Level and function of epidermal growth factor receptor predict the metastatic potential of human colon carcinoma cells.

The purpose of this study was to determine whether production of liver metastasis by human colon carcinoma (HCC) cells depends on the response of tumor cells to organ-derived growth factors. HCC cells were isolated from several surgical specimens whose malignant potential differed (Dukes' stage B or D tumors), adapted to grow in culture, and assessed for expression of the epidermal growth factor receptor (EGF-R). Northern blot analyses revealed that highly metastatic HCC cells expressed >5-fold the number of EGF-R mRNA transcripts as low metastatic cells. The level of mRNA correlated with the amount of EGF-R protein as detected by Western blotting, immunohistochemistry, and Scatchard analyses. HCC growth response in vitro to picograms of transforming growth factor alpha was associated with functional cell surface EGF-Rs as determined by receptor tyrosine kinase activity assays. The EGF-R gene was not amplified or rearranged in highly metastatic cells. However, fluorescence in situ hybridization analysis showed that the copy number of chromosome 7 was higher in the highly metastatic cells. HCC cells were selected in vitro for low or high expression of EGF-R. Subsequent to injection into nude mice, only cells with high expression of EGF-R produced a high incidence of liver metastasis. These data demonstrate that expression of EGF-R by HCC cells directly correlates with their ability to produce hepatic metastasis.

[Effect of unfractionated histones on isolated mouse liver lysosomes].

Total histones increased the permeability of lysosomal membranes for enzymes of matrix in the preparation of isolated lysosomes, where free and nonsedimented activity of acid phosphatase was estimated. The phenomenon depended on concentration of histones in a mixture. Histones apparently destructed the lysosomal membranes at concentration of 100 mg/ml and higher, since the level of free and nonsedimented activity of acid phosphatase approximated and even exceeded the enzymatic activity after treatment of the lysosomes with 0.2% Triton X-100. These properties of histones depended on their macromolecular structure. Histones did not affect the activity of the solubilized enzyme.

Are Changes in the Capillary Zone Electrophoresis Profile in the Beta-Gamma Region Reliable Indicators of a Monoclonal Component in the Tested Serum?

RisinCapillary zone electrophoresis (CZE) has significant advantages in analysis of major protein fractions in the serum: it is fully automated, has short analytical time, high resolution, micro-volume sampling, and low reagent and labor costs.Because of this it is becoming a method of choice in the current laboratory practice.At the same time this method has limitations in detecting small monoclonal immunoglobulin (MI) components resulting from uncertainty in interpreting minor curve distortions in the betagamma region.The goal of this project is to analyze the predictive power of various curve distortions.This is a retrospective study based on analysis of paired serum CZE reports describing different types of curve distortions, raising suspicion of MI, and immunofixation electrophoresis (IFE) reports.Total 457 paired reports were analyzed, IFE confirmed presence of a MI in 206 cases (45%): 103 (50%) showed a minor gamma peak on CZE, 22 (11%) -a minor gamma peak and gamma curve asymmetry, 26 (12.6%) -gamma curve distortion only; 19 (9%) -elevation of the beta-2 peak; 20 (10%) -elevation of the beta-2 peak and gamma curve asymmetry; 1 (0.5%) -elevation of the alpha-2 peak; 10 (5% ) flattening of the gamma part of the curve (hypogammaglobulinenmia) with no distortion; 3 (1.5%)elevation of the gamma part of curve (hypergammaglobulinemia) with no distortion.Presented result emphasize the importance of considering all the clinical information in addition to the CZE profiles in ordering IFE testing to avoid both missing of small MI components and overutilization of the IFE testing.

Exploring New Ways to Deliver Value to Healthcare Organizations: Algorithmic Testing, Data Integration, and Diagnostic E-consult Service.

As the USA Health Care System undergoes transformation and transitions to value-based models it is critical for laboratory medicine/clinical pathology physicians to explore opportunities and find new ways to deliver value, become an integral part of the healthcare team. This is also essential for ensuring financial health and stability of the profession when the payment paradigm changes from fee-for-service to fee-for-performance. About 5 years ago we started searching for ways to achieve this goal. Among other approaches, the search included addressing the laboratory work-ups for specialists' referrals in the HarrisHealth System, a major safety net health care organization serving mostly indigent and underserved population of Harris County, TX. We present here our experience in improving the efficiency of laboratory testing for the referral process and in building a prototype of a diagnostic e-consult service using rheumatologic diseases as a starting point. The service incorporates algorithmic testing, integration of clinical, laboratory and imaging data, issuing structured comprehensive consultation reports, incorporating all the relevant information, and maintaining personal contacts and an e-line of communications with the primary providers and referral center personnel. Ongoing survey of providers affords testimony of service value in terms of facilitating their work and increasing productivity. Analysis of the cost effectiveness and of other value indicators is currently underway. We also discuss our pioneering experience in building pathology residents and fellows training in integrated diagnostic consulting service.
