Assessment of Safety of Retropupillary Iris-Claw Intraocular Lens as a Viable Option in Surgical Aphakia

Background: Aphakia is the most dreaded and inadvertent complication of cataract surgery. Visual rehabilitation with wide variety of surgical options is a challenge for the ophthalmologist. Our study evaluates the safety and efficacy of the retropupillary implantation of the iris-fixated intraocular lens (IOL) in a 2-month follow-up period. Aims: This study aims to assess the visual outcome, identify complications, and assess the long-term stability of eyes undergoing retropupillary iris-claw lens implantation for aphakia. Settings and Design: This was a prospective interventional study (single-group, before–after study). Materials and Methods: Sixty eyes with surgical aphakia fulfilling the inclusion criteria were considered. Preoperatively, evaluation of visual acuity, slit-lamp examination, fundus examination, intraocular pressure, corneal endothelial cell count, macular thickness, and edema was done. Anterior vitrectomy was done, and retropupillary iris-claw lens was implanted. Postoperative best-corrected visual acuity (BCVA), IOP, corneal endothelial cell density, macular edema, and IOL stability were evaluated. Statistical Analysis: The computer software statistical package for the social sciences, version 20.0 (SPSS) was used, and a 0.05 level of significance was considered. Results: The major cause of aphakia was posterior capsular rupture (55%). Postoperative BCVA on day 60 was 6/6–6/18 in 83.33% of cases. Three patients had disenclavation of the haptic which required retucking of IOL. No patient had corneal decompensation, cystoid macular edema, or IOP rise. Conclusion: Our results demonstrate that retropupillary technique of iris-claw implantation is a safe and effective method for visual rehabilitation in aphakic patients. This surgical procedure has an advantage of posterior chamber implantation with a low intraoperative and postoperative complication profile.

Triad of rubric assessment, constructive feedback and video recorded surgeries key assessment for competent ophthalmic surgeon

Ophthalmic surgery involves very precise surgical skill, which is difficult to teach and even more cumbersome in assessment of resident’s surgical skill. Hence it’s a need of time to adopt newer tool for transferring as well as assessing surgical skill. With this concept in mind International Council of Ophthalmology (ICO) has developed various tools for assessing surgical skills. If we use this tool not only as learning tool but also to give constructive feedback on the surgical skills of resident doctors it will help in creating a competent ophthalmic surgeon and eventually help society in general. 1To develop more standardized surgical training; 2. To assess efficacy and feasibility of new tool in improving surgical skills of Post Graduate(PG) student; 3. To know the effect of constructive feedback on surgical performance. Small incision cataract surgery training is done by Rubric designed by ICO- OSCAR. The same tool was used to assess video recorded cataract surgery of residents by different faculties and assess their surgical skill. The assessor simply circled the observed performance description at each step of the procedure. The ICO-OSCAR score was completed. At the end of the case assessor immediately discussed operated case with student to provide timely, structured, specific performance feedback. Oscar score was recorded and analysed with inter rater agreement. OSCAR TOOL has very good inter rater agreement i.e.(0.96). Analysis of student & Observer feedback infers that OSCAR Tool is best tool for learning as well as assessment tool and is easy to use. Recorded surgeries & constructive feedback from assessor helped Post Graduate students to improve surgically. This resulted in best outcome for patient in terms of good visual acuity post operatively. The formative assessment of surgical skills becomes an integral part of our formal residency, training framework, it would be in the interest of our trainees and trainers that we should adopt the OSCAR tools to train and assess. These tools can add immense value to our residency as well fellowship surgical training and possibly help create a generation of competent trainee. Formative Assessment and constructive feedback in surgical training will improve the competency of new ophthalmic surgeons. Structured surgical training will be relatively easy to observe and perform, as trainee learns what is required to be competent. This will ultimately improve the overall quality of patient care.

Comparison of visual acuity in primary and secondary iris claw implantation in western Maharashtra

A potential complication of cataract surgery is posterior capsular rent, where PCIOL can’t be implanted. Primary Iris claw implantation means that the IC-IOL is implanted in the same setting after cataract removal. While secondary Iris claw lens implant is done in an aphakic eye without capsular support and post cataract surgeries when the eye is left aphakic. To compare the visual outcome in cases of primary and secondary iris claw lens implantation after cataract surgery. This was a hospital-based Prospective Interventional Longitudinal study,conducted on patients in a tertiary care centre, in Western Maharashtra from September 2018 to August 2020. 31 eyes underwent primary iris claw implant and 19 underwent secondary iris claw implant. Highly significant difference in visual acuity preoperatively due aphakia among subjects who underwent secondary iris claw implantation (p=0.000) was seen. Highly significant difference in visual acuity post operatively till 15 days was also seen due to less corneal oedema& inflammation in patients who underwent secondary iris claw implantation. No significant difference in visual acuity was seen on day 40 as lens was put in both the groups and the signs of inflammation had reduced (p=0.759). Overall visual outcome after 40 days is comparable in primary and secondary cases. It is better to do a primary retro fixated iris claw implantation, as in case of a secondary implant the patient will have undue anxiety, to undergo a second surgery along with increased financial burden and patient can develop complications due to aphakia.

A study of complications during cataract surgery in patients with pseudoexfoliation syndrome

Aim: Pseudoexfoliation (PXE) is a common and clinically important systemic condition that affects elderly people, who are also likely to undergo cataract surgery. It can cause serious complications during cataract surgery due to the pupillary rigidity and zonular weakness and instability. The purpose of this study was to study the frequency and types of complications of small incision cataract surgery (SICS) and phacoemulsification surgery in patients with cataract and PXE. Materials and Methods: This cross-sectional descriptive study was carried out on 52 eyes of 52 patients with cataract and PXE who underwent SICS or phacoemulsification surgery in a tertiary care hospital. Their perioperative and post-operative complications were documented and analyzed through statistical package for social sciences version 15.0 (IBM). Results: Poor pupillary dilatation in spite of use of standard mydriatic drops and non-steroidal anti-inflammatory drugs was the most common finding. This single factor made subsequent steps of surgery very difficult due to poor peripheral visualization. Pupillotomy was done in 25% cases. Other problems encountered were accidental iridodialysis, posterior capsule rupture, vitreous loss, retained cortical matter, decentered intraocular lens and zonular dialysis. Conclusion: Presence of associated PXE in cataract patients significantly increases the risk of vision threatening complications. Use of flexible iris hooks for small pupils and for capsular stability, capsular tension rings and high viscosity viscoelastics are useful modifications of surgical technique for good visual outcome.

Analysis of conjunctival flora in diabetic and non-diabetic individuals and their antibiotic sensitivity pattern

To compare the conjunctival flora of diabetic individuals and non-diabetic individuals and assess their antibiotic sensitivity pattern. A hospital based cross-sectional and descriptive study was carried out from September 2016 to August 2018 with 92 patients of age group from 40-70 years, to assess the conjunctival flora and antibiotic sensitivity pattern in diabetic and non-diabetic individuals without any pre-existing ocular diseases and adnexal disease. The rate of positive culture in Group A (Diabetics) was significantly higher as compared to Group B (Non-Diabetics) (21.7% vs. 4.3% respectively) (p<0.05). It was noted that there was a significantly higher incidence of the bacteria, Staphylococcus epidermidis in Group A (Diabetics) as compared to Group B (Non-Diabetics) (p<0.05). The antibiotic sensitivity of bacteria isolated in patients in Group A (Diabetics) showed the following observations - in Staphylococcus epidermidis, Staphlococcusaureus maximum sensitivity was seen with Gentamicin. A higher positive culture rate was seen in diabetic individuals as compared to non-diabetic individuals. It was then concluded that, as gentamicin shows maximum sensitivity to gram-posit ive and gram-negative organisms, Gentamicin is the drug of choice which takes cares of gram positive as well as gram negative bacteria and can be considered as routine pre-operative topical medication. Comparative study of endothelial cell loss in trabeculectomy with combined small incision cataract surgery (SICS), trabeculectomy procedure and temporal small incision cataract surgery (SICS) alone

When a patient suffers from co-existing cataract and glaucoma, the choice of surgery may be a combined procedure of cataract extraction and trabeculectomy as a one-stage procedure, or trabeculectomy first to restore visual rehabilitation and to prevent progressive optic nerve damage. The main purpose of this study was to evaluate the endothelial cell loss in trabeculectomy with small incision cataract surgery (SICS) against trabeculectomy and SICS alone. Materials and Methods: This study was conducted on 90 eyes divided into 3 groups among whom 30 cases underwent trabeculectomy through a superior incision, 30 cases underwent SICS with PC-IOL implantation with trabeculectomy through a common superior incision and 30 patients underwent manual SICS with PCIOL. The corneal endothelial cell count and loss was assessed on first postoperative day, 1st week, 1st month and at 3 months. IOP was assessed on postoperative 1st week, 1st month and at 3 months. Results: The endothelial cell loss was more in combined SICS with trabeculectomy group than trabeculectomy group alone. Conclusion: Preoperative evaluation of endothelial cell count must be considered and adequate intra-operative and postoperative measures should be taken to avoid cell loss. However, to avoid further endothelial cell loss due to a two staged surgery, a combined procedure would be a better choice. Combined SICS with trabeculectomy would also reduce the economical burden, postoperative morbidity and the visits of patient to operation theatre.
