Three Autopsy Cases of Acute Klebsiella Pneumonia

Three autopsy cases of acute Klebsiella pneumonia were reported. Two of them were male leprosy patients in Komyoen Leprosarium and the other a female patient of a general hospital. All of them were over 50 years old. The onset of each case was sudden and they died within one week. However, physical examination of the lungs revealed mild deterioration in all cases. Their symptoms were not limited to the respiratory system.Autopsy findings revealed typical lobar pneumonia due to Klebsiella pneumoniae. Infection of this gram negative bacilli did not relate to leprosy; it is the same as other bacterial pneumonia which occurrs equally between leprous and non-leprous patients. The histopathological findings of the lung showed remarkable lymphocytic infiltration rather than polymorphonuclear leukocytic one; it was different from the description of textbooks. Infiltrating cells of pulmonary lesion were slightly different among the three cases. Old lepromata with a mild lymphocytic infiltration was seen in the liver, spleen or adrenal of leprosy cases. Acid-fast bacilli were not found in these lesions.Klebsiella pneumoniae is thought to exist in normal flora of the human pharynx. In our cases, leukopenia was not the cause of this infection. One leprosy patient was a heavy drinker. Alcoholism seemed to relate to etiology of the pneumonia; there had been little reference in previously reported cases in Japan. Sulfon therapy of long duration for leprosy may effect the human flora, but it was not clear in our cases. For treatment and improvement of prognosis, it is necessary to identify the bacilli and choose sensible antibiotics.

Endobronchial mass lesion due to sarcoidosis: complete resolution with corticosteroids.

Sarcoidosis is a granulomatous disorder of unknown aetiology affecting many systems, including the lungs and mediastinal lymph nodes.The usual manifestations of sar- coidosis on the chest radiograph are well known.Although the bronchial mucosa is often affected in sarcoidosis, endobronchial mass lesions are extremely rare.'Endobronchial lesions are most often due to bron- chogenic carcinoma and less often to relatively benign causes such as bronchial adenoma, lipoma, broncholith, and tuberculosis.26We report the first case of a distinct endobronchial mass lesion due to sarcoidosis that resolved with corticosteroid treatment.

Elemental content in alveolar septa in various pneumoconioses.

The elemental content of alveolar septa in various pneumoconioses was examind for a possible relationship to interstitial fibrosis. Silicon content, as well as heavy metal content, were evaluated to deterrmine possible interrelationships between the various inorganic dusts. Silicon levels were determined on the basis of silicon-sulphur ratios to accommodate for differences in tissue mass. A study of the variation of silicon-sulphur ratios in silicotics and normal individuals showed no false positives by our diagnostic criteria. A study of 11 cases of siderosis revealed that silicon was not responsible for the fibrosis as previously believed. An analysis of the lungs of 25 non-silicotic fibroses revealed that silicon-sulphur ratios were within normal limits. This technique should be useful in quantitating high exposure to certain dusts and in studying the relationship of such dusts to fibrosis.

Pneumoconiosis in workers exposed to silicon carbide.

Two men who had been exposed only to silicon carbide for many years in a factory manufacturing refractory bricks developed bilateral reticulonodular densities, as shown on chest radiograms, and complained of dyspnea. An open lung biopsy of one of them showed a large amount of black material in the fibrosed alveolar septums. Studies by X-ray diffraction revealed that the silicon carbide to which they were exposed did not contain quartz; X-ray powder diffraction analysis of the lung tissue revealed at least 6 different silicon carbides, traces of tungsten carbide, and an insignificant amount of quartz. Line-width analysis of the pattern suggested that the lung contained foreign material, with a significant number of particles smaller than 0.1 micron. The etiologic role of silicon carbide for tissue fibrosis is discussed.
