Analysis of factors influencing outcome of depressed fracture of skull

The outcome of patients with depressed fracture varies and depends on multiple factors. There has been no previous study on the significance of these factors on the outcome of depressed fracture of the skull and hence this study. Aims: The primary aim of our study is to find the factors affecting the outcome in cases of depressed skull fracture (DSF). This will help us improve outcomes and give more accurate prediction of long-term outcomes.Prospective observational study.Institutional Ethics Committee approval was taken for doing this observational study. This was conducted in a tertiary care institute by collecting data of fifty cases of DSFs in 2 years, between January 2012 and December 2013. The study included patients who were diagnosed with DSFs admitted to our tertiary care public hospital. Patients with comorbidity involving injury to other organs or medical disorders and pediatric patients were excluded from our study.Chi-square test and Fisher exact test. Results: There was a statistically significant impact on age, sex, Glasgow Coma Scale (GCS) score at presentation, type of DSF, and site of DSF in the long-term outcome of patients. The patients with GCS score of 13 or more fared well with good long-term outcome as against those with GCS score below it. Any additional brain injury in the form of hematomas, etc., has a significant negative impact on long-term outcome of the patient and warrant urgent surgical intervention. Complications such as dural tear, cerebral contusions, wound infections, and seizures have adverse effect on the recovery.Our observation suggests that patients brought to hospital with minimal delay, with GCS score between 13 and 15, with simple DSF and normal brain parenchyma without dural tear, have the best outcome.

A Successful Strategy for Surgical Treatment of Boerhaave’s Syndrome Sans Thoracotomy

Boerhaave's syndrome is a rare life threatening condition that is often misdiagnosed and fatal if not treated promptly. While the gold standard is early surgical intervention, recent studies have showed success with conservative management. We report a case of Boerhaave's syndrome that was managed conservatively by decompressive gastrostomy, feeding jejunostomy, bilateral intercostal drainage tubes with added proximal diverting cervical esophagostomy. The patient recovered completely and stoma closure was done two months later.

Analysis of expression pattern of proteins associated with AKT/mTOR signaling pathway in kidney cancer development

Introduction: The purpose of this study was to analyze the expression status, reciprocal interplay, and prognostic significance of AKT1 and hypoxia-inducible factor 1 alpha (HIF-1α) in AKT/mechanistic target of the rapamycin pathway and to enable them to be studied as possible therapeutic targets. Materials and Methods: This prospective study included 25 patients with clear cell renal cell carcinoma (ccRCC) operated between December 2019 and January 2022. Tumor and adjacent normal tissue samples were subjected to immunohistochemical analysis, RNA extraction, cDNA synthesis, and quantitative real-time polymerase chain reaction for AKT and HIF-1α. The fold changes were then calculated by ∆∆Ct method. Results: The included 25 ccRCC patients had 1.5-fold greater HIF-1 mRNA expression and 0.9-fold higher AKT1 gene expression in the ccRCC tissues compared to the corresponding healthy control. High, moderate, and low expression of HIF-1α was seen in 15, 6, and 1 of 25 samples, respectively. High, moderate, and low expression of p-AKT1 was seen in 18, 2, and 3 of 25 samples, respectively. Conclusion: Our study data predicted higher gene expression as well as protein expression of HIF-1α and AKT. The proteins HIF-1α and AKT are localized in the nucleus of the RCC tumor samples compared to normal. Overexpression of these proteins might play significant roles in tumor development and differentiation as reported by others previously. This study can help clarify the biological role of HIF-1α and AKT in RCC to develop new strategies for this malignancy.

Small Intestinal Obstruction Secondary to Jejunal Trichobezoar Removed Per Anum Without an Enterotomy: A Case Report

Trichobezoars can rarely present with obstruction. This is usually due to collection of a hair ball in the stomach. We encountered an interesting case of small bowel obstruction due to a jejunal trichobezoar. The treatment generally is an enterotomy with removal of the hair ball. We report a case of a 29-year-old post partum female who presented to us with sub acute intestinal obstruction. Exploratory laparotomy revealed an impacted mass in the distal jejunum which was removed per anum without an enterotomy. Postoperative gastroscopy did not show trichobezoar in the stomach. This case highlights the importance of trichobezoar as a differential diagnosis in young women with small bowel obstruction that can be treated without an enterotomy and avoiding the risks and morbidities associated with it.
