic therapy. Over this period of time, the patient developed significant worsening of her MAC-LD, macrolide resistance and failure to thrive. Upon presentation 5 years after her initial diagnosis, she had developed MAC-Pleural Disease with an empyema and broncho-pleural fistula. This case illustrates the progression of MAC-LD from nodular bronchiectasis to cavitary disease and pleural involvement leading to clinical deterioration. It highlights challenges related to short and long term management of macrolide resistant MAC-LD and the importance and need for surgical intervention and drainage procedures in patient with MAC-Pleural Disease.

A Systematic Review of Breast Implant-Associated Squamous Cell Carcinoma

Breast augmentation is considered safe, but rare cases of breast implant-associated squamous cell carcinoma (BIA-SCC) have been reported. This study aimed to systematically review published cases of BIA-SCC, providing valuable clinical data. The review included 14 articles and 18 cases of BIA-SCC. An increasing trend in reported BIA-SCC cases was observed, with four cases in the 1990s and 14 cases since 2010. The mean age of affected patients was 56 years, and symptoms typically appeared around 21 years after breast augmentation. Silicone implants used in cosmetic procedures were most commonly associated with BIA-SCC. Implant removal was necessary in all cases, and some patients required a mastectomy. Treatment approaches varied, with the selective use of chemotherapy and/or radiotherapy. The estimated 6-month mortality rate was 11.1%, while the 12-month mortality rate was 23.8%. The estimated 6-month mortality rate should be cautiously interpreted due to the limited sample size. It appears lower than the rate reported by the American Society of Plastic Surgeons, without clear reasons for this discrepancy. This study highlights the importance of enhanced monitoring and information sharing to improve detection and management of BIA-SCC. Healthcare providers should maintain vigilance during the long-term follow-up of breast augmentation patients.

Challenges Faced by Rohingya Refugees in the COVID-19 Pandemic

In August 2017, Bangladesh saw a massive influx of Rohingya refugees following their violent persecution by the Myanmar authorities. Since then, the district of Cox's Bazar has been home to nearly 900,000 Rohingya refugees living in the densely populated and unhygienic camps. The refugees have been living in makeshift settlements which are cramped into one another, making it extremely difficult to maintain "social distance". The overcrowded conditions coupled with the low literacy level, lack of basic sanitation facilities, face masks and gloves and limited communication make these camps an ideal place for the virus to spread rapidly. As nations struggle to contain the SARS-CoV-2 virus, refugees are one such population who are extremely vulnerable to the effects of this outbreak. If issues are not addressed at an early stage, its effects can be catastrophic.

Hydroxychloroquine

SARS-CoV-2 is a novel virus that has infected millions of people across the world. Given the compelling need to develop a therapeutic strategy, hydroxychloroquine has been advocated as an effective drug for the infection. However, multiple clinical trials conducted using hydroxychloroquine have yielded contrasting results. An electronic search using the primary databases from WHO, PubMed and Google Scholar was performed that yielded 21 studies eligible for inclusion. Among a total of 1,350 patients who received hydroxychloroquine, 689 (51.04%) were females. The most commonly reported comorbidities include hypertension (15.18%), diabetes mellitus (8.44%) and pulmonary disease (8.96%). Of the hydroxychloroquine-treated patients, 70% were virologically cured compared to 12.5% of the control group ( p = 0.001). A good clinical outcome with virological cure was reported in 973 patients (91%) within 10 days out of 1,061 hydroxychloroquine-treated patients. A total of 29 (65%) renal transplant recipients achieved complete recovery following hydroxychloroquine administration. A total of 37 (2.7%) patients reported QT prolongation. Hydroxychloroquine was found to reduce mortality in healthy, SARS-Cov-2 positive patients and improve clinical recovery in renal transplant recipients. However, a definitive conclusion regarding its effect on viral clearance can only be reached by conducting more clinical trials involving bigger and diverse samples.

DIAGNOSIS AND TREATMENT OF IDIOPATHIC INTRACRANIAL HYPERTENSION AT A TERTIARY CARE MULTISPECIALTY HOSPITAL IN NORTHEAST INDIA

Idiopathic intracranial hypertension (IIH) is a condition of unknown etiology and is characterized by elevated intracranial pressure. In this case series we describe the clinical features and responses to treatment of patients presented with IIH. It is a retrospective study of four patients who were presented in our hospital between August 2020 and December 2021 and all patients ethnically belonged to northeast India. The patients were evaluated and diagnosed based on history, physical examination, imaging as well as modied Dandy criteria. Low doses of acetazolamide were recommended for all the patients. Timely diagnosis and accurate treatment of IIH is of utmost importance to avoid any irreversible visual loss.
