An Unusual case of neurocysticercosis leading to blindness in a young Indian male

Neurocysticercosis is an endemic disease in India. It is caused by infection from the larval form of Taenia solium and humans serve as the intermediate host. Its diverse manifestations include seizures, headaches, focal neurological signs. Obstructive hydrocephalus, with its potential for death is the most disastrous complication. We hereby report a case of 24 year old boy presenting with blindness without hydrocephalus.

Primary vitrectomy versus conventional retinal detachment surgery in phakic rhegmatogenous retinal detachment

This study aimed to compare the results of primary vitrectomy and conventional scleral buckling procedures (conventional retinal detachment surgery) in phakic rhegmatogenous retinal detachment (RRD).We carried out a randomized, prospective, clinical controlled trial of 61 consecutive phakic eyes with primary RRD, not complicated by proliferative vitreoretinopathy >or= grade C. Subjects were randomized to either scleral buckling (group 1) or pars plana vitrectomy (group 2).At 6 months follow-up, the primary reattachment rate was 80% (24/30 cases) in group 2 and 80.6% (25/31 cases) in group 1; the difference between the two groups was not statistically significant (p = 0.213). Best corrected visual acuity improved significantly from a preoperative median of 1.78 (1/60) (mean 1.73 +/- 0.91, range 0.3-3) to a median of 0.6 (6/24) (mean 0.689 +/- 0.35, range 0.18-1.48) in group 2 and from a preoperative median of 1.48 (2/60) (mean 1.43 +/- 0.92, range 0-3) to a median of 0.6 (6/24) (mean 0.608 +/- 0.36, range 0-1.78) in group 1; the difference between the two groups was not statistically significant (p = 0.376). Cataract developed in five cases (17%) in the vitrectomy group (group 2), with a statistically significant difference of p = 0.018.Although primary vitrectomy can achieve anatomical and functional success rates comparable with those achieved by scleral buckling in uncomplicated forms of phakic RRD, the major drawback of the procedure is the high incidence of postoperative cataract formation. Moreover, visual rehabilitation takes place earlier with scleral buckling than with vitrectomy. Scleral buckling should thus be used as the primary surgical modality in the treatment of uncomplicated RRD where the media are sufficiently clear.

Vogt–Koyanagi–Harada disease in a patient of chronic myeloid leukemia

This case report describes the concurrent development of Vogt–Koyanagi–Harada (VKH) disease in a 39 year old male patient of chronic myeloid leukemia (CML). The patient being reported was a known case of CML in remission with history of painless sudden loss of vision in both eyes. Cases of leukemia can present with visual loss due to multiple ocular manifestations of leukemia itself or side effects of modern drugs used for its treatment. Clinical examination and multimodal imaging of our patient were suggestive of concurrent development of VKH. The patient was started on oral steroids, to which he showed a good response. Thus, the cases of CML may rarely develop concurrent ocular disorders not related to leukemia. These associated ocular disorders need to be distinguished from the ocular manifestations of leukemia itself. Our case highlights the concurrent development of VKH as the etiology of visual loss in a case of CML.

Diabetic macular edema (DME) is the leading cause of moderate vision loss in diabetics. Modalities to image and monitor DME have evolved much in the last decade. Systemic control is the most important part of management. Available ocular management options include intravitreal antivascular endothelial growth factor (anti-VEGF) agents, laser, steroids (intravitreal or peribulbar), vitrectomy, topical medications and others. Anti-VEGF agents are increasingly being used in clinical practice with good clinical response and are currently the preferred mode of treatment worldwide. Topographic distribution of retinal neovascularization in proliferative diabetic retinopathy using ultra-wide field angiography

To analyze the topographic distribution of neovascularization (NV) and capillary nonperfusion (CNP) using ultra-wide field fluorescein angiography (UWFFA) in patients with proliferative diabetic retinopathy (PDR).This was a prospective, single-center, observational study in which all patients who presented between March 2019 and December 2020 and satisfied the inclusion criteria were recruited. In our study, patients with treatment-naïve PDR without any fibrovascular proliferation underwent UWFFA. The images were analyzed qualitatively for the topographic distribution of NV and the CNP area was quantified. The number of lesions picked by UWFFA was compared with 7 standard field (7SF) image using overlay of 7SF. The main outcome measure was characteristics of neovascularization, such as the number, location, and area of CNP, measured using UWFFA, which was considered with 95% confidence intervals (CI).Two hundred and fifty-three eyes of 187 patients with a mean age of 56.03 ± 8 years were included. Mean neovascularization elsewhere (NVE) was 2.91 ± 3.43. Maximum NVEs were seen in the superotemporal (ST; 0.9 ± 1.13) quadrant, followed by the inferotemporal (IT; 0.7 ± 1.08), inferonasal (IN; 0.66 ± 1.02) and superonasal (SN; 0.66 ± 1.01) quadrants. Maximum CNP area was seen in the SN (13.75 ± 8.83 disc diameter square [DD2]) quadrant, followed by the IN (13.48 ± 8.59 DD2), IT (11.34 ± 8.37 DD2), and ST (11.3 ± 8.34 DD2) quadrants. Mean CNP area was maximum in patients with only neovascularization of disc (NVD; 64.99 ± 41.47 DD2), followed by both NVD and NVE (61.37 ± 35.61 DD2), and was minimum in patients with only NVE (36.44 ± 22.03 DD2). Eighty-one (32%) eyes out of 253 had NVE and 189 (75%) out of 253 had CNP area outside 7SF (overlay) of Early Treatment Diabetic Retinopathy Study (ETDRS).Diabetic NV lesions and CNP areas are distributed asymmetrically throughout the retina and are not restricted to the posterior pole. Compared to conventional 7SF imaging, UWFFA reveals significantly more retinal vascular pathology in patients with PDR.

Cataract in retinopathy of prematurity – A review

Preterm babies with retinopathy of prematurity (ROP) can become blind if they do not receive appropriate timely intervention. The presence of cataract in these individuals in addition to visual deprivation amblyopia, also delays proper screening, adequate treatment, and makes follow-up assessment difficult. Anatomical differences in these infants and amblyopia management, especially in unilateral cataract, are other important concerns, and hence, management of these cases with cataract and ROP is challenging. In this review, studies where ROP cases were associated with cataract, were evaluated with a focus on preterm individuals less than 6 months age. Preterm babies are at increased risk of developing cataract because of systemic factors. In addition, those with ROP may have cataract associated with retinal detachment or treatment received. The type of cataract, risk factors, and pathophysiology associated with each cause varies. This review highlights these different aspects of cataract in ROP including causes, pathophysiology, types of cataracts, and management. The management of these cases is critical in terms of the timing of cataract surgery and the challenges associated with surgery and posterior segment management for ROP. Anatomical differences, preoperative retina status, pupillary dilatation, neovascularization of iris in aggressive posterior ROP, fundus examination, amblyopia, and follow-up are various important aspects in the management of the same. The preoperative workup, intraoperative challenges, postoperative care, and rehabilitation in these individuals are discussed.

Isolated unilateral acute retinal necrosis syndrome as the initial manifestation of HIV infection.

We report a rare case of isolated unilateral acute retinal necrosis syndrome without any history of associated herpes zoster or other conditions attributed to HIV infection as the initial manifestation of HIV infection. A 30-year-old man presented with a history of sudden diminished vision in the right eye with no apparent predisposing factors. Examination revealed right eye acute retinal necrosis. All laboratory test results were normal except the patient tested positive for HIV. This case of an atypical ophthalmic manifestation as initial presentation of HIV infection highlights the need for increased index of suspicion for HIV infection in young patients with acute retinal necrosis syndrome.
